Abstract Purpose. The aim of this paper is to illustrate imaging features of patients affected by congenital aural atresia (CAA) before and after treatment with a Vibrant SoundBridge (VSB) device implanted on the round window. Materials and methods. Ten patients (5 males and 5 females; mean age 22.1 years) with CAA underwent preoperative high-resolution computed tomography (HRCT) to estimate the degree of involvement of the middle- and inner-ear structures and highlight radiological landmarks useful for surgical planning. Results. Bilateral CAA, mostly of the mixed type, was present in 7 patients and ossicular chain abnormalities in 16 ears (94% of cases). The round window region was normal in all patients, whereas facial-nerve course and/or caliber abnormalities were present in 6 ears (35.3%). The tympanic cavity was small in 13 ears (76.5%), whereas the mastoid was well pneumatized in 8/17 (47%). Conclusions. HRCT provides accurate information about anatomy and malformations of the middle and inner ear and can thus assist the surgeon in planning the procedure. Riassunto Obiettivo. Lo scopo di questo studio è di descrivere le caratteristiche radiologiche pre-operatorie e postoperatorie dei pazienti affetti da atresia congenita aurale (ACA) impiantati mediante Vibrant SoundBridge (VSB) alla finestra rotonda. Materiali e metodi. Dieci pazienti (5 maschi e 5 femmine; età media: 22,1 anni) con ACA sono stati sottoposti ad un esame di tomografia computerizzata (TC) ad alta risoluzione per stimare il grado di coinvolgimento delle strutture dell’orecchio medio ed interno ed evidenziare utili punti di riferimento per l’intervento. Risultati. Sono state riscontrate atresia bilaterale in 7 pazienti, prevalentemente di tipo misto, ed anomalie della catena ossiculare in 16 orecchi (94% dei casi). La finestra rotonda era nei limiti in tutti i soggetti, mentre in 6 orecchi (35,3%) erano presenti anomalie di decorso e/o di calibro del nervo faciale. La cassa timpanica era ipoplasica in 13 orecchi (76,5%), mentre la mastoide era ben pneumatizzata in 8/17 (47%). Conclusioni. La TC ad alta risoluzione fornisce le corrette informazioni circa l’anatomia e le malformazioni dell’orecchio medio ed interno, utili per il chirurgo per la programmazione operatoria. Introduction

Introduzione

Congenital aural atresia (CAA) is a birth defect characterized by conductive hearing loss due to the presence of varying degrees of external auditory canal (EAC) deformity. On assessing the patient for surgical repair, it is crucial to know whether the malformation involves the EAC and auricle only or whether there is diffuse involvement of the temporal bone and, especially, of inner ear structures [1, 2]. Because the pathogenic insult may occur at different stages of embryological development, not all atretic ears are the same [3]. The presence of an associated sensorineural hearing loss due to coexisting inner ear abnormalities constitutes a contraindication to conventional surgery [4]. In the event that the inner ear is intact, different approaches have been proposed, including bone-conducting hearing aids, boneanchored hearing aids and reconstructive surgery. In view of the high rate of complications [5–7] and the need for surgical revision in approximately one third of cases of reconstructive surgery [8–10] there was a need for an alternative treatment, specifically designed for children, to prevent impairment of normal cognitive and verbal development. With the innovative surgical implantation of a Vibrant SoundBridge (VSB, Med-EL, Innsbruck, Austria) floating mass transducer (FMT) on the round window, the malformed EAC and the damaged ossicular chain are bypassed, and the implant delivers its energy directly onto the cochlea [11, 12]. The excellent clinical results of this approach indicate that it may represent a viable alternative to conventional surgery [13, 14]. CAA diagnosis and proper pre-operative assessment rely on clinical and functional examinations (history, clinical examination, audiogram, electrophysiological tests) and radiological investigations, such as computed tomography (CT) and magnetic resonance (MR) imaging. CT scan of the temporal bone is crucial to establish the indication for surgical intervention in that it provides information regarding external-, middle- and inner-ear anatomy and reveals the type of bone dysplasia and associated pathologies, if present [2]. Post-operative radiological investigations are mandatory both within hours after surgery to exclude early complications, and at different intervals to highlight any implant malposition/displacement. The aims of this study was to identify anatomical landmarks and middle ear structures to be visualized on CT to achieve proper pre-operative assessment of patients with CAA who are eligible for VSB implantation on the round window.

