Intrakorneale Ringsegmente zur Myopiekorrektur. Ergebnisse der Kleinschnitttechnik Zusammenfassung Hintergrund und Fragestellung. Seit Mai 1998 ist zur Implantation der intrastromalen kornealen Ringsegmente (ICRS) ein Schmalschnittinstrumentarium verfügbar, das für die korneale Inzision nur noch 1,2 mm statt 1,8 mm benötigt.Es erfolgt ein Vergleich der Ergebnisse. Patienten/Methodik. Bei 29 Patienten wurden 43 ICRS in konventioneller Technik, nachfolgend 30 ICRS bei 22 Patienten in Schmalschnitttechnik implantiert.Die durchschnittliche Ausgangsmyopie betrug –3,5 dpt. Ergebnisse. Nach sechs Monaten haben 66,7% der Patienten beider Gruppen ihre Zielrefraktion ±0,5 dpt erreicht.Bereits am ersten postoperativen Tag wird die Endrefraktion nahezu erreicht und ist über ein Jahr hinweg stabil, ohne eine Regression aufzuweisen.Nach sechs Monaten weisen 80,0% der konventionell operierten Patienten bzw. 77,8% der Patienten der Schmalschnittgruppe unkorrigiert einen Visus von 1,0 oder besser auf.Der Unterschied ist statistisch nicht signifikant.Die Präparation des kornealen Dissektionskanals in zwei Drittel Hornhautstromatiefe ist mit der Schmalschnitttechnik schonender und für den Operateur sicherer durchführbar.Perforationen nach intern oder extern sind nahezu ausgeschlossen.Die kleinere Wunde benötigt im Gegensatz zur konventionellen Technik keinen Nahtverschluss. In beiden Gruppen betrug die Implantationstiefe mindestens 50%, Dezentrierungen von 1 mm und mehr traten in keiner Gruppe auf. Schlussfolgerung. Die Kleinschnitttechnik erhöht die intraoperative Sicherheit bei der Dissektion der stromalen Lamellen in der richtigen Ebene.Beide Techniken sind in ihren Ergebnissen bezüglich Visus und Refraktion vergleichbar.Die kleinere Hornhautwunde erspart die Naht.
Intracorneal ring segments for the correction of myopia. Results of the small incision technique Abstract Background and objective. Since May 1998, small incision instruments for the implantation of intrastromal corneal ring segments (ICRS) are available enabling incisions only 1.2 mm in width instead of 1.8 mm to be made.After a follow-up period of 22 months in the conventional group and 8 months in the small incision group, the results of both groups were compared. Patients and methods. Since April 1997, 73 intrastromal corneal ring segments have been implanted, the first 43 ICRS implantations in 29 patients were performed using the conventional technique, the next 30 ICRS implantations in 22 patients were performed with the small incision technique.The average preoperative myopia was –3.5 D. Results. After 6 months, 66.7% of the patients of both groups achieved the refraction target ±0.5 D. Even on the first postoperative day, the final refraction is achieved and shows stability for 1 year without regression. After 6 months, 80% of the conventionally operated patients and 77.8% of the patients operated on with the small incision technique showed an uncorrected visual acuity of 1.0 or better.With the small incision technique the preparation of the corneal dissection channel in 2/3 corneal stromal depth is significantly easier and can be carried out more safely.. Perforations towards the inner or outer surface of the cornea can be practically excluded. Due to the smaller superficial wound in contrast to the conventional technique, a corneal suture is not necessary. Stromal depth of implantation was more than 50% and in both groups no decentration of 1.0 mm or more occurred. Conclusions. The small incision technique increases the intraoperative safety in the dissection of the stromal lamellae in the correct level.Both techniques are comparable with regards to visual acuity and refraction.The smaller corneal wound does not need a corneal suture.Because of these advantages we have completely switched our technique to the small incisions.

