Abstract

Riassunto

Purpose. This study was done to evaluate the use of multidetector computed tomography (MDCT) in the evaluation of jaw osteonecrosis, a recently described medical entity in patients receiving long-term intravenous or oral bisphosphonates, and to help radiologists recognise it to enable early diagnosis and appropriate management. Material and methods. Thirty-eight patients with a history of neoplasm (breast, prostate, multiple myeloma, pancreas) or osteoporosis and bisphosphonate use and recent oral surgery who presented with pain in the mouth and temporomandibular joints, soft tissue infections, altered taste and numbness were examined with MDCT. Results. In all patients, MDCT revealed the presence of structural bony alterations referable to osteonecrosis associated with orosinusal fistula in four of them and bony intraoral spicules in 25. Conclusions. Jaw osteonecrosis is an established medical entity discovered in patients who have undergone treatment with bisphosphonates and dental extraction or other oral surgery. MDCT with multiplanar (MPR), volume rendering (VR) and three-dimensional (3D) reconstructions allows accurate assessment of affected bone structures, enabling early diagnosis and suitable treatment planning. Radiologists should be aware of the risk of osteonecrosis in patients treated with bisphosphonates and be able to distinguish it from other bone diseases (osteomyelitis and osteoradionecrosis) with which it enters the differential diagnosis.

Obiettivo. Valutare con TC multidetettore (TC-MD) l’osteonecrosi mascellare, recente entità nosologica riscontrabile in pazienti sottoposti a terapia iv od orale per lungo tempo con bifosfonati, ed allertare i radiologi a riconoscerla come tale per una diagnosi precoce ed un adeguato management terapeutico. Materiali e metodi. Sono stati esaminati con TC-MD 38 pazienti, con storia di neoplasia (mammella, prostata, mieloma multiplo, pancreas) od osteoporosi, terapia con bifosfonati e recente chirurgia orale, giunti alla nostra osservazione con algie a livello del cavo orale e delle articolazioni temporo-mandibolari, infezioni dei tessuti molli, alterazioni del gusto e della sensibilità. Risultati. In tutti i pazienti, all’esame TC, è stata riscontrata la presenza di alterazioni ossee strutturali riferibili ad osteonecrosi, associate in 4 di essi a fistola oro-sinusale ed in 25 a spicule ossee endo-orali. Conclusione. La TC-MD con ricostruzioni multiplanari (MPR), volume rendering (VR) e 3D consente una valutazione accurata dell’osteonecrosi mascellare associata a terapia con bifosfonati fornendo informazioni utili sulle strutture ossee coinvolte per un adeguato planning terapeutico. I medici radiologi devono essere a conoscenza del rischio di osteonecrosi al quale vanno incontro i pazienti trattati con bifosfonati ed essere in grado di distinguerla da altri processi patologici dell’osso (osteomielite e osteoradionecrosi) con le quale entra in diagnosi differenziale. Introduction

Introduzione

Bisphosphonates are inhibitors of osteoclastic activity and are used in the treatment of osteoporosis, hypercalcaemia associated with malignancies and osteolytic metastatic lesions secondary to breast cancer, multiple myeloma, Paget’s disease and prostate cancer [1–3]. Although the mechanism of action of bisphosphonates essentially involves a powerful inhibition of bone resorption, the manner in which they block the destruction of hydroxyapatite and interfere with osteoclast function is still unclear [4]. It is known that arrival of bisphosphonates into osteoclasts disrupts the cytoskeleton and blocks vesicular function, hindering resorption and leading to apoptosis. In addition to this, they have an antiangiogenic action that inhibits proliferation of endothelial cells. At the molecular level, bisphosphonates have been shown to influence osteoclast activity by modulating a cell-surface receptor or an intracellular enzyme [5, 6]. A precise causal relationship between bisphosphonate therapy and jaw osteonecrosis has yet to be established. Nonetheless, risk factors have been identified and guidelines published for the prevention, early diagnosis, and management of this side effect, and there are multidisciplinary recommendations for reducing the incidence of jaw osteonecrosis in patients undergoing treatment with bisphosphonates and requiring oral surgery [7]. Dental extraction is often the triggering event, exposing bone to changes and alterations, even though the appearance of spontaneous gingival lesions, toothache, loosening of teeth and numbness is not uncommon. Unfavourable factors include the presence of dental comorbidities, such as caries, abscesses, periodontitis etc.; concurrent steroid treatment and chemo- and radiotherapy [8]. The aim of this study was to use multidetector computed tomography (MDCT) to evaluate jaw-bone changes caused by bisphosphonate-related osteonecrosis to provide a radiological description of this disease entity and help radiologists differentiate it from osteomyelitis and osteoradionecrosis and enable, with the aid of an accurate history and clinical assessment, its early diagnosis and appropriate treatment.

