PREMESSE La terapia dell'obesitä è ancora quasi esclusivamente fondata sulle misure dietetiche; l'aspetto ]armacologico viene in/atti considerato solo in/unzione delLa possibilitä di interferire sul senso dell'appetito, e limitato perciò al ruolo di coadiuvante delle restrizioni dietetiche. Non vi è dubbio che questa impostazione terapeutica, scaturita dalla comune nozione che l'eccesso ponderaIe rappresenta l'epifenomeno dello squilibrio /ra apporto calorico e dispendio energetico, sia sempre valida, poichè la riduzione dell'alimentazione, comunque indotta, si accompagna sempre ad un caIo dell' eccesso ponderale. Gli aspetti nuovi del problema si collegano Jondamentalmente a due acquisizioni, di cui una di carattere esclusivamente clinico - - la non stretta correlazione tra variazioni del peso corporeo ed entitä di apporto calorico (KEKWICK e PAWAN 1~; CUCURACHI e Coll. 9') - l'.altra piü a carattere dottrinale, e cioè la recente sistemazione della malattia, distinta in alcune fasi differenziate per aspetti metabolici, ormonali e cardiorespiratori. Questi sono probabilmente la

The therapy of obesity is still nearly exclusively based on dietary restrictions; drugs are used primarily as means to interfere with appetite. Undoubtedly this therapeutic schema derived from the common concept that overweight is the result of an excess in caloric intake against a low energy expenditure, is still effective because the reduction in food intake, however induced, always causes a weight reduction.
Therapy of obesity should now be reconsidered in the light of more recent knowledge of the different metabolic, hormonal and cardiorespiratory stages of the disease and the lack of constant correlation between caloric intake and body weight variations (KEKwICK and PAWAN 14; CUCURACHI et al.~). These general considerations derive from the clinical observations that in the dynamic metabolic stage, obese subjects present no profound neuroendo-

conseguenza di una mutata situazione, « sindrome di adaltamento », scatenata da/attori metabolico-umorali con la partecipazione piü o meno attiva del tessuto adiposo (CucURACHI e Coll. S). A queste considerazioni generali corrisponde in/atti la constatazione che nell'obesitä in/ase dinamica non sono presenti significative deviazioni dell'equilibrio neuro-endocrino e metabolico e che la terapia dietetica è prontamente e costantemente efficace, mentre nella /ase statica della malattia, generalmente non accompagnata da iper/agia, le misure dietetiche sono meno attive, forse proprio in rfferimento al nuovo atteggiamento metabolico venutosi a determinare. Tale situazione, indipendentemente dal concorso di /attori neuro-endocrinj, riconosce il suo comune denominatore in una alterazione metabolica glucido-lipidica, caratterizzata da una prevalente utilizzazione dello zucchero a livello del tessuto adiposo, con distrazione di questo dal muscolo, che viene quindi a trovarsi nelle condizioni di utilizzare prevalentemente grassi (CucuRACHI e Coll.*). Si hanno /ondati motivi di ritenere che la resistenza al calo ponderale dei soggetti in questa rase di obesitä e la facilitä con cui essi riacquistano il loro peso, siano rfferibili all'alterazione metabolica. L'aspetto da chiarire è se inter/erendo su questa particolare situazione, si possono ottenere dei risultati efficaci e tali da confortare la nuova impostazione dottrinale. A questo scopo risponde la sperimentazione da noi eseguita con le biguanidi. E' stato di recente osservato che tali sostanze determinano nel diabetico obeso una riduzione del peso corporeo e che la loro azione, per alcuni riconducibile al semplice effetto anoressante (UNGER e CoI1. ~4, BINGLE e JAMES 2 PATEL e STOWERS 17), da altri (GRoDSKY e Coll. ~a, SCHLESS 10) è staLa attribuita ad un'attivitä specifica favorente l'utilizzazione del glucosio a livello muscolare e alla capacitä di abbassare la risposta iperinsulinemica allo stimolo
crine alterations and that dietary restrictions are promptly and constantly effective, while in the later more static stages of the disease, generally not associated with hyperphagia, the diet control is less effective, indicating that new metabolic regulations are present, possibly related to glucolipid disorders with prevalent utilization of glucose in adipose tissue compared with muscle, which in turn utilizes mainly fatty acids (CuCURACHI et al.S).
At the later stages of obesity it is therefore important to control the peripheral utilization of glucose. An effective way to handle this problem is the clinical use of hypoglycemic biguanides.
Such drugs have recently been observed to induce a loss of body weight in obese diabetic patients. This effect has been attributed to anorexia (UN¢ER et al. 24, BINGLE and JAMES 2, PATEL and STOW1~RS lv). However some AA. (GRODSKY et al la, SCHLESS 19) underline the specific activity of biguanides in increasing glucose utilization in muscle and the capacity to reduce insulin oversecretion in response to hyperglycemia, which is present in static obesi581

glucidico, f e n o m e n i peraltro alterati nell'obesitä statica. Esse in sostanza dovrebbero interferire nella ripartizione muscolo-tessuto adiposo dell'insulina e migliorare quei tests di esplorazione del m e t a b o l i s m o glucidico che nell'obeso dimostrano una ridotta assimilazione ecl utilizzazione peri/erica dello zucchero. Q u e s t e interessanti premesse non hanno a v u t o che scarse e discutibili doc u m e n t a z i o n i (DANOWSKI lO) e meritano perciò di essere ulteriormente sottoposte al vaglio sperimentale, in m o d o da dare una risposta al problema terapeutico suscitato da questi n u o v i aspetti /armaco-biologici delle biguanidi. Nella presente nota riJeriremo i prim i risultati della nostra sperimentazione nell' obesitä essenziale di due composti della serie delle biguanidi, Jenetil- e dimetil-biguanide, con particolare ri/erim e n t o ai loro effetti sul peso corporeo.