L’atresia congenita aurale (ACA) è una condizione malformativa congenita caratterizzata da sordità trasmissiva per la presenza di diversi gradi di anomalia del condotto uditivo esterno (CUE). Nel momento in cui il paziente viene valutato per una correzione chirurgica, è fondamentale sapere se la malformazione coinvolge solo il CUE ed il padiglione auricolare o se è presente un diffuso interessamento dell’osso temporale, in particolare delle strutture dell’orecchio medio [1, 2]. Infatti, poiché l’insulto patogeno può colpire in differenti fasi dello sviluppo embriogenetico, non tutti gli orecchi atresici sono uguali [3]. La presenza di un’associata ipoacusia neurosensoriale per una contemporanea anomalia delle strutture dell’orecchio interno rappresenta una controindicazione all’intervento chirurgico tradizionale [4]. Nel caso in cui l’orecchio interno sia integro, sono stati proposti diversi approcci chirurgici, in particolare apparecchi acustici a conduzione ossea, apparecchi acustici ancorati all’osso ed infine la plastica ricostruttiva. A causa dell’alto tasso di complicanze [5–7] e alla necessità di revisione dell’intervento in circa 1/3 dei casi di chirurgia ricostruttiva [8–10] si sentiva il bisogno di una alternativa, soprattutto indirizzata a pazienti pediatrici, al fine di impedire compromissioni del normale sviluppo cognitivo e verbale. Con l’innovativo intervento chirurgico di posizionamento della massa vibrante (MV) del Vibrant SoundBridge (VSB, Med-EL, Innsbruck, Austria) sulla finestra rotonda il CUE malformato e la catena ossiculare danneggiata vengono superati e la protesi scarica la sua energia direttamente sulla coclea [11, 12]. Gli ottimi risultati clinici di questo approccio lo indicano come vera alternativa alla chirurgia tradizionale [13, 14]. La diagnosi di ACA ed il corretto inquadramento prechirurgico si basano su esami clinici e funzionali (anamnesi, visita clinica, audiometria e test elettrofisiologici) e su indagini radiologiche come la tomografia computerizzata (TC) e la risonanza magnetica (RM). L’esecuzione della TC dell’osso temporale è cruciale per l’indicazione all’intervento chirurgico, fornendo informazioni circa l’anatomia ossea dell’orecchio esterno, medio ed interno per evidenziare il tipo di displasia ossea presente ed eventuali patologie associate [2]. Valutazioni radiologiche post-operatorie sono obbligatorie a poche ore dall’intervento per escludere complicanze chirurgiche precoci e successivamente, a differenti intervalli, per evidenziare eventuali mal posizionamenti/dislocazioni della protesi. Lo scopo del lavoro è stato di identificare i punti di riferimento anatomici e le strutture dell’orecchio medio da descrivere per ottenere il corretto inquadramento pre-operatorio dei pazienti con ACA e candidati al posizionamento di VSB alla finestra rotonda.

