Abstract Purpose. This paper reports our preliminary experience with the endovascular treatment of intracranial aneurysms using flow-diverter stents (FDs) and compares it with the literature data. Materials and methods. From May 2009 to April 2012, 28 patients (6 men and 22 women; mean age, 54 years) with a total of 35 aneurysms were treated with FDs. We evaluated postprocedural technical success and long-term efficacy, with follow-up examinations performed at 3–7 days [computed tomography (CT)/magnetic resonance (MR) angiography] and at 3, 6 and 12 months (digital subtraction angiography, DSA). A total of 43 FDs were placed, 36 Pipeline and 7 Silk. Results. A total of 30 procedures were performed (two patients were treated twice). Technical success was 96.6%, with one case of postprocedural death; the aneurysm exclusion rate at 3, 6 and 12 months was 60%, 73% and 89%, respectively. There was no case of acute stent thrombosis, and only two cases of nonsignificant stenosis. All covered side branches were patent, except one case of steno-occlusion of the ophthalmic artery. Conclusions. Our results are consistent with the literature and demonstrate the effectiveness and safety of FDs in

Riassunto Obiettivo. Scopo del presente lavoro è riportare la nostra esperienza nel trattamento endovascolare degli aneurismi cerebrali mediante stent a diversione di flusso (FD), confrontandola con i dati della letteratura. Materiali e metodi. Da maggio 2009 ad aprile 2012 sono stati trattati mediante FD 28 pazienti (6 maschi e 22 femmine; età media 54 anni) portatori di 35 aneurismi cerebrali. Abbiamo valutato il successo tecnico procedurale e l’efficacia a distanza mediante angiotomografia computerizzata (TC) o angio-risonanza magnetica (RM) a 3–7 giorni, quindi angiografia digitale a sottrazione (DSA) a 3, 6 e 12 mesi. Sono stati posizionati complessivamente 43 FD, 36 Pipeline e 7 Silk. Risultati. Complessivamente sono stati eseguiti 30 interventi (due ritrattamenti a 3 mesi per copertura parziale del colletto aneurismatico). Abbiamo ottenuto il successo tecnico nel 96,6% dei casi, con un caso (3,4%) di insuccesso esitato in decesso post-operatorio. L’esclusione dal circolo dell’aneurisma è stata a 3, 6 e 12 mesi rispettivamente del 60%, 73% ed 89%. sono risultati pervi, eccetto un caso di steno-occlusione dell’arteria oftalmica. Conclusioni. I risultati da noi ottenuti, in linea con i dati della letteratura, documentano che il trattamento degli aneurismi cerebrali mediante FD rappresenta un’opzione sicura ed efficace in casi selezionati (aneurismi a largo colletto, fusiformi, blister-like), con buoni risultati nel tempo. Introduction

Introduzione

The treatment of brain aneurysms represents a major challenge for specialists in the field. In 1974, Serbinenko [1] was the first to describe the use of an intravascular latex balloon. Since then, the inventiveness of the pioneers coupled with the remarkable technological advances in materials have allowed the endovascular treatment of intracranial aneurysms to shift from the occlusion of the parent artery to the occlusion of the aneurysm dome [2]. With the refining of techniques and the introduction of coils, endovascular treatment gradually established itself, in part thanks to the stability of the results obtained [3]. Recently, in an effort to further improve the endovascular technique, alternative devices have been developed such as flow diverters (FD) (Pipeline-PED, ev3 Neurovascular Irvine California; Silk, Balt Extrusion, Montmorency, France) and meshed stents, which cover the aneurysm neck and modify haemodynamic flow within the aneurysm and parent artery to induce thrombosis, while preserving the patency of covered side branches [4–6]. The primary purpose of this paper is to report our preliminary experience with the treatment of intracranial aneurysms with FD, in terms of complete exclusion of the aneurysm dome and patency of the stent and covered branches.

Il trattamento degli aneurismi intracranici rappresenta una grande sfida per gli specialisti del settore. Dal 1974, epoca in cui Serbinenko [1] descrisse per primo il trattamento di aneurismi cerebrali per via endovascolare mediante l’uso di un palloncino di lattice, in pochi anni l’inventiva dei pionieri di tale metodica, associata al notevole sviluppo tecnologico dei materiali, ha consentito il passaggio del trattamento endovascolare degli aneurismi intracranici dall’occlusione del vaso portante al trattamento selettivo endosacculare [2]. Con l’affinamento delle tecniche e l’introduzione delle spirali, il trattamento endovascolare è andato sempre più affermandosi grazie anche alla stabilità dei risultati ottenuti [3]. Nel tentativo di migliorare ulteriormente la tecnica endovascolare, negli ultimi anni, sono stati sviluppati device alternativi alle spirali quali gli stent a diversione di flusso (FD) (Pipeline-PED, ev3 Neurovascular Irvine California, USA; Silk, Balt Extrusion, Montmorency, Francia), stent a maglie strette, che posizionati a copertura del colletto aneurismatico, modificano l’emodinamica del flusso nella sacca e nel vaso portante, assicurando la trombosi dell’aneurisma, con conservata pervietà del vaso d’origine e dei suoi rami collaterali ricoperti [4–6]. Scopo primario del lavoro è riportare la nostra preliminare esperienza nel trattamento di aneurismi intracranici con FD, in termini di esclusione completa della sacca aneurismatica dal circolo, pervietà dello stent e dei vasi ricoperti.