I bifosfonati svolgono una azione inibitrice sull’attività degli osteoclasti e sono attualmente utilizzati per la cura dell’osteoporosi, dell’ipercalcemia associata a patologie maligne e delle lesioni metastatiche osteolitche secondarie a tumore mammario, mieloma multiplo, malattia di Paget, tumore prostatico [1–3]. Sebbene il meccanismo d’azione di questi farmaci sia principalmente basato sul potente effetto inibitore che esercitano sul riassorbimento osseo, resta poco chiaro come questi blocchino la distruzione dell’idrossiapatite e interferiscano sulla funzione degli osteoclasti [4]. È noto che l’ingresso dei bifosfonati nelle cellule osteoclastiche disgreghi il citoscheletro e ostacoli le funzioni vescicolari, determinando un arresto del riassorbimento e l’instaurarsi di processi apoptosici. A tutto ciò si aggiunge l’azione antiangiogenica che inibisce la proliferazione delle cellule endoteliali. A livello molecolare, è stato dimostrato, che i bifosfonati influenzano l’attività degli osteoclasti attraverso la modulazione di un recettore di superficie cellulare o un enzima intracellulare [5, 6]. Una precisa relazione causale fra la terapia con bifosfonati e l’osteonecrosi della mascella non è stata ancora stabilita. Esiste però una identificazione dei fattori di rischio e lo sviluppo di linee guida per la prevenzione, la diagnosi precoce, il management terapeutico ed esistono raccomandazioni multidisciplinari per ridurre l’incidenza delle osteonecrosi dell’osso mascellare nei pazienti sottoposti a terapia con bifosfonati e che devono far ricorso a chirurgia orale [7]. Una estrazione dentaria è spesso il primum movens dell’esposizione ossea alle modificazioni ed alterazioni ossee, anche se non è insolita la comparsa di lesioni gengivali spontanee, mal di denti, mobilità degli stessi e perdita della sensibilità. Fattori sfavorevoli sono la presenza di comorbilità dentarie, che includono carie, ascessi, periodontiti ecc, concomitanti terapie steroidee, chemio e radio-terapia [8]. Scopo del nostro lavoro è stato quello di valutare con TCMD le alterazioni dell’osso mascellare riferibili ad osteonecrosi da bifosfonati per una maggiore riconoscibilità radiologica di tale tipo di osteonecrosi, anche con l’ausilio di una corretta anamnesi e una opportuna valutazione clinica, per una diagnosi differenziale con l’osteomielite e le osteoradionecrosi, una diagnosi precoce di osteonecrosi ed un adeguato planning terapeutico.

Materials and methods Thirty-eight patients presenting with pain in the mouth and temporomandibular joints, soft tissue infection, altered taste, numbness, and lip paraesthesia were examined by MDCT. All patients had a history of neoplasm or osteoporosis, bisphosphonate use and recent oral surgery. Ten of the patients had breast cancer, 12 had multiple myeloma, five had prostate cancer, nine had osteoporosis, one had breast cancer and osteoporosis and one had multiple myeloma and pancreatic cancer. All patients with neoplastic disease at the time of the MDCT scan had received 4 mg monthly of intravenous zoledronate for periods ranging from 12 to 35 months. The patients with osteoporosis had been treated with oral Alendronate (70 mg/week) or Alendronate and Clodronate (100–70 mg weekly). The patient with breast cancer and os604

Materiali e metodi Sono stati esaminati con TC-MD 38 pazienti, giunti alla nostra osservazione con algie a livello del cavo orale e delle articolazioni temporo-mandibolari, infezioni dei tessuti molli, alterazioni del gusto e della sensibilità, parestesia delle labbra. Tutti i pazienti riferivano storia di cancro od osteoporosi, terapia con bifosfonati e recente chirurgia orale. Dei 38 pazienti, 10 erano affetti da ca mammario, 12 da mieloma multiplo, 5 da ca prostatico, 9 da osteoporosi, 1 da carcinoma mammario ed osteoporosi ed 1 da mieloma multiplo e carcinoma del pancreas.

teoporosis had used intramuscular Clodronate (300 mg weekly) for 12 months. The triggering event for osteonecrosis was a tooth extraction for all but the patient with breast cancer and osteoporosis, who had undergone endodontic treatment. Six of the 38 patients (three with breast cancer, two with prostate cancer and one with multiple myeloma) had also received radiotherapy. All patients had undergone oral radiography and orthopantomography prior to CT imaging; some had also had a biopsy, whereas others were to undergo biopsy within days of the CT study. CT imaging was performed with a multidetector (8-detector-row) scanner (General Electric) in all cases. Axial helical 2.5-mm-thick scans were taken without contrast administration; the images were then reconstructed (1.25-mm thickness, 0.6-mm intervals) in the coronal and sagittal planes and processed with volume rendering (VR) and three-dimensional (3D) reconstructions using an HP ADW 4.2 workstation and specific software for the study of bone and facial bone. All patients received antibiotics and underwent debridement and/or reconstructive oral surgery and a further CT study within a short time.