Materials and methods We evaluated CT examinations of 10 patients affected by severe CAA [5 males and 5 females; age range 2 months

to 50 years; mean age 22.1 years; standard deviation (SD) 5.1 years] in whom a FMT was placed on the round window between May 2005 and May 2010. All images were retrospectively reviewed by two radiologists with >5 and 10 years of experience, respectively, in ear imaging, and compared with the intra-operative findings. Pre-operatively, all patients underwent high-resolution CT (HRCT) to allow visualization of the external-, middle- and inner-ear anatomy and to determine the degree of dysplasia. The examination was also aimed at depicting any possible associated conditions such as infections or chronic otitis media and possible sequelae from previous surgical repair. In fact, three patients had already undergone tympanoplasty (one to three procedures). Scans were performed with the CT units available at our centre: a CT Brilliance 64 (Philips, Eindhoven, The Netherlands; 140 kV; 150 mA; slice thickness 0.67 mm; spiral technique; pitch 0.32; matrix 512×512) and a CT Brilliance 6 (120 kV; 150 mA; slice thickness 0.8 mm; spiral technique; pitch 0.4; matrix 256×256). In addition, on the same day, the patients had undergone a brain MR scan completed with a study of the ponto-cerebellar angle to exclude malformations or disorders of inner ear structures or central auditory pathways. MR imaging was carried out with a Symphony Maestro 1.5-T scanner (Siemens, Erlangen, Germany) with anatomical T1- and T2-weighted SE (spin echo) and TSE (turbo spin echo) sequences and completed with axial constructive interference in steady-state sequences [CISS; 28 slices; TR 17.3; TE 6.9; thickness 0.65; flip angle 70°; matrix 358×512* (interpolation); NEX 2; FOV 100×100]. Then, maximum intensity projection (MIP) reconstructions were obtained to gain an overall view of the anterior and posterior labyrinth, and parasagittal multiplanar reconstructions (MPR) to visualize nerve structures of the inner ear canal. Post-operatively, all patients underwent radiological assessment to exclude early complications and incorrect placement of the FMT (CT protocol within 8 hours) using CT scan of the skull and petrous bones. Patients were then followed up from 12 to 60 months after surgery. All patients were implanted with a VSB on the round window: this is made up of an external portion consisting of an audio processor with microphone and receiver–transmitter, and an internal portion consisting of the receiver connected via a wire to the FMT, a 2×1.5-mm titanium-coated magnetic coil weighing 25 mg that vibrates when current runs through it. The implant was activated between day 1 and day 4 after surgery. The patients were evaluated and classified according to the following variables: affected side, atresia type, ossicular-chain involvement, facial nerve course, oval and round window patency, mastoid pneumatization and aeration and the presence of vascular abnormalities (Table 1).

Materiali e metodi Sono state valutate le indagini TC espletate in 10 pazienti affetti da severa ACA (5 maschi e 5 femmine, età compresa fra 2 mesi e 50 anni: media 22,1 anni, deviazione standard: 5,1 anni) ed operati con posizionamento di MV alla finestra rotonda nel periodo compreso fra maggio 2005 e maggio 2010. Tutte le immagini sono state analizzate retrospettivamente da due radiologi con esperienza otologica superiore a 5 e 10 anni e confrontate con i reperti intra-operatori. In sede pre-operatoria tutti i dieci pazienti hanno eseguito l’esame TC ad alta risoluzione per ottenere i dettagli anatomici dell’orecchio esterno, medio ed interno e delineare il grado di displasia. Inoltre l’esame doveva anche fornire informazioni su possibili patologie associate, come infezioni o otite media cronica, ed eventuali esiti di precedenti revisioni chirurgiche. Infatti 3 di questi soggetti erano già stati sottoposti a timpanoplastica (da 1 a 3 interventi). Le scansioni sono state espletate mediante le apparecchiature TC a disposizione presso il nostro istituto: TC Brilliance 64 (Philips, Eindhoven, Olanda; 140 kV, 150 mA, spessore dello strato 0,67 mm, tecnica spirale, pitch 0,32, matrice 512×512) e TC Brilliance 6 (120 kV, 150 mA, spessore dello strato 0,8 mm, tecnica spirale, pitch 0,4, matrice 256×256). Nella stessa seduta giornaliera i dieci pazienti avevano eseguito inoltre la RM encefalo completato con studio dell’angolo ponto-cerebellare per escludere malformazioni o patologie delle strutture dell’orecchio interno o delle vie acustiche centrali. L’esame RM è stato eseguito mediante apparecchiatura Symphony Maestro da 1,5 T (Siemens, Enlargen, Germania) con scansioni anatomiche spin echo (SE) e turbo spin echo (TSE) T1 e T2-dipendenti e completato con immagini assiali constructive interference in steady state (CISS; 28 strati, tempo di ripetizione [TR]: 17,3, tempo di eco [TE]: 6,9, spessore 0,65, flip angle 70°, matrice 358×512* (interpolazione), NEX 2, campo di vista [FOV] 100×100). Sono state successivamente ottenute ricostruzioni in proiezione di massima intensità (MIP) per avere una visione globale del labirinto anteriore e posteriore e ricostruzioni multiplanari (MPR) para-sagittali per visualizzare le strutture nervose nel condotto uditivo interno. In sede post-operatoria tutti i pazienti sono stati valutati radiologicamente per escludere complicanze precoci ed il corretto posizionamento della MV (protocollo TC entro 8 ore) mediante esame TC cranio e delle rocche petrose; il follow-up successivo veniva eseguito in un periodo compreso fra i 12 e i 60 mesi dall’operazione. Tutti i pazienti sono stati impiantati con una VSB sulla