Materials and methods Twenty eight consecutive patients (6 men and 22 women; mean age, 54 years) with 35 aneurysms, were treated with FD between June 2009 to and April 2012. The study was approved by the local ethics committee. Six patients were treated for asymptomatic (incidental) aneurysm, seven because of headaches, five due to mass effect (brainstem and cranial nerves), nine due to recanalisation following previous endovascular/surgical repair, one for a subarachnoid haemorrhage (SAH) due to ruptured blister-like aneurysm (Table 1). sidered on the basis of the morphology of the aneurysm (fusiform or saccular), width of the neck (dome-to-neck ratio <2) and when the treatment with coils and conventional stents would be difficult (high probability of failure, complications or recurrence rate). In some cases, the patient’s strong motivation made us lean towards this type of treatment. The exclusion criterion was the presence of a small neck with a dome-to-neck ratio >2 [4]. All treatments were performed with a Philips Integris V5000 angiograph (Philips Medical Sciences, Best, Netherlands) not equipped with 3D rotational technique or virtual stenting software. Study design was prospective.

trattato recidive post-chirurgiche e post embolizzazione, in 1 caso è stato trattato in urgenza un paziente con emorragia subaracnoidea (ESA) da rottura di aneurisma blister-like (Tabella 1). L’indicazione al trattamento con FD è stata posta in base alla morfologia dell’aneurisma (fusiforme o sacciforme), all’assenza di colletto (dome/neck ratio<2) e quando il trattamento con tecniche convenzionali (spirali e stent) sembrava essere difficoltoso (alta probabilità di fallimento, di complicanze o recidiva). In alcuni casi, la forte motivazione del paziente ci ha fatto propendere verso questo tipo di trattamento. Il criterio di esclusione è stato essenzialmente la presenza di un colletto di piccole dimensioni, con dome/ neck ratio>2 [4]. Tutti i trattamenti sono stati eseguiti su angiografo monoplano Philips Integris V5000 (Philips Medical Sciences, Best, Olanda), apparecchiatura non dotata di tecnica 3D rotazionale o software per neuronavigazione. Lo studio è stato effettuato in modo prospettico.

cording to the diameter of the parent artery and extension of coverage. Vessel analysis software (PACS Kodak Carestream; Easteam Kodak Company) was used to calculate the vessel’s real axial diameter and to create linear images of the vessel. Materials Two types of commercial FD were employed, for a total of 43 FD, 36 PED (diameter from 2.5 to 4.5 mm, length from 16 to 20 mm) and seven Silk (diameter from 3.5 to 4.5 mm, length from 25 to 50 mm). Pipeline (PED, ev3 Neurovascular) was used in 25 patients; in one case of a giant basilar aneurysm, in addition to PED we used a 4×20 mm Solitaire stent (ev3 Neurovascular) to ensure better aneurysm coverage as well as coils (3D Axium, ev3 Neurovascular) to accelerate thrombosis of the aneurysm sac (Fig. 1a,b).

Nel post-processing sono state realizzate ricostruzioni MipPR (multiplanar reconstruction based on maximum intensity projection images), per poter scegliere la misura più appropriata dello stent (diametro e lunghezza) in base al valore del diametro del vaso da ricostruire e dell’estensione della copertura. È stato inoltre utilizzato un algoritmo di ricostruzione analisi vaso, presente nel software del nostro sistema di archiviazione immagini (PACS Kodak Carestream; Easteam Kodak Company), in grado di creare immagini lineari del vaso di interesse e di calcolarne il reale diametro assiale. Materiali impiegati Sono stati impiegati i due tipi di FD in commercio, posizionando complessivamente 43 FD, 36 PED (diametro da 2,5 a 4,5 mm, lunghezza da 16 a 20 mm) e 7 Silk (diametro da 3,5 a 4,5 mm, lunghezza da 25 a 50 mm).