Optimierung der Laserparameter für die intrastromale Schnittführung mittels ultrakurzer Laserpulse Zusammenfassung Hintergrund. Mithilfe ultrakurzer Laserpulse lassen sich intrastromal äußerst präzise Schnitte mit Schnittbreiten von wenigen Mikrometern erzeugen.Der Vorteil von fs-Pulsen gegenüber längeren Pulsen liegt darin, dass die benötigten Energien beträchtlich geringer sind und somit auch die thermischen und mechanischen Nebeneffekte sowie die Strahlenbelastung der Retina deutlich schwächer in Erscheinung treten. Methode. Mit einem Titan-Saphir-Lasersystem wurde die Schneidwirkung von fsLaserpulsen bei 780 nm Wellenlänge und 200 fs Pulsdauer an Hornhautstroma untersucht.Durch licht- und elektronenmikroskopische Analyse der bestrahlten Proben wurden Schnittqualität und Reproduzierbarkeit sowie erreichbare Genauigkeiten ermittelt. Das Ausmaß der mechanischen Belastung des umliegenden Gewebes durch fs-Photodisruption konnte durch Auswertung von Druckmessungen mittels geeigneter Druckaufnehmer und Schattenfotografien abgeschätzt werden. Schlussfolgerung. Das thermische und mechanische Schädigungspotenzial der fs-LaserKeratomileusis ist vergleichbar mit dem bei Einsatz eines Excimerlasers und lässt daher dieses Verfahren als sehr geeignet für die Behandlung von Fehlsichtigkeiten erscheinen.

Abstracts Optimisation of the laser parameters for intrastromal incisions using ultrashort laser pulses Abstract Background. With the assistance of ultrashort laser pulses (ca.200 fs pulse duration) it is possible to perform precise incisions inside the corneal stroma with a width of a few microns.The advantage of ultrashort pulses is that the required energy of a few microjoules is more than an order of magnitude lower compared with longer pulse durations, i.e.ps or ns pulses.Therefore, the secondary effects, such as thermal and mechanical damage to the surrounding tissue and the amount of radiation reaching the retina, are reduced.This method of intrastromal photodisruption allows a very well defined deposition of energy within the laser focus inside the corneal stroma, accompanied by minimal collateral damage. Methods. The possibilities of performing intrastromal cuts using fs-laser pulses at a wavelength of 780 nm and pulse durations of 200 fs were studied using a titanium-sapphire laser system.The treated tissue samples were analysed by light and scanning electron microscopy to determine incision quality, reproducibility and achievable accuracy.The mechanical side effects of fs-photodisruption inside the surrounding tissue were analysed by pressure measurements using pyroelectric transducers. Conclusion. The thermal and mechanical side-effects of this method are very low and comparable to the effects during excimer treatment.Therefore an application of ultrashort laser pulses in refractive surgery appears to be a feasible alternative.