Results MDCT of the facial bone showed areas of bone alterations consistent with osteonecrosis in the upper jaw (Fig. 1) (21 cases) and lower jaw (Fig. 2) (17 cases). In five patients, two with breast cancer and three with multiple myeloma, there was simultaneous involvement of both the upper and lower jaw (Fig. 3). In all patients, osteonecrosis originated from the alveolar ridge involved in tooth extraction surgery and extended to the surrounding structures. On MDCT, the central portion of the lesions appeared as areas of bone sequestration, reduced in size and with lytic areas inside. In most patients (n=25), small areas of increased bone density, as in new bone apposition, could be seen peripherally to the lesions, forming intraoral spicules. In four patients with upperjaw tooth extractions, MDCT also revealed an oroantral fistula, with the maxillary sinus floor ipsilateral to the osteonecrotic lesion, as in spread by contiguity of the necrotic process itself (Fig. 4). Follow-up MDCT showed encouraging results, which should, however, be validated by longer-term monitoring. The exposed portion of the necrotic bone was infected, and the perilesional soft tissue were oedematous and inflamed. This clinical finding, which was not visualised on CT, would have been best assessed with magnetic resonance imaging (MRI), the technique of choice for the study of the soft tissues and a complementary technique in the study of bisphosphonate-related jaw osteonecrosis.

Discussion Osteonecrosis of the jaw arising as a complication of chemotherapy in edentulous patients was first reported by

Tutti i pazienti affetti da patologia neoplastica al momento dell’esame TC avevano effettuato terapia con Zoledronato, somministrato per via endovenosa, in dose di 4 mg/mese per un periodo che andava dai 12 ai 35 mesi. I pazienti con osteoporosi erano stati sottoposti a terapia con Alendronato (70 mg/settimana) o Alendronato e Clodronato (100–70 mg/settimana), somministrati per via orale. La paziente affetta da carcinoma mammario ed osteoporosi aveva effettuato terapia con Clodronato intramuscolo (300 mg/settimana) per un periodo di 12 mesi. L’evento scatenante l’osteonecrosi era stato per tutti i pazienti un’estrazione dentaria tranne che per la paziente affetta da carcinoma mammario e da osteoporosi che aveva subito invece un trattamento endodontico. Sei dei 38 pazienti (3 con carcinoma mammario, 2 con carcinoma prostatico e 1 con mieloma multiplo) erano stati sottoposti anche a trattamento radioterapico. Tutti i pazienti, prima dell’esame TC, avevano già effettuato un esame radiografico endorale ed ortopantomografico; ad alcuni di essi era già stato effettuato anche un prelievo bioptico; altri sarebbero stati sottoposti a biopsia nei giorni successivi l’esame TC. L’esame TC è stato effettuato per tutti i pazienti con una apparecchiatura TCMD (8 banchi) della General Electric (GE). Sono state effettuate scansioni TC assiali elicoidali di 2,5 mm di spessore senza somministrazione di MdC; sono state poi effettuate ricostruzioni (spessore 1,25 mm, intervallo 0,6 mm) sui piani coronale e sagittale e rielaborate, utilizzando una workstation HP ADW 4.2, e softwares dedicati per lo studio dell’osso ed il massiccio facciale, con ricostruzioni in volume rendering (VR) e 3D. Tutti pazienti sono stati trattati con terapia antibiotica, curretage e/o chirurgia orale ricostruttiva e sottoposti a controllo TC a breve distanza di tempo.