The Silk device (Balt Extrusion) was selected in three patients owing to operator experience, flexibility of the device and its ability to better adapt to variations in calibre of the parent vessel: in two cases of vertebro-basilar fusiform aneurysms (giant and large, respectively), in addition to Silk, a total of four Leo+ stents (Balt Extrusion) were used (diameter 4.5 mm, length 40 to 75 mm) as a scaffold for the FD (Fig. 2a-c). Procedural protocol Before treatment all patients were informed about the type of treatment planned and the associated benefits and risks, and signed an informed consent form. All treatments were performed in the angiography suite using a Philips Integris V5000 single-plane angiograph (Philips Medical Sciences, Best, Netherlands), with the patients under general anaesthesia and using a percutaneous transfemoral approach (6 Fr). Il Pipeline (PED, ev3 Neurovascular) è stato posizionato in 25 pazienti; in un caso di aneurisma gigante dell’arteria basilare, oltre al PED, abbiamo utilizzato uno stent Solitaire 4×20 mm (ev3 Neurovascular) per migliorare la ricostruzione e la copertura dell’aneurisma in associazione a spirali (3D Axium, ev3 Neurovascular) in modo da accelerare il fenomeno di trombosi endosacculare (Fig. 1a,b). Il Silk (Balt Extrusion) è stato scelto in 3 pazienti, in base all’esperienza dell’operatore, alla nota flessibilità del device e alla sua capacità di adattarsi meglio a variazioni di calibro del vaso parente: in due casi di aneurismi fusiformi (rispettivamente giant e large) dell’asse vertebro-basilare, oltre al Silk sono stati utilizzati complessivamente 4 stent Leo+ (Balt Extrusion) (diametro da 4,5 mm, lunghezza da 40 a 75 mm) come impalcatura per il FD (Fig. 2a-c). was performed using a 4 Fr diagnostic catheter (Cordis Corp, Johnson and Johnson, Miami Lakes, FL, USA) and a 0.035 inch hydrophilic guidewire (Terumo Corp, Tokyo, Japan); subsequently, a carrier catheter (6 Fr Envoy, Corp, Johnson and Johnson; 6 Fr Penumbra Neuron, Penumbra Inc, Harbor Bay Pkwy Alameda, CA, USA) was inserted to reach a suitable location in the cavernous segment of ICA or the V3 segment of the vertebral artery. A delivery microcatheter was advanced, with the coaxial technique, along a neurovascular microguide up to the distal portion of the aneurysm neck. In all cases treated with PED, Marksman (ev3 Neurovascular) or Rebar (ev3 Neurovascular) microcatheters were used. In patients treated with Silk, Vasco+21 and Vasco+25 (Balt Extrusion) microcatheters were used. In all cases, Transend 205 EX Floppy (Boston Scientific, Natek, NA, USA) or Synchro 14 (Boston Scientific) microguides were used. Then, the FD system was pushed through the delivery microcatheter and aligned with the distal tip of the microcatheter; the microcatheter was retracted slowly to unsheathe the stent, while a forward tension was maintained on the stent to keep it in place, with a “push and pull” technique. The stent was placed across the aneurysm neck. After placement of the stent, angiography was performed to evaluate technical success, intended as covering the aneurysm neck and proper distention of the stent within the vessel lumen. Medical therapy All patients were pretreated with 250 mg of ticlopidine twice daily for 5 days. Only in one of the patients, treated as an emergency for a ruptured aneurysm, was a bolus of 300 mg of clopidogrel administered 6 hours before the procedure. At the beginning the procedure all patients received heparin (50 IU/kg+1000 IU/h) while a bolus of acetylsalicylic acid (500 mg) was administered when releasing the stent. After the procedure and during the first 3 months, dual antiplatelet therapy (250 mg of ticlopidine twice daily and 300 mg of acetylsalicylic acid once daily) was prescribed; single antiplatelet therapy with 300 mg of acetylsalicylic acid once daily was then continued for 3 to 6 months. Clopidogrel was not used because of the high percentage of drug resistance and because at the time of treatment this drug was not reimbursed by the Italian National Health System. Follow-up All patients underwent follow-up by CT or MR 3 to 7 days after treatment to assess correct positioning of the stent.