Nachbetreuung von Trabekulektomien. Komplikationen und therapeutische Konsequenzen Zusammenfassung Fragestellung. Die intensive Nachbetreuung nach Glaukomoperation ist unerlässlich für einen langfristigen Erfolg.Hierbei müssen postoperative Komplikationen erkannt und adäquat behandelt werden. Patienten und Methoden. Im Rahmen einer retrospektiven Studie wurden 113 Augen von 113 konsekutiven Patienten nach Trabekulektomie bezüglich postoperativer Komplikationen und therapeutischer Konsequenzen ausgewertet.Der Nachbeobachtungszeitraum betrug 6 Monate.Als Erfolgskriterium wurde ein intraokularer Druck (IOD) <21 mmHg ohne drucksenkende Medikation sowie eine IOD-Senkung >20% im Vergleich zum präoperativen behandelten IOD definiert. Ergebnisse. Die meisten Komplikationen traten innerhalb der ersten 3 Wochen auf: beginnende Sickerkissenvernarbung (47,6%), Aderhautamotio (18,6%),Vorderkammerabflachung (8%), Fibrinreaktion (5,3%), Überfiltration (4,4%), verminderter Abfluß unter dem Skleradeckel (4,4%), Hornhautdelle (3,5%), Irisprolaps (3,5%), äußere Fistulation (2,7%), Steroideffekt (1,8%), Ziliarkörperabhebung (1,8%) und malignes Glaukom (0,9%). Postoperativ benötigten 45,1% der Augen zusätzliche invasive therapeutische Maßnahmen (5-Fluorouracil (5-FU)-Injektionen 28,3%, Needling + 5-FU-Injektionen 14,2%,Öffnung eines Skleradeckelfaden mit dem Laser 6,2%; Irisreposition,YAG-Laser-Iridotomie, Sickerkissen-Laserkoagulation, Eigenblutinjektion, Zyklokryokoagulation beschränkten sich auf Einzelfälle).36,3% erhielten konservative therapeutische Maßnahmen (z.B. Änderung der Applikationshäufigkeit von Steroiden, intensivierte Zykloplegie).Nach 6 Monaten betrug die Erfolgsquote ohne Anwendung von Glaukommedikamenten 88,5%. Schlussfolgerung. Frühzeitiges Erkennen postoperativer Komplikationen,insbesondere bei beginnender Sickerkissenvernarbung, ermöglicht rechtzeitige Therapieentscheidungen, die bei etwa der Hälfte der Patienten notwendig sind.Dadurch lässt sich eine wesentlich höhere Erfolgsquote erreichen.
Postoperative complications and therapeutic consequences after trabeculectomy Abstract Background Intensive follow-up after trabeculectomy is important for long-term success.In the postoperative period, complications must be detected early in order to initiate adequate therapeutic measures. Patients and methods. In a retrospective study 113 eyes from 113 consecutive patients were followed-up for a period of 6 months after trabeculectomy with regard to postoperative complications and therapeutic decisions (invasive and conservative).Criteria of success were defined as an intraocular pressure (IOP) <21 mmHg without antiglaucomatous treatment as well as an IOP reduction >20% compared to the preoperatively treated IOP. Results. Most of the postoperative complications occurred within the first 3 weeks: scarring of the filtering bleb (47.6%),choroidal detachment (18.6%),shallowing of the anterior chamber (8%),fibrinous reaction (5.3%), overfiltration (4.4%),reduced flow under the scleral flap (4.4%),corneal dellen (3.5%),prolapse of the iris (3.5%),external fistulation (2.7%),increase of the IOP due to steroids (1.8%),detachment of the ciliary body (1.8%) and malignant glaucoma (0.9%).In the postoperative period,45.1% of the eyes needed further interventions such as 5-fluorouracil (5-FU) injections (28.3%),needling and 5-FUinjections (14.2%),laser suturolysis (6.2%).Repositioning of the iris,YAG-laser iridotomy,laser treatment of the filtering bleb,autologous blood injection and cyclocryoapplication were needed only in rare cases and 36.3% received other conservative treatment (i.e.modification of topical steroids,increase of cycloplegic treatment).After 6 months the success rate without antiglaucoma medication was 88.5%. Conclusions. An intensive follow-up after trabeculectomy is mandatory to avoid postoperative failure.In about half of the patients postoperative additional therapeutic measures became necessary,particularly if scarring of the filtering bleb occurred.Paying attention to these factors yields in a high success rate after filtering surgery.

Ergebnisse nach Implantation einer Worst Iris Claw Lens zur Korrektur der hohen Myopie Zusammenfassung Fragestellung. Ziel dieser prospektiven klinischen Studie war die Erfassung der Refraktion, des korrigierten Visus und des Vorderkammerreizzustandes nach Implantation einer Worst Iris Claw Lens zur Korrektur der hohen Myopie. Patienten und Methode. Von Dezember 1996 bis Februar 1999 wurde bei insgesamt 44 phaken Augen von 28 Patienten eine Worst Iris Claw Lens implantiert.Es folgte eine Nachbeobachtungszeit von 6 Wochen bzw.6 Monaten. Ergebnisse. Das mittlere präoperative sphärische Äquivalent betrug –17,35±3,57 dpt. Sechs Monate postoperativ lag die Refraktion bei 79% der Augen im Intervall von ±1,0 dpt von der Zielrefraktion entfernt.Der mittlere korrigierte Visus stieg von präoperativ 0,43 auf postoperativ 0,65 an.Die postoperative Lasertyndallometrie ergab nach 6 Monaten bei 21% der Augen keinen nachweisbaren Vorderkammerreiz.Bei 76% aller Augen konnte ein milder Vorderkammerreizzustand festgestellt werden.Der mediane Flarewert lag bei 20 p/ms. Schlussfolgerung. Die Implantation einer Worst Iris Claw Lens stellt eine präzise Methode zur Korrektur der hohen Myopie dar.Die postoperativen Ergebnisse zeigten bei fast allen Augen einen guten Visus und nur einen geringen Vorderkammerreizzustand.Langzeitbeobachtungen stehen bisher jedoch noch aus.
Results of implantation of a Worst Iris Claw lens for correction of advanced myopia Abstract Background. The purpose of this clinical study was to evaluate refraction, best visual acuity and anterior chamber flare after implantation of a Worst Iris Claw Lens to correct high myopia. Patients and methods. From September 1996 to February 1999 the implantation of a Worst Iris Claw lens was performed on 44 highly myopic phakic eyes of 28 patients. Data after 6 weeks and 6 months follow-up were assessed. Results. The mean preoperative spherical equivalent was –17.35±3.57 dpt and 6 months after implantation 79% of the eyes were within ±1.0 dpt of target refraction. The mean best corrected visual acuity increased from 0.43 preoperatively to 0.65 postoperatively and 6 months after the treatment there was no flare (<10 photons/ms) in 21% and a mild anterior chamber flare (10–80 photons/ms) in 76% of the treated eyes.The median laser flare was 20 p/ms. Conclusions. In conclusion, implantation of a Worst Iris Claw lens has proved to be a precise method to correct high myopia.No or mild anterior chamber flare and a good visual acuity were measured in most eyes 6 weeks and 6 months after surgery.Longer follow-up periods are needed to assess the long-term safety and efficiency.