Risultati L’esame TC del massiccio facciale ha mostrato aree di alterazioni ossee strutturali riferibili ad osteonecrosi, in corrispondenza dell’osso mascellare superiore (Fig. 1) (21 casi) e inferiore (Fig. 2) (17 casi). In 5 dei pazienti esaminati, 2 con carcinoma mammario e 3 con mieloma multiplo, le lesioni osteonecrotiche coinvolgevano più segmenti ossei (mascella e mandibola) contemporaneamente (Fig. 3). In tutti i pazienti l’osteonecrosi aveva origine dalla cresta alveolare su cui si era intervenuti chirurgicamente per l’estrazione dentaria e si estendeva alle strutture circostanti. Le lesioni osteonecrotiche alla TC apparivano, nella loro porzione centrale, come zone di “sequestro osseo”, ridotte in ampiezza e con aree litiche nel contesto. Perifericamente alle lesioni erano presenti, nella maggior parte dei pazienti (25), piccole aree di addensamento osseo, come da neoapposizione, che formavano delle vere e proprie “spicule” endo-orali. In 4 dei pazienti, in cui l’estrazione dentaria era avvenuta nei segmenti mascellari superiori, si è dimostrata anche la presenza di una soluzione di continuità, una fistola oro-sinusale, con il pavimento del seno mascellare omolaterale alla lesione osteonecrotica, come da diffusione per contiguità del processo necrotico stesso (Fig. 4). 605

Schwartz in 1982 [9]. Other authors described patients with metastasis and jaw osteonecrosis after chemotherapy [2]. The toxic effects of chemotherapy for head and neck cancers, especially on vascular and bony structures, have been 606

Il follow-up dopo terapia ha mostrato risultati incoraggianti che però andrebbero validati con controlli a più ampia distanza di tempo. La parte esposta dell’osso necrotico era infetta ed i tessuti molli perilesionali erano edematosi e anch’essi coinvolti dal processo flogistico; questo rilievo clinico, non visualizzabile con la TC, sarebbe stato ben valutato con la Risonanza Magnetica (RM), tecnica d’elezione per lo studio dei tessuti molli e complementare nello studio delle osteonecrosi mascellari da uso di bifosfonati.

discussed for more than 20 years (Maduro et al., 2003 [10]). Avascular jaw necrosis was thought to be a chemically induced disease [11] and was grouped among the jaw alterations known as “phosphorus necrosis” [12]. These lesions were typically refractory to local treatment and long-term antibiotics, and protracted therapy clearly facilitated extension of the disease. In the United States, recent reports of a characteristic jawbone alteration, which never occurs after a single treatment with pamidronate or zoledronate [13], have attributed the origin of jaw osteonecrosis to the widespread use of bisphosphonates, which are recommended by the American Society of Clinical Oncology (ASCO) for the treatment of bone metastasis to breast cancer [14, 15]. The correlation between

Discussione L’osteonecrosi dell’osso mascellare come complicanza della chemioterapia in pazienti edentuli è stata descritta per la prima volta da Schwartz nel 1982 [9]. Altri autori hanno descritto di pazienti con metastasi ed osteonecrosi della mandibola in seguito a chemioterapia [2]. Sugli effetti tossici, in particolar modo di quelli vascolari ed ossei, della chemioterapia nel trattamento dei tumori della testa e del collo si discute da oltre 20 anni (Maduro et al., 2003 [10]). La necrosi avascolare della mascella si pensava fosse una malattia chimicamente indotta [11] ed era contemplata fra alterazioni mascellari designate “phosphorus necrosis” [12]. bisphosphonate use and the development of jaw osteonecrosis has been well established. The changes in bone metabolism associated with surgical treatment and implant trauma seem to be key factors in the development of osteonecrosis. A tooth extraction is the most common trigger [11, 13, 15, 608

16], even though there have been cases of lesions arising spontaneously in areas not subjected to dental extractions. The most commonly reported sites of jaw osteonecrosis are the upper jaw in 38%–80.5% of cases, the lower jaw in 14%–63% and both jaws in 5.5%–23% [2, 13, 17]; this is in agreement with the findings of our study. The diagnosis of bisphosphonate-related jaw osteonecrosis primarily relies on clinical and radiographic evidence and an accurate history. Biopsy is not always required and should be performed only if a metastatic lesion at the site of the osteonecrosis is suspected. Microbial cultures (aerobic and anaerobic) will identify the pathogens responsible for the secondary infection [5]. Imaging studies play an important role in treatment planning. Oral radiography and/or orthopantomography are the first-line techniques for the investigation of dental or maxillary disease. These allow identification and localisation of the osteonecrotic and inflammatory lesions and detection of possible associated complications. MDCT is, however, fun-