Tutti gli interventi sono stati eseguiti in sala angiografica, mediante angiografo monoplano Philips Integris V5000 (Philips Medical Sciences, Best, Olanda), in anestesia generale, previa puntura percutanea trans-femorale comune, con accesso vascolare 6 Fr; è stato eseguito un esame diagnostico preliminare mediante l’utilizzo di catetere 4 Fr (Cordis Corp, Johnson and Johnson, Miami Lakes, FL, USA) e guida idrofilica 0,035 inch (Terumo Corp, Tokyo, Giappone); successivamente è stato posizionato un catetere portante (Envoy 6 Fr, Corp, Johnson and Johnson; Penumbra Neuron 6 Fr, Penumbra Inc, Harbor Bay Pkwy Alameda, CA, USA) nel vaso di interesse in una sede adeguata al trattamento. Infine al suo interno, in tecnica coassiale, è stato avanzato un microcatetere su microguida neurovascolare, sino a raggiungere una posizione distale al colletto dell’aneurisma. In tutti i casi trattati con PED è stato impiegato un microcatetere Marksman (ev3 Neurovascular) o in alternativa Rebar (ev3 Neurovascular Irvine); nei pazienti trattati con Silk sono stati usati un microcatetere Vasco +21 e Vasco +25 (Balt Extrusion). In tutti i casi sono state impiegate microguide Transend 205 EX Floppy (Boston Scientific, Natek, NA, USA) o Synchro 14 (Boston Scientific). Il FD è stato quindi inserito all’interno del microcatetere, sino ad allinearlo all’estremo distale del microcatetere; lo stent è stato poi rilasciato, a cavallo del colletto aneurismatico, con una combinazione tra retrazione del microcatetere e spinta della guida di rilascio, in tecnica di “push and pull”. Dopo il posizionamento dello stent, sono stati eseguiti i controlli angiografici per valutare il successo tecnico, inteso come copertura del colletto aneurismatico e la corretta distensione dello stent all’interno del lume vasale. Terapia medica Tutti i pazienti hanno assunto ticlopidina 250 mg×2/die da 5 giorni prima dell’intervento. Nel solo caso di trattamento di aneurisma blister-like rotto, trattato in urgenza, è stato somministrato un bolo di clopidogrel 300 mg, 6 ore prima della procedura. Durante l’intervento sono stati somministrati eparina (50 UI/kg+1000 UI/h) e un bolo di flectadol 500 mg al rilascio dello stent. Dopo il trattamento e per i primi 3 mesi è stata impostata una duplice terapia antiaggregante (ticlopidina 250 mg×2/die+acido acetilsalicilico 300 mg×1/die); dal 3° al 6° mese è stata proseguita la singola terapia antiaggregante con acido acetilsalicilico 300 mg×1/die. Eccetto che in un caso non abbiamo utilizzato Clopidogrel a causa dell’alta percentuale di resistenza e la non mutuabilità del farmaco. Follow-up with digital subtraction angiography (DSA) or alternatively CT was performed at 3, 6 and 12 months. The following parameters were analysed: aneurysm occlusion and patency of stent/side branches, in agreement with the literature data [7, 8].

Results Risultati Thirty treatments were performed in 28 patients (two cases were retreated at 3 months because of partial coverage of the aneurysm neck). Table 1 shows the characteristics of the patients and aneurysms. In 29 cases treatment was elective; in one case, the presence of a ruptured blister-like aneurysm of the carotid siphon, warranted emergency treatment by placement of two overlapping PED (Fig. 3c). The patient died in intensive care 21 days after the procedure

Abbiamo effettuato complessivamente 30 interventi in 28 pazienti (due ritrattamenti a 3 mesi per copertura parziale del colletto aneurismatico). La Tabella 1 mostra le caratteristiche degli aneurismi e dei pazienti trattati. In 29 casi l’intervento è stato eseguito in regime di elezione ed in un caso in regime d’urgenza per presenza di aneurisma blister-like del sifone carotideo associato ad ESA per rottura dello stesso, mediante posizionamento di due PED embricati (Fig. 3a-c).

Technical success and intraprocedural complications

Successo tecnico e complicanze intra-procedurali

Technical success (adequate coverage of the aneurysm neck without intraprocedural complications) was achieved in 29/30 treatments (96.6%). In one case of a dissecting middle cerebral aneurysm we had a defect of PED distal opening, with locking and distal migration of the system in the parent artery. Attempts to recover the system (SnareAndra Andramed GmbH Germany, ev3 Neurovascular Hyperform balloon; ev3 Neurovascular Retriever Alligator) were not effective, with subsequent vessel perforation and death of the patient after 24 hours. We therefore had an overall mortality of 7% (2/28 patients).

Il successo tecnico (adeguata copertura del colletto aneurismatico in assenza di complicanze intra-procedurali) si è ottenuto in 29/30 interventi (96,6%); in un caso (3,4%) di aneurisma della cerebrale media si è verificata la difettosa apertura distale dello stent (PED) con incarceramento e migrazione distale dello stesso nel vaso parente. Sono stati effettuati numerosi tentativi di recupero dello stent (SnareAndra Andramed GmbH, Germania; ev3 Neurovascular Hyperform balloon; ev3 Neurovascular Retriever Alligator) risultati tuttavia infruttuosi, con successiva perforazione della cerebrale media e decesso del paziente a distanza di 24 ore. Abbiamo pertanto osservato una mortalità complessiva del 7% (2/28 pazienti).