Morphologische Ergebnisse nach exzentrischer perforierender Keratoplastik Zusammenfassung Hintergrund und Ziele. Ziel der Studie war es, morphologische Erfolge nach geplant exzentrischer homologer perforierender Keratoplastik (PK) in Abhängigkeit von der Diagnose sowie Transplantatdurchmesser und -lage zu untersuchen. Patienten und Methoden. In unserer retrospektiven Studie wurden 122 exzentrische PK an 96 Augen ausgewertet, die bei 104 Patienten wegen eines Ulcus corneae, 6 mit Narben, 7 mit ektatischen Hornhauterkrankungen und 5 sonstigen durchgeführt wurden.Folgende Parameter wurden bei 84 Augen quantifiziert: Transplantatdurchmesser (TDM) und -transparenz, Abstand Hornhautmitte von der zentralen Trepanationskante (ABS) sowie von der Transplantatmitte (Dezentrierung, DEZ).Nach Lage des Transplantates (TPL) in Bezug zur optischen Achse wurden n=17 in „exzentrisch periphere“ (EPK) und n=67 in „exzentrisch zentrale“ Keratoplastik (EZK) unterteilt. Ergebnisse. Der mittlere Nachbeobachtungszeitraum betrug 25 Monate.Bei EPK war der TDM signifikant kleiner als bei EZK. Die DEZ war bei EPK signifikant größer als bei EZK.Der ABS betrug bei EZK –2,0±1,0 mm, bei EPK +1,3±0,9 mm.Bei EPK/EZK waren 46%/57% kristallklar, 18%/15% leicht trüb und 36%/28% vollständig eingetrübt.Bei Ulkus/Narben/Ektasien/Sonstige waren 45%/75%/100%/75% kristallklar, 18%/25%/0%/0% leicht trüb und 37%/0%/0%/25% vollständig eingetrübt. Mit zunehmendem Grad der Eintrübung nahm die DEZ von 1,4±1,2 mm (kristallklar)

Abstracts über 1,7±1,3 mm (leicht trüb) auf 2,2±1,3 mm (vollständig trüb) zu.Transplantate mit größerem TDM und ABS waren signifikant häufiger klar.Ulzera waren hochsignifikant häufiger trüb als Narben/Ektasien. Die Lage des TPL und dessen Klarheit waren nicht signifikant miteinander korreliert. Schlussfolgerungen. Bei exzentrischer perforierender homologer Keratoplastik ist die Grunderkrankung prognostisch bedeutsam für die Transparenz des Transplantates.Eine zunehmende Exzentrizität des Transplantates scheint sich nicht zwangsläufig ungünstig auf die Klarheit auszuwirken.Im reizfreien Intervall nach exzentrischer tektonischer Minikeratoplastik kann eine zentrierte perforierende Re-Keratoplastik erwogen werden. Morphological results after eccentric penetrating keratoplasty Abstract Objective. Due to unfavourable optical and immunological conditions, an eccentric position of the corneal transplant is limited to curative or tectonic indications.The purpose of this study was to assess the morphological results of eccentric homologous penetrating keratoplasty (PK) with respect to diagnosis as well as position and size of the corneal graft. Patients and methods. In our retrospective study, 122 cases of eccentric homologous PK in 96 eyes between 01/1989 and 10/1997 were included, which in 104 patients was necessary because of a corneal ulcer (36 previous PK, 21 areactive/rheumatic, 20 herpetic, 15 bacterial, 12 other).Elective PK was performed on 6 eyes with corneal scars, 7 eyes with ectatic corneal diseases (ECT) and 5 others.Using postoperative slides the following parameters were quantified: the graft diameter (GD) and transparency, the