Results Pre-operative findings Seven of ten patients (70%) had bilateral atresia (Fig. 1), 2 had unilateral right atresia (20%) and 1 had unilateral left atresia, for a total of 17 atretic ears. In 15/17 ears (88%), atresia was of the mixed type, whereas in only 2 (12%) it was of the isolated membranous type. The ossicular chain appeared rudimentary and dislocated in 16/17 ears (94%). In 10 of 17 (58%) cases, the malleus and incus were fused (Fig. 2). Stapes was not recognizable in 3/17 ears (17.6%), a finding confirmed at surgery, whereas a poorly developed stapes was seen in 8/17 ears (47%). The facial nerve was normal in 11/17 ears (64.7%); in detail, it was dehiscent in the tympanic segment in 3/6 (50%), anteriorly displaced in the mastoid segment in 2/6 (33.3%) and absent or hypoplastic in 1/6 (16.6%) (Fig. 3). The oval window was stenotic and obliterated (Fig. 4) in 5/17 ears (29%), whereas the round window was patent in all atretic ears (100%), which is a prerequisite for surgical treatment. The mastoid appeared well pneumatized in 8/17 ears (47%) but poorly aerated in 2/8 (25%), as it was occupied by hypodense inflammatory tissue. In the remaining 9/17 ears (53%), the mastoid was poorly pneumatized. The tympanic cavity was normal in size in 4/17 ears (23.5%) and hypoplastic in the remaining 13 (76.5%) (Fig. 5). Only one of 17 atretic ears (6%) showed vascular abnormality, which consisted of a small diverticulum of the internal jugular vein (Fig. 3). None of the patients had any alteration in Eustachian-tube dimensions or any innerear abnormalities. Post-operative findings No patient showed evidence of early post-operative complications, in particular, intracranial haemorrhage, at CT performed within 8 hours after the procedure. In all patients (100%), scans focusing on the temporal bone showed small gas bubbles and a moderate quantity of slightly hyperdense tissue, referable to reactive inflammatory tissue mixed with blood, occupying the site of surgery. Follow-up CT at 12– 60 months showed complete restoration of normal aeration of the tympanic cavity in all cases. The FMT was correctly lodged on the round window in all patients at post-operative CT (Fig. 6); there were no cases of implant displacement during follow-up. Discussion In patients with auricle and EAC deformity, the rationale for radiological imaging is based on the need to identify possible associated abnormalities of the middle or inner ear, describe them in detail and classify the atresia as bony,

finestra rotonda: essa consiste di una componente esterna, ossia un processore audio esterno con microfono e ricetrasmittente, e di una componente interna, formata dal ricevitore connesso con un cavo di collegamento alla MV, una bobina magnetica rivestita di titanio con dimensioni di 2×1,5 mm e 25 mg di peso che, se attraversata da corrente, può vibrare. L’impianto è stato attivato dalla prima alla quarta giornata post-operatoria. I pazienti sono stati valutati e classificati in base alle seguenti variabili: lato affetto, tipo di atresia, coinvolgimento della catena ossiculare, decorso del nervo faciale, pervietà della finestra ovale e rotonda, pneumatizzazione ed aerazione della mastoide e presenza di anomalie vascolari (Tabella 1).