Aneurysm occlusion rate at follow-up Table 2 shows the occlusion rate of treated aneurysms according to size. Complete occlusion of the aneurysm was documented at 3-month follow-up in 60% of cases (18/30 aneurysms), at 6 months in 73% of cases (19/26) and at 12 months in 89% (16/18). In three patients with 24 months angiographic follow-up we observed complete occlusion of the aneurysm in two cases (67%). In the remaining cases minimal opacification of a residual sac was seen, with persistence of the contrast medium in the venous phase of the study, interpreted as a partial thrombosis of the aneurysm dome [6]. Patency of the stent, parent artery and covered side branches Parent artery patency was observed in 100% at 3, 6 and 12 months, without any occurrence of acute stent thrombosis. Intrastent intimal hyperplasia, with haemodynamically nonsignificant stenosis and without symptoms, was observed at 3 months follow-up of two carotid ophthalmic aneurysms treated with PED implantation. In both cases we prolonged the dual antiplatelet therapy [ticlopidine 250 mg×2/die, acetylsalicylic acid (ASA) 300 mg/die] to six months and the single antiplatelet therapy (ASA 300 mg/day) up to 12 months, with full restoration of regular stent patency, in the absence of bleeding complications (Fig. 4). At 6-month follow-up, in a case of cavernous aneurysm of the ICA, there was a reduction in the caliber of the ophthalmic artery and occlusion of the collateral orbital branches, with maintained patency of the retinal and

Esclusione dal circolo dell’aneurisma La Tabella 2 mostra l’esclusione dal circolo degli aneurismi trattati in relazione alle loro dimensioni. A 3 mesi abbiamo osservato complessivamente un tasso di esclusione dal circolo del 60% (18/30 aneurismi), a 6 mesi del 73% (19/26) ed a 12 mesi dell’89% (16/18). Nei tre pazienti di cui disponiamo di follow-up angiografico a 24 mesi, in due casi (67%) abbiamo osservato completa esclusione dal circolo degli aneurismi. Nei restanti casi si osservava alla DSA un’opacizzazione parziale e tardiva della sacca aneurismatica indicativa di trombosi incompleta [6]. Pervietà dello stent, dei vasi parenti e delle branche collaterali ricoperte dallo stent In tutti i casi abbiamo osservato completa pervietà dello stent a 3, 6 e 12 mesi, in assenza di trombosi intra-stent. Al follow-up a 3 mesi abbiamo riscontrato due casi di iperplasia intimale intra-stent determinanti stenosi non emodinamicamente significative, in assenza di sintomatologia clinica associata (trattamento di due aneurismi del tratto carotido-oftalmico della carotide interna con PED). In entrambi i casi abbiamo prolungato la doppia terapia antiaggregante (ticlopidina 250 mg×2/die; acido acetilsalicilico 300 mg/die) fino a sei mesi e la singola terapia antiaggregante (acido acetilsalicilico 300 mg/die) fino a 12 mesi, con completo ripristino della regolare pervietà degli stent, in assenza di complicanze emorragiche (Fig. 4). In un caso di aneurisma del tratto cavernoso della carotide interna, al follow-up angiografico a 6 mesi, abbiamo osservato riduzione di calibro dell’arteria oftalmica ed occlusione di vasi collaterali orbitali. Tali reperti, determinanti sintomi lievi (scotomi senza deficit del campo visivo), sono stati confermati al controllo a 12 mesi. Non abbiamo osser-

lachrymal branches, with the presence of intermittent scotomas, without visual field defects. After 12 months we confirmed the ophthalmic artery stenosis, with regression of symptoms. There were no other cases of occlusion of the covered side branches. Clinical outcome Clinical follow-up was performed with a modified Rankin Scale (mRS) in 26 patients. We observed four cases of transient, self-limited worsening of neurological symptoms in the immediate postprocedural period: three of these patients had giant/large aneurysm with mass effect. One of

them (with a giant basilar artery aneurysm) experienced a major clinical deterioration 4 months after treatment. This required cerebral angiography (Fig. 1b) and MRI (Fig. 1c): however, the examinations showed the stability of the reconstruction, in the presence of a significant mass effect on the brainstem, without evidence of occlusive embolic phenomena affecting the perforating branches of the basilar artery. Given the stability of the endovascular repair up to the 12-month MRI follow-up (Fig. 1d) it is likely that the late clinical deterioration was caused by progressive thrombosis of the sac and the associated mass effect and oedema on the brainstem. At 3 months, one patient treated for a cavernous carotid aneurysm reported the appearance of intermitted scotomas, without visual field defects; the symptoms had regressed completely at 12 months follow-up.