distance of the geometric centre of the cornea from the central trephination margin (ABS) as well as from the geometric centre of the graft (decentration, DEZ).Of these, 17 grafts were classified as eccentric peripheral keratoplasty (EPK) with the optical axis through the host cornea and 67 as eccentric central keratoplasty (ECK) with the optical axis through the graft. Results. The average follow-up period was 25 months.In EPK (ulcers only, 5.0±1.7 mm) the mean GD was significantly lower than in ECK (78% ulcers, 7.0±1.3 mm) (p<0.001). The decentration of EPK (3.5±1.1 mm) was significantly higher than for ECK (1.4±0.8 mm, p<0.001).ABS of EPK was +1.3±0.9 mm and for ECK was –2.0±1.0 mm.Of the grafts in EPK and ECK 46%/57%, respectively were crystal clear, 18%/15% respectively showed minor decompensation and 36%/28% were completely cloudy.In cases of ulcers/scars/ECT/others, 45%/75%/100%/75% were crystal clear, respectively, 18%/25%/0%/0% showed minor decompensation and 37%/0%/0%/25% were completely cloudy.With increasing stages of cloudiness, the mean graft decentration increased from 1.4±1.1 mm (crystal clear) to 1.7±1.2 mm (minor decompensation) and 2.2±1.4 mm (completely cloudy).Transplants with greater GD (p=0,04) and ABS (p=0,01) were significantly more often clear. Ulcers were significantly more cloudy than scars/ECT (p<0,01).The position of the graft and its cloudiness was not significantly correlated. Conclusion. The diagnosis leading to eccentric PK seems to have the greatest impact on long-term graft transparency.Our results indicate that an increasing decentration of the graft is not inevitably associated with an increasing probability of clouding.A central penetrating repeat keratoplasty may be considered in the non-inflamed interval after eccentric tectonic mini-keratoplasty.

Immunsuppressive Therapie mit Mycophenolat Mofetil (CellCept®) in der Behandlung der Uveitis Zusammenfassung Fragestellung. Ziel der vorliegenden multizentrischen Studie war, die Wirksamkeit von Mycophenolat Mofetil (MMF) als Monotherapeutikum bei verschiedenen Formen der Uveitis zu untersuchen. Patienten und Methode. Zehn Patienten mit anteriorer Uveitis (n=3), intermediärer Uveitis (n=2), Panuveitis (n=4) und retinaler Vaskulitis (n=1) wurden in einer prospektiven Studie mit 2×1 g MMF/Tag therapiert. Ergebnisse. Der Behandlungszeitraum betrug 1–12 Monate.Bei 8 Patienten zeigte sich unter der MMF-Therapie kein weiteres Rezidiv.Zwei Patientinnen wurden erst durch Umstellung auf Methotrexat bzw. durch eine begleitende Steroidmedikation rezidivfrei.An Nebenwirkungen klagte ein Patient über Diarrhö, die zum Absetzen der Therapie führte.Bei einer Patientin kam es 10 Monate nach Therapiebeginn zu Übelkeit, Erbrechen und Haarausfall. Schlussfolgerung. Die Immunsuppression mit MMF hat bei 8 von 10 Uveitispatienten zu einer Beruhigung der Entzündungsaktivität geführt.Die Nebenwirkungen sind eher milde verglichen mit denen anderer Immunsuppressiva.
Immunosuppressive treatment with Mycophenolate mofetil (CellCept) in the therapy of uveitis Abstract Background. Severe forms of uveitis can often only be managed sufficiently with systemic immunosuppression.All available drugs are known for their relative high rate of side-effects.Mycophenolate mofetil (MMF), an immunosuppressant successfully used in management after organ transplantation and many autoimmune diseases, has shown remarkably less side-effects when used for various forms of uveitis in monotherapy or in combination with corticosteroids.The aim of this multicenter-study was to investigate if monotherapy with MMF is effective in various forms of uveitis. Method and patients. Ten patients with anterior uveitis (n=3), intermediate uveitis (n=2), panuveitis (n=4) and retinal vasculitis (n=1) were treated in a prospective study with 2×1 g MMF daily.Previous immunosuppression had been discontinued because of side-effects or ineffectivity in all patients.In these patients MMF was given in addition to the other immunosuppressant at the beginning of treatment. Results. The follow-up time ranged from 1 to 12 months (mean 4.5 months).Under therapy with MMF (monotherapy in 4 patients, additional prednisolone in 5 patients and additional metotrexate in 1 patient) 8 patients remained free of recurrences.In one female patient depression of inflammation activity was only achieved after cessation of therapy with Cyclosporin A in combination with MMF and a switch to methotrexate.Another patient with a bilateral uveitis was free of recurrences in only one eye, the second eye did not develop recurrence due to the additional corticosteroid treatment.Sideeffects were diarrhoea in one patient and
probably gastrointestinal problems in another (leading to cessation of therapy in both patients) and in another case nausea, vomitus and alopecia 10 months after beginning therapy. Conclusions. MMF as a new immunosuppressant stopped inflammation or drastically reduced the rate of recurrences in 8 out of 10 patients with uveitis which was previously not brought under control by other immunosuppressants.The side-effects were tolerable in comparison with other immunosuppressive agents.More patients, longer follow-up times and a comparative study with Cyclosporin A are required to assess the long-term therapeutical success.