Risultati Reperti pre-operatori Sette pazienti/10 (70%) erano affetti da atresia bilaterale (Fig. 1), 2 pazienti/10 (20%) da atresia monolaterale destra e 1 monolaterale sinistra per un totale di 17 orecchi atresici. In 15/17 orecchi (88%) l’atresia era di tipo misto mentre solo 2 (12%) era di tipo membranoso isolato. La catena ossiculare appariva rudimentale e dislocata in 16/17 orecchi (94%). In 10 casi/17 (58%) il martello e l’incudine erano fusi fra loro (Fig. 2). La staffa non era riconoscibile in 3/17 orecchi (17,6%), quadro confermato in sede di intervento, mentre un abbozzo stapediale era riconoscibile in 8/17 orecchi (47%). Il nervo faciale era normale in 11/17 orecchi (64,7%); nei rimanenti era anomalo, in particolare in 3/6 (50%) deiscente nel suo tratto timpanico, in 2/6 (33,3%) dislocato anteriormente nel tratto mastoideo ed assente/ipoplastico in 1/6 (16,6%) (Fig. 3). La finestra ovale era stenotica e obliterata (Fig. 4) in 5/17 orecchi (29%), mentre la finestra rotonda era pervia e nei limiti in tutti gli orecchi atresici (100%), condizione essenziale per effettuare l’intervento. La mastoide appariva ben pneumatizzata in 8 orecchi/17 (47%), tuttavia in 2/8 (25%) risultava scarsamente areata poiché occupata da tessuto ipodenso flogistico per fenomeni suppurativi sovrapposti. Nei rimanenti 9/17 (53%) la mastoide era scarsamente pneumatizzata. La cavità timpanica era normale per dimensioni in 4/17 orecchi (23,5%), mentre risultava ipoplasica nei rimanenti 13 (76,5%) (Fig. 5). In un solo orecchio atresico/17 (6%) si è dimostrata la presenza di un’anomalia vascolare, consistente in un piccolo diverticolo della vena giugulare interna (Fig. 3). Nessun paziente mostrava alterazioni nelle dimensioni della tuba di Eustachio, né anomalie delle strutture dell’orecchio interno.

Reperti post-operatori Nessun paziente ha mostrato la presenza di complicanze precoci post-operatorie, in particolare emorragie intracraniche, mediante TC entro 8 ore come da protocollo. In tutti i soggetti (100%), nelle scansioni focalizzate sull’osso temporale, è stato possibile riconoscere piccole bolle di gas ed una modesta quantità di tessuto leggermente iperdenso, riconducibile a materiale flogistico reattivo misto a sangue, ad occupare la sede dell’intervento. Nelle TC di controllo a 12–60 mesi in tutti casi si è osservato il completo ripristino della normale areazione della neocavità timpanica. La MV era normo-allogata all’interno della finestra rotonda in tutti i 10 pazienti alla TC post-operatoria (Fig. 6); nel follow-up in nessun caso è stata dimostrata una dislocazione della protesi.

membranous or mixed. In patients without EAC deformity, imaging is needed to confirm or rule out the suspicion of CAA [15–17]. CT represents the examination of choice for its intrinsic high-contrast resolution, which well depicts environments such as the external and middle ear, which mostly consist of air and bone [18]. CT can help to diagnose suspected congenital malformations as well as other causes of conductive hearing loss, such as tympanosclerosis and otosclerosis [19], and therefore it is a necessary step in pre-operative planning [20]. Standard radiography of the skull with dedicated otological projections and conventional tomographic studies are not indicated or even useless in the diagnostic assessment of patients with CAA. Compared with CT, MR imaging is unable to provide a detailed representation of external- and