Data la stabilità della ricostruzione endovascolare sino al controllo RM a 12 mesi (Fig. 1d) è probabile che il peggioramento clinico tardivo sia stato causato dalla trombosi graduale della sacca e dal conseguente effetto massa con edema sul tronco encefalico. In un caso di aneurisma del tratto cavernoso della carotide interna, il paziente ha lamentato a 3 mesi dall’intervento, la comparsa di scotomi intermittenti, senza deficit del campo visivo: la sintomatologia è poi regredita completamente al controllo a 12 mesi dal trattamento. Discussione

Discussion Endovascular treatment of intracranial aneurysms has become a valuable alternative to surgery [9]. Although surgery has a lower relapse rate in the region of the aneurysm neck (1.4%–8%) [10–12] compared to endovascular treatment (9%–59%) [10, 13, 14], it is associated with higher morbidity and mortality rates, especially in cases of large/ giant or wide neck aneurysms [10–12]. Endovascular treatment of cerebral aneurysms with a conventional device (coils with the possible association of a stent) represents an effective option which is, however, associated with a risk of failure in cases of unfavorable anatomy, difficulty obtaining a total filling of the aneurysm with coils [15, 16], and difficulty achieving complete coverage of the neck, with a high risk of recurrence [3]. The use of FD changes the treatment approach from intrasaccular to parent artery reconstruction, obtaining greater neck coverage than with coils alone [6]. This new type of stent, by reconstructing the parent artery, determines a reduction of the whirling flow within the sac, inducing thrombosis and simultaneously ensuring patency of the covered side branches. In our experience, we achieved technical success in 29/30 treatments (96.6%), with adequate coverage of the aneurysm neck, without intraprocedural complications. In one case (3.8%) technical failure occurred leading to the patient’s death (treatment-related mortality, 3.5%), in line with the literature data [8, 17]. There was one additional death in a case of a ruptured blister-like aneurysm that was treated with PED implantation in an emergency setting. This patient died in intensive care 21 days after treatment due to complications unrelated to the aneurysm (sepsis), despite the technical success of

Il trattamento endovascolare degli aneurismi cerebrali rappresenta attualmente una valida alternativa alla chirurgia [9]; quest’ultima presenta un più basso grado di recidiva nella regione del colletto (1,4%–8%) [10–12] a confronto con il trattamento endovascolare (9%–59%) [10, 13, 14] ma è associata ad un maggior tasso di mortalità e morbilità, particolarmente in aneurismi large/giant o con ampio colletto [10–12]. Il trattamento endovascolare degli aneurismi cerebrali con device convenzionali (spirali con eventuale associazione di stent) rappresenta un’opzione efficace, associata tuttavia a rischio di fallimento terapeutico in caso di anatomia sfavorevole, difficoltà nell’ottenere un riempimento totale dell’aneurisma con spirali [15, 16], difficoltà nella copertura completa del colletto, con alto rischio di recidive [3]. L’utilizzo dei FD ha cambiato l’approccio del trattamento endovascolare dalla tecnica classica endosacculare a quella innovativa endoluminale, con ricostruzione del vaso d’origine e con una maggiore copertura del colletto aneurismatico rispetto a quella ottenuta con tecniche convenzionali [6]. Questo tipo di stent, ricostruendo il vaso da cui origina l’aneurisma, determina una riduzione del flusso vorticoso all’interno della sacca, inducendone la trombosi e garantendo contestualmente la pervietà dei vasi arteriosi collaterali. Nella nostra esperienza abbiamo ottenuto un successo tecnico in 29/30 interventi (96,6%), con adeguata copertura del colletto aneurismatico, in assenza di complicanze intraprocedurali. In un solo caso (3,8%) si è verificato insuccesso tecnico con decesso del paziente (mortalità intervento-correlata: 3,5%), in linea con i dati della letteratura [8, 17]. Abbiamo riscontrato un ulteriore decesso nel caso di un aneurisma blister-like rotto, trattato in urgenza con PED. Questo paziente, nonostante il successo tecnico della procedura, confermato anche dallo studio angio-TC dopo 7 giorni (Fig. 3d), è tuttavia deceduto per complicanze non correlate all’intervento a distanza di 21 giorni (sepsi; mortalità complessiva: 7%).