Sind diabetische Erkrankungen beim älteren Patienten ein Kataraktriskofaktor? Zusammenfassung Die höhere Prävalenz des Diabetes mellitus für Katarakt im Rahmen der multifaktoriellen Genese der altersassoziierten Linsentrübungen ist in verschiedenen epidemiologischen Studien nachgewiesen worden.Überwiegend kommen sie zu dem Ergebnis,dass diese Stoffwechselerkrankung ein hochsignifikanter Risikofaktor für die Entstehung der Alterskatarakt in beiden Geschlechtern darstellt.Das höhere Kataraktrisiko der Diabetiker gegenüber Nichtdiabetikern nimmt nach dem 65.Lebensjahr wieder ab und betrifft morphologisch nicht einheitliche Kataraktformen.Neben dem osmotischen Mechanismus kommt es bei der diabetischen Hyperglykämie infolge der nichtenzymatischen Glykosylierung zu einem verstärkten Anfall von zellschädigenden Endprodukten.Im Rahmen dieser speziellen Pathogenese wird von mehreren Autoren die besondere Rolle des Linsenepithels bei der Kataraktogenese her-

Are diabetic diseases a risk factor for cataracts in older patients? Abstract The higher prevalence of diabetes for cataracts as a component of the multifactorial genesis of age-related cataracts has been proven in several epidemiological studies. Most of these results identified diabetes as a highly significant risk factor for cataracts in older diabetic males and females.The increased cataract risk of diabetics then decreases after the age of 65 years and is related to different types of opacity.In addition to the sorbitol pathway, diabetic hyperglycaemia causes increased formation of glucose-derived advanced glycation cytotoxic end-products.In connection with these specific pathological changes, the crucial role of the lens epithelium with respect to cataractogenesis is emphasised in several studies.The results of our own prospective clinical study proved the significantly lower mean cell density in type II diabetics compared with eyes from non-diabetics.Together with the other determined morphological cell characteristics, this result seems to be due to the cataractogenic influence of diabetic metabolic disorders on the lens There are some indications for the primary cataractogenic importance of the lens epithelium in type II diabetes.