Le modificazioni del metabolismo osseo associate ad un insulto chirurgico o ad un trauma protesico sembrano essere i fattori “chiave” nello sviluppo delle osteonecrosi. Un’estrazione dentaria è la più comune causa scatenate [11, 13, 15, 16] anche se esistono casi in cui si verificano lesioni spontanee ed in zone non precedentemente interessate da avulsioni dentarie. La localizzazione delle lesioni dai dati della letteratura risulta essere come segue: nel 38%–80,5% nella mascella superiore, nel 14%–63% nella mascella inferiore e nel 5,5%–23% in entrambe le mascelle [2, 13, 17] e concorda con i risultati del nostro studio. La diagnosi di osteonecrosi da bifosfonati dovrebbe essere basata principalmente su evidenze cliniche e reperti radiografici, oltre che su una accurata anamnesi. La biopsia non è sempre necessaria e dovrebbe essere eseguita solo se esiste il sospetto di una lesione metastatica nella sede dell’osteonecrosi. Culture microbiche (aerobico ed anaerobico) consentono la identificazione dei patogeni responsabili della secondaria infezione [5]. Le indagini radiologiche rivestono un ruolo importante nella definizione del planning terapeutico. L’esame radiografico endo-orale e/o l’ortopantomografia sono le metodiche di prima istanza a cui si ricorre in caso di patologia dentaria o propriamente mascellare. Queste indagini consentono di definire la identificazione e localizzazione delle lesioni osteonecrotiche e flogistiche e la presenza di eventuali complicanze associate. damental for patient management. The intrinsic features of CR – high spatial and contrast resolution and its being ideally suited for the study of bone, associated with continuous technological evolution and the possibility of multiplanar reconstructions (MPRs), VR and 3D imaging – ensured, in our experience, a correct diagnosis and accurate staging of the osteonecrotic lesions as well as the detection of complications. Currently, no established treatment exists for this type of drug-induced lesion. Although it may seem reasonable to think that withdrawal of intravenous bisphosphonates might improve the rate of regression of the disease, this has not yet been proposed as a form of management [18], because there is no evidence of a clinical improvement of osteonecrosis [17]. This is accounted for by the fact that bisphosphonates are avidly bound to the bone matrix around the active osteoclasts and, not being metabolised, they remain there in high concentrations over long periods of time [4]. There has been much discussion about the treatment strategies to be adopted: antibiotic therapy, conservative or reconstructive surgery and hyperbaric oxygen therapy [17, 19].

Non esiste attualmente un trattamento definito per questo particolare tipo di lesioni farmaco-indotte. Anche se può sembrar ragionevole pensare che la sospensione della somministrazione intravenosa di bifosfonati possa migliorare la percentuale di regressione di tale fenomeno, non c’è nessuno che ancora lo proponga come gestione terapeutica [18]. Infatti, non è stata accertata una evidenza di un miglioramento clinico dell’osteonecrosi [17]. Questo può essere ben spiegato considerando che i bifosfonati restano avidamente legati alla matrice ossea che circonda gli osteoclasti attivi e, non essendo metabolizzati, ivi permangono a concentrazioni ossee elevate per periodi lunghi di tempo [4]. Molto si discute sulle strategie terapeutiche da adottare: terapia antibiotica, chirurgia conservativa e o ricostruttiva e terapia iperbarica [17, 19].

Jaw osteonecrosis is an established disease entity encountered in association with bisphosphonate use and following dental extractions or other oral surgical procedures. MDCT with MPR, VR and 3D reconstructions helps accurate diagnosis and provides detailed information about the structures involved and is therefore useful for appropriate treatment planning and for follow-up. However, it suffers the limitation of not being able to provide an accurate evaluation of soft tissues. As a result, MRI should be used to complement MDCT. Radiologists need to be aware of the risk of osteonecrosis in patients receiving bisphosphonate treatment and should be able to differentiate it from other bone diseases, such as osteomyelitis and osteoradionecrosis [20], even with the help of an accurate history and thorough clinical assessment.

L’osteonecrosi della mascella è ormai una accertata entità nosografica che si riscontra in associazione a terapia con bifosfonati ed in seguito ad estrazione dentaria o altra chirurgia orale. La TC-MD con ricostruzioni MPR, VR e 3D consente di farne una accurata diagnosi e di fornire precise informazioni sulle strutture coinvolte per un adeguato planning terapeutico e per il follow-up. Esiste però il limite di non poter effettuare una idonea valutazione dei tessuti molli per la quale ruolo complementare e di valido ausilio riveste la RM. I medici radiologi devono essere a conoscenza del rischio di osteonecrosi al quale vanno incontro i pazienti sottoposti a terapia con bifosfonati ed essere in grado di distinguerla da altri processi patologici dell’osso, quali osteomielite e osteoradionecrosi [20] con le quali entra in diagnosi differenziale, anche grazie ad una accurata anamnesi e ed una adeguata valutazione clinica.