the procedure, as confirmed by CT after 7 days (total mortality, 7%) (Fig. 3d). The treatment of ruptured blister-like aneurysms with FD is an option reported in the literature [18–20]: in fact, this type of aneurysm has a fragile and poorly defined wall, without a recognisable neck, which makes surgical and endovascular treatment very difficult. FD currently represents the only therapeutic option that allows a quick and safe endovascular repair of the diseased vessel, in both elective and emergency settings [18-20]. SAH is still the greatest limit to the use of FD, given the need for dual antiplatelet therapy: in the cases of ruptured blister-like aneurysms, however, the need for immediate treatment overcomes this limitation. In our experience, we administered a bolus of 300 mg of clopidogrel 6 hours before the procedure and continued with the usual dual antiplatelet therapy postoperatively, in the absence of bleeding complications. The literature shows a different therapeutic management with administration of dual antiplatelet therapy only in the postoperative period [18–20]. We observed complete occlusion of the aneurysm in 60% of cases at 3 months, 73% at 6 months, 89% at 12 months (Table 2). These data are in agreement with the current literature, as in the Pipeline Intracranial Treatment of Aneurysms (PITA) trial [21], which showed complete aneurysm occlusion in 93% and 97% at 6 and 12 months, respectively, or the Buenos Aires [17] and Budapest experiences [8]. These results demonstrate the effectiveness of the FD in the treatment of intracranial aneurysms, with a high rate of complete occlusion of the aneurysm sac at follow-up. These innovative devices preserve the patency of covered side branches, as demonstrated in preclinical [4] and clinical studies [7]. We experienced one case of steno-occlusion of the orbital branches of the ophthalmic artery and

Il trattamento di aneurismi blister-like rotti con FD è un’opzione riportata in letteratura [18–20]: questo tipo di aneurismi presenta, infatti, una parete fragile e non ben definita, in assenza di colletto riconoscibile, che rende difficile il trattamento sia chirurgico che endovascolare con tecniche convenzionali. I FD rappresentano attualmente l’unica opzione terapeutica che consente una corretta e sicura ricostruzione endovascolare del vaso malato, tanto in elezione quanto in urgenza [18–20]. L’ESA rappresenta ancora oggi la maggiore controindicazione all’utilizzo del FD, data la necessità della doppia terapia antiaggregante: negli aneurismi blister-like rotti tuttavia la necessità di un immediato trattamento supera questa controindicazione. Nella nostra esperienza abbiamo somministrato un bolo di 300 mg di clopidogrel 6 ore prima dell’intervento e proseguito poi con la consueta doppia terapia antiaggregante nel post-operatorio, in assenza di complicanze emorragiche. In letteratura è riportato un differente management terapeutico con somministrazione della doppia terapia antiaggregante solo nel postoperatorio [18–20]. Abbiamo osservato una completa esclusione degli aneurismi dal circolo del 60% a 3 mesi, del 73% a 6 mesi e dell’89% a 12 mesi (Tabella 2). Tali risultati sono in accordo con i dati della letteratura, come dimostrato dallo studio Pipeline Intracranial Treatment of Aneurysms (PITA) [21] che riporta tassi di occlusione a 6 e 12 mesi, rispettivamente del 93% e 97% e dalle esperienze preliminari di Buenos Aires [17] e di Budapest [8]. Questi risultati documentano l’efficacia dei FD nel trattamento degli aneurismi intracranici con alto tasso di completa occlusione della sacca aneurismatica al follow-up. Tali innovativi device preservano la pervietà delle branche collaterali ricoperte, come dimostrato da studi preclinici [4] e clinici [7]. Nella nostra esperienza abbiamo osservato un solo caso di steno-occlusione delle branche orbitali dell’arteria oftalmica e riduzione di calibro di quest’ultima, con comparsa di scotomi intermittenti, risoltisi poi nel tempo.

reduction of the artery’s caliber with intermittent scotomas, which resolved over time. The risk of thrombosis and intrastent stenosis is a problem related to the use of intracranial stents; hence the need for dual antiplatelet therapy. In our experience, there were no cases of acute or late stent thrombosis; only two cases of haemodynamically nonsignificant and temporary stenosis occurred (Fig. 4). Our results are consistent with those reported in the literature: the PITA trial [21] reported only one case of moderate stenosis and Szikora et al. [8] found no significant stenoses among 19 treated aneurysms. On the contrary, Lubicz et al. [6] reported the presence of haemodynamically significant stenoses in 33% of cases, most of which were short and located in the distal portion of the stent, especially in cases in which the difference between proximal and distal tracts of the parent artery was wide. The prolonged antiplatelet therapy which we administered beyond the usual limits of time allowed the complete resolution of stenosis (Fig. 4). Also, in contrast to the literature, we observed no cases of stent thrombosis. Probably the peculiarity of the antiplatelet regimen used, different from that used in the major studies in the literature [8, 17, 21], contributed to this result. FD stents were introduced for the treatment of fusiform, saccular, large, giant or wide-neck aneurysms [5, 7, 17]. Large and giant aneurysms, which manifest primarily through a mass effect on the surrounding anatomical structures, can be treated with conventional devices, but the results are unsatisfactory [10, 13, 14]. The FD progressively excludes the aneurysm from the circulation, eliminating the risk of rupture and providing thrombus resorption (shrinkage of the aneurysm), with reduction of the mass effect. However, induction of thrombosis could trigger the mass effect, with exacerbation of clinical symptoms [5]. In our experience, we treated four aneurysms with mass effect: in the immediate posttreatment period, symptoms worsened in 3/4 cases (75%) due to dome thrombosis and consequent increase of mass effect on the brainstem; another patient with giant aneurysm of the basilar trunk also presented a late worsening of the compressive symptoms after 4 months, but with unchanged aneurysm size and without rupture (Fig. 1c,d).