Ultraschallbiomikroskopische Diagnose einer traumatischen Choroidaleffusion ohne Zyklodialyse Zusammenfassung Hintergrund. Traumatisch bedingte Kammerwinkelveränderungen stellen eine diagnostische Herausforderung in der modernen Ophthalmologie dar.Anhand dieses Fallberichtes sollen zwei Untersuchungstechniken einander gegenübergestellt werden: die seit bald einem Jahrhundert verwendete und stetig weiterentwickelte Gonioskopie und die 1991 erstmals bei klinischen Fragestellungen angewandte Ulltraschallbiomikroskopie. Kasuistik. Wir berichten über einen 14-jährigen Patienten, der eine Bulbuskontusion durch ein Soft-Gun-Geschoss erlitten hatte. Gonioskopisch wurde bei okulärer Hypotension und breiter Goniosynechie der Verdacht auf eine Zyklodialyse gelenkt.Die bei aufgehobener Vorderkammer ultraschallbiomikroskopisch durchgeführten Verlaufskontrollen zeigten jedoch hinter der Synechie einen intakten und zirkulär anliegenden Ziliarkörper, eine zirkuläre Choroidaleffusion, eine Anteversion des Ziliarkörpers und ein nach vorne verlagertes Iris-Linsen-Diaphragma. Schlussfolgerung. Bei einigen Krankheitsbildern, v.a.aber beim Bulbustrauma, ist die Ultraschallbiomikroskopie der Gonioskopie klar überlegen.Daher sollte die Ultraschallbiomikroskopie bei der Beurteilung traumatischer Bulbuspathologien nicht nur eine wertvolle Ergänzung zur Gonioskopie darstellen, sondern zunehmend auch eine tragende Rolle einnehmen.
Ultrasound biomicroscopical diagnosis of traumatic choroidal effusion without cyclodialysis Abstract Background. Chamber angle changes due to trauma represent a diagnostic challenge in modern ophthalmology and two examination techniques are compared: gonioscopy which has been used in ophthalmology for almost a century and is still undergoing continuous improvements and ultrasound biomicroscopy (UBM) which was introduced into clinical ophthalmology in 1991. Case report. We report the case of a 14-year-old boy with ocular trauma caused by a soft gun projectile.Gonioscopy showed a large goniosynechia in the presence of ocular hypotension, therefore, cyclodialysis was suspected.However, a control investigation using UBM showed an intact and circularly attached but anteverted ciliary body behind the synechia, a circular choroidal effusion and an anterior displacement of the iris-lens diaphragm. Conclusion. In ocular trauma, UBM may under certain conditions clearly be of a higher diagnostic value than gonioscopy.Therefore, UBM should not only be considered as an additional examination technique in the evaluation of traumatic ocular pathologies but rather as the technique of choice.

Differenzierung zwischen Legasthenie und okulär bedingten Lesestörungen Zusammenfassung Hintergrund. Legasthenie wird definiert als eine Lese-Rechtschreib-Störung (LRS), die nach Ausschluss und Therapie organischer Ursachen persistiert.Studien belegen, dass bei als Legastheniker eingestuften Patienten mit Lese-/Schreibstörung okuläre Ursachen, v.a.kleine Refraktionsfehler und Motilitätsstörungen, oft nicht vollständig ausgeschlossen und behandelt wurden.

Methode. Retrospektiv werteten wir die Daten von 33 Patienten aus, die in unsere Abteilung mit der Diagnose der Legasthenie im Zeitraum von Dezember 1997 bis März 2000 überwiesen wurden. Ergebnisse. Bei 28 (84,8%) Patienten konnten wir okuläre Störungen finden und bei 26 (78,8%) Kindern verbesserte sich die LRS nach Therapie dieser Störungen.Bei diesen 26 Kindern lagen vor allem Akkommodationsprobleme vor: unkorrigierte kleine Hyperopien, Hypoakkommodation und/oder Exophorie mit Kompensation über die akkommodative Konvergenz. Schlussfolgerung. Unsere Ergebnisse zeigen, dass die Korrektur von sehr kleinen Refraktionsfehlern und/oder Motilitätsstörungen bei Kindern mit LRS äußerst wichtig ist. Eine Legasthenie konnte dabei nur in einem sehr kleinen Prozentsatz bestätigt werden. Differentiation between dyslexia and reading disorder due to ocular causes Abstract Background. Dyslexia is defined as a reading and/or writing disability persisting after exclusion of organic causes.Studies show that ocular disorders, especially small refraction errors, hypoaccommodation and symptomatic heterophoria, are often not detected or treated in cases of reading and/or writing problems which were otherwise diagnosed as dyslexia. Methods. We evaluated the data of patients referred to our department from December 1997 to March 2000 with the diagnosis of dyslexia. Results. We found ocular disturbances in 28 (84.8%) out of 33 children, 26 (78.8%) showed improved reading after therapy. They had mostly accommodative problems: uncorrected hyperopia, hypoaccommodation and/or exophoria compensated by accommodative convergence (pathophoria). Conclusions. Our results underline the importance of the correction of even small refraction and/or motility errors in the presence of reading and writing difficulties.