Nei pazienti con malformazione del padiglione auricolare e del CUE lo scopo dell’approfondimento radiologico si basa sulla necessità di indagare eventuali anomalie associate delle strutture dell’orecchio medio o interno, per descriverle nei minimi dettagli e per classificare l’atresia come ossea, membranosa o mista. Nei pazienti senza malformazione del CUE l’imaging serve per confermare o escludere il sospetto di ACA [15–17]. La TC rappresenta l’indagine di scelta in corso di ACA per l’intrinseca elevata risoluzione ad alto contrasto che esalta ambienti come l’orecchio esterno e medio composti per lo più da aria ed osso [18]: in questo modo la TC riesce ad evidenziare eventuali malformazioni congenite sospette, includendo anche altre cause di ipoacusia trasmissiva come timpanosclerosi e otosclerosi [19], rappresentando uno step obbligatorio nel planning pre-chirurgico dell’ACA [20]. Le radiografie standard del cranio, nelle proiezioni dedicate per lo studio otologico e gli studi tomografici convenzionali non presentano indicazioni all’espletamento né risultano utili ai fini diagnostici in pazienti con ACA. La RM, paragonata alla TC, non è in grado di dimostrare nel dettaglio le malformazioni dell’orecchio esterno e medio [19], ma riveste un ruolo fondamentale nell’iter radiologico pre-chirurgico perché è l’unica indagine di imaging in grado di studiare nel dettaglio le strutture dell’orecchio interno, il nervo cocleare e le vie acustiche centrali [21–23], la cui integrità è condizione necessaria per la candidatura all’impianto VSB. Una cassa timpanica poco sviluppata ed un antro mastoideo ipoplasico associati ad ACA possono rendere difficoltoso l’approccio chirurgico alla finestra rotonda. La

middle-ear malformations [19], but it has a fundamental role in pre-operative imaging, being the only modality capable of providing a detailed study of the inner ear, cochlear nerve and central auditory pathways [21–23], the integrity of which structures is a prerequisite for considering the patient as a candidate for prosthetic implantation. A poorly developed tympanic cavity and a hypoplastic mastoid antrum associated with CAA may render surgical access to the round window difficult. Pneumatization and size of the middle ear and mastoid are inversely proportional to the thickness of the atretic plate [16]. Superimposition of middle-ear infections and the association of congenital cholesteatoma are rare [24, 25]. Abnormalities of the ossicular chain are frequently associated with CAA and include partial or total agenesis, deformity, ectopia, malrotations and ossicular fusion [2, 16, 19]. In our study, the most frequent findings were the presence of a rudimentary malleus and incus that were fused to each other. Window abnormalities have been reported to occur in association with CAA and mainly consist of agenesis, stenosis and otosclerosis [16, 26]. In our study, all patients had a normal round window with caliber >2 mm; abnormalities of the oval window do not, however, constitute a contraindication for the surgical procedure. Abnormalities of the facial-nerve course are commonly found in association with external- and middle-ear congeni-

La sovrapposizione di infezioni dell’orecchio medio o colesteatoma congenito associato ad ACA sono eventi rari [24, 25]. Anomalie a carico della catena ossiculare frequentemente si associano ad ACA, comprendendo agenesie parziali o totali, deformità, ectopie, malrotazioni e fusioni ossiculari [2, 16, 19]: nel nostro studio i reperti più frequenti sono stati la presenza di un martello e di un’incudine rudimentali e fusi fra loro. L’associazione di anomalie fenestrali e ACA sono riportate in letteratura e consistono principalmente in agenesia, stenosi e otosclerosi [16, 26]: nel nostro studio tutti i pazienti presentavano una finestra rotonda regolare, con calibro superiore a 2 mm, mentre anomalie a carico della finestra ovale non rappresentavano una controindicazione. Anomalie di decorso del nervo faciale si associano frequentemente a malformazioni congenite dell’orecchio esterno e medio. Ai fini chirurgici risultano più importanti le anomalie del tratto timpanico e del mastoideo. La condizione più frequente è la deiscenza del tratto timpanico che potrebbe rendere difficoltoso l’accesso alla finestra rotonda; sono descritte inoltre anomalie di decorso e stenosi/ipoplasia del canale osseo per il nervo faciale [16],