Conclusions The small population size and the short follow-up period are the main limitations of our study. The results obtained are in agreement with those reported in the literature [6– 8, 17–21], and in particular with the recently published French multicentre study [22] and German single-centre

Il rischio di trombosi e stenosi intrastent è un problema legato all’impiego di stent intracranici, da qui la necessità di eseguire una doppia terapia antiaggregante. Nella nostra esperienza non abbiamo osservato nessun caso di trombosi acuta o tardiva dello stent; si sono verificati solo due casi di stenosi temporanee non emodinamicamente significative (Fig. 4). I nostri risultati sono in linea con i dati della letteratura: nel trial PITA [21] è stato osservato solo un caso di stenosi moderata e Szikora et al. [8] non hanno riportato stenosi significative nei 19 aneurismi trattati. Al contrario Lubicz et al. [6] hanno riportato la presenza di stenosi emodinamicamente significative nel 33% dei casi trattati, la maggior parte delle quali brevi e localizzate nella porzione distale dello stent, soprattutto nei casi in cui il diametro del vaso distalmente al colletto dell’aneurisma era molto minore rispetto a quello prossimale. Nella nostra esperienza, la prosecuzione della terapia antiaggregante, da noi impostata oltre i consueti limiti di tempo, ha permesso la completa risoluzione delle stenosi, già non emodinamicamente significative (Fig. 4). È opportuno considerare che nella nostra esperienza, differentemente da quanto riportato dalla letteratura, non abbiamo osservato alcun caso di trombosi intrastent. Probabilmente la peculiarità dello schema terapeutico antiaggregante da noi adottato, differente da quello dei maggiori studi della letteratura [8, 17, 21], ha contribuito ad ottenere tale risultato. Gli stent FD sono stati introdotti per il trattamento di aneurismi fusiformi, sacciformi, di grandi dimensioni o senza colletto [5, 7, 17]. Gli aneurismi large e giant, che si manifestano primariamente con l’effetto massa sulle strutture anatomiche limitrofe, possono essere trattati con device convenzionali, con risultati tuttavia insoddisfacenti [10, 13, 14]. Il FD escludendo completamente dal circolo l’aneurisma, ha come obbiettivo l’annullamento del rischio di rottura e il riassorbimento del trombo endosacculare, con riduzione dell’effetto massa; l’induzione della trombosi può tuttavia aggravare l’effetto massa stesso, con esacerbazione della sintomatologia clinica [5]. Nella nostra esperienza abbiamo trattato 4 aneurismi che determinavano effetto massa: in 3/4 casi (75%) si è verificato, nell’immediato post-intervento, un peggioramento clinico transitorio, dovuto alla trombosi della sacca e al conseguente aumento dell’effetto massa sul tronco encefalico; in un caso si è verificato un peggioramento clinico tardivo per sintomatologia compressiva, senza rottura dell’aneurisma o variazioni delle dimensioni della sacca (Fig. 1c, d). Conclusioni I limiti del nostro studio sono rappresentati dal basso numero di pazienti e dal breve follow-up. I risultati da noi ottenuti sono in linea con i dati della letteratura [6–8, 17–21], in particolare con lo studio multicentrico francese [22] e lo studio monocentrico tedesco del gruppo di Stoccarda [23]

Thanks to the high rate of occlusion of the aneurysm sac and low incidence of intra- and postprocedural complications, we believe that the treatment of cerebral aneurysms with FD represents a safe and effective option in selected cases (wide-neck, saccular, fusiform and blister-like aneurysms) and can become, in a short time, the treatment of choice for this condition.

Riteniamo pertanto che il trattamento degli aneurismi cerebrali mediante FD rappresenti un’opzione sicura ed efficace in casi selezionati (aneurismi sacciformi a largo coletto, fusiformi e blister-like), potendo garantire un alto tasso di occlusione della sacca aneurismatica e una bassa incidenza di complicanze intra- e post-procedurali e possa diventare in breve tempo l’opzione terapeutica di scelta per tale patologia.