tal malformations. For the purposes of surgery, tympanic and mastoid-segment abnormalities are more important. The most frequent of these abnormalities is tympanic segment dehiscence, which might obstruct access to the round window; additional abnormalities concern facial-nerve canal course and stenosis/hypoplasia [16], a situation present in only one of our patients. One jugular diverticulum was seen in our study that, together with other vascular abnormalities, constitutes a rare but potential risk of intra-oper-

ative bleeding [2, 16]. We found no Eustachian tube abnormalities, despite reports of pathological dilatation of its bony segment [16]. We did not detect abnormalities in the cochlea, vestibule, semicircular canals or internal auditory canal in patients selected for the procedure, eventually recognizable with MR imaging. CAA may be associated with syndromic disorders involving cranio-facial malformations due to embryological damage to structures deriving from the first two branchial arches [27, 28], but no such cases were identified in our study. After VSB device placement, MR imaging is contraindicated, even though some studies regarding the safety of high-field-strength MR imaging and hearing implants have been published in the literature [29]. CT is therefore the only imaging technique capable of verifying the accurate position of the FMT on the round window, given that conventional radiography has already been shown inadequate for this purpose [30]. Relatively recently, cone-beam CT (CBCT) has been proposed as an alternative to HRCT for studying the temporal bone in cases of conductive hearing loss and for the follow-up of patients with cochlear implant

La ACA può associarsi a quadri sindromici di malformazioni cranio-facciali per danno embriologico delle strutture derivanti dai primi due archi branchiali [27, 28] che tuttavia nel nostro studio non sono stati riscontrati. Successivamente al posizionamento della VSB l’esecuzione della RM risulta controindicata, nonostante alcuni studi riguardanti la sicurezza RM ad alto campo e gli impianti acustici siano presenti in letteratura [29]. La TC è pertanto la tecnica a disposizione del radiologo per verificare l’esatta collocazione della MV nella finestra rotonda essendo la radiografia convenzionale già stata dimostrata inadeguata per questo scopo [30]. In tempi relativamente recenti la cone beam computed tomography (CBCT) è stata proposta in letteratura come indagine alternativa alla TC ad alta risoluzione nello studio dell’osso temporale in casi di ipoacusia trasmissiva e nel follow-up di pazienti con impianto cocleare o VSB alla finestra rotonda [30–32]. Questa tecnica TC condivide con le apparecchiature TC

or VSB at the round window [30–32]. This technique has in common with CT the delivery of ionising radiation, good high-contrast resolution and the possibility of obtaining multiplanar and volumetric reconstructions; CBCT offers some advantages over standard CT, such as low costs, limited size and low radiation dose, but also some disadvantages, including small field of view and lower image quality [33, 34].

Conclusioni

Conclusions HRCT is the examination of choice for pre-operative assessment of patients with CAA, thanks to its high level of anatomical detail when studying the temporal bone, its ability to accurately depict all the structures relevant to candidate patients for VSB placement on the round window and to its ability to help the surgeon to predict possible risks and intra-operative difficulties.

La TC ad alta risoluzione rappresenta l’esame di scelta nello studio pre-operatorio di pazienti con ACA per l’elevato dettaglio anatomico nello studio dell’osso temporale e la capacità di descrivere accuratamente tutte le strutture d’interesse per candidare il soggetto al posizionamento di VSB alla finestra rotonda e prevedere gli eventuali rischi e difficoltà intra-operatorie.