METODOLOGIA
METHODS
Le nostre ricerche sono state condotte su pazienti in controllo ambuIatoriale presso il Centro per gli Stati Disendocrini e Dismelabolici - Sezione Obesitä. Alla sperimentazione sono stati ammessi 40 soggetti di entrambi i sessi (28 donne e 12 uomini), di etä compresa tra i 18 ed i 62 anni, tutti presentanti un'obesitä essenziale, per la maggior parte con iper/agia, ed un incremento ponderaIe rispetto al proprio peso ideale non inferiore al 30 %. Sono stati perciò esclusi tutti quei casi che per caratteristiche cliniche o controlli ormonaIi potevano essern inquadrati in obesitä secondarie o miste. Nella selezione della casistica si è tenuto conto di alcune caratteristiche, quali l' etä, la durata dello stato di obesitä, il sovrappeso rispetto al peso ideale, in modo da ottenere 4 gruppi di pazienti biIanciati ed il piü possibile omogenei. DaIl'esame della tab. 1, in cui sono riportati i valori medi +_ d.s. dell'etä, durata della malattia, peso corporeo e sovrappeso in %, si nota in/atti che tali parametri non differiscono significativamente, dal punto di vista statistico, nei vari gruppi di soggetti. Anche il /abbisogno calorico basale, calcoIato con nomogramma di VANZETTI e GATTI~õ, risultava pertanto pressochè sovrapponibile per i vari gruppi, in cui i due sessi erano rappresentati con un rapporto di 6-7 donne contro 3-4 uomini per ciascun gruppo. Nelia scelta della casistica si è inoltre tenuto conto del tipo di attivitä lavorativa, in rnodo che anche il dispendio energetico dei pazienti dei singoli gruppi ammessi alla prova fosse sostanzialmen-
Out studies were performed on out-patients of the Centre for Endocrine and Metabolic Diseases-Obesity Section. Forty male and Bemale (28 women and 12 men) subjects underwent the experiment. The subjects ranged from 18 to 62 years, and they were all affected by essential obesity (an ovetweight not inferior to 30 % of their ideal weight); most of them also presented hyperphagia. Patients with non-essential or mixed obesities were excluded. In selecting the subjects the following patterns were taken into account: age, duration of obesity and overweight in order to divide the patients into 4 groups as balanced and homogeneous as possible. The mean values of age, duration of disease, body weight and overweight in %, ate teported in tab. 1. No significant differences in the statistical analysis among the different groups are present. The basal calotic requirements as calculated according to nomograms by VANZETTI and GATTI'~~, were therefore similar for the various groups in which males and females were represented in a ratio of 6-7 women to 3-4 men in each group. The type of working activity was also considered in order to select patients with similar energy expenditure in each group. Women were generally housewires, while all the men were in a sedentary work (5 clerks, 3 students, 2 retired patients and 2 dealers). Such a homogeneity among the groups was, in fact, necessary for out experimental design. According to the criteria alteady discussed in out ptevious papet (STRATA

These interesting clinical observations are however not fully documented (DAa o w s K i 10) and therefore it was decided to control further and extend clinical experimentation.
I n the present report the preliminary results of our studies on the effect of two compounds of the series of biguanides, phenethyl- and dimethyl-biguanide, in patients with essential obesity are described.

te analogo: la generalitä delle donne era costituita da casalinghe, mentre gli uomini svolgevano tutti un lavoro di tipo sedentario (5 erano impiegati, 3 studenti, 2 pensionati e 2 commercianti). Tale omogeneitä tra i gruppi era in/atti necessaria perchè il disegno sperimentale prevedeva, in base ai criteri ri/eriti e discussi in un nostro precedente lavoro (STRATA e ColI. ~~), l'assegnazione di una dieta ipocalorica standard, aggirantesi sulIe 1.200 calorie distribuite in 3 pasti giornalieri *, in associazione a 4 diversi tipi di trattamento /armacologico. Il paziente veniva perciò assegnato ad 1 dei 4 gruppi e, poichè la sperimentazione era contenuta entro limiti di tempo ben definiti e relativamente brevi (130 giorni), non si rendevano necessari ulteriori criteri discriminativi d'ordine dietetico per il singolo soggetto. Inoltre, eventuaIi interferenze del peso iniziale dei singoli soggetti nella valutazione finale dei risultati, venivano escluse esprimendo la caduta ponderale come riduzione percentuale rispetto al sovrappeso di base. A i pazienti cos~ selezionati venivano prescritti, appena espletati i controlli di base (in genere entro 4 giorni), la dieta ipocalorica standard e 1 dei 4 trattamenti previsti dal disegno sperimentale. L'esperimento, deI tipo a « cecitä semplice », è stato condotto con 4 diversi tipi di trattamento: placebo, anoressanti noti, /eniletilbiguanide e dimetilbiguanide per un periodo di circa 18 settimane, con controlli intermedi a 35, 70 e 100 giorni, e controllo finale dopo 130 giorni. I/armaci usati sono statt i seguenti:

The experiments were carried out for relatively short periods of time (130 days), and further discriminative criteria of dietetic order for the individual subjects were not necessary. In addition possible interferences with the initial weight of individual subjects in the final evaluation of results were excluded by expressing the weight loss as percentage reduction in regards to the basal overweight. As soon as the basal controls were carried out (generally within 4 days), the standard hypocaloric diet and 1 of the 4 treatment according to the experimental design were prescribed for the selected patients. The « simple blind » experiment was carried out by 4 different kinds of treatment as above mentioned: placebo, anorectic drugs, phenethylbiguanide and dimethylbiguanide for a period of about 18 weeks, with intermediate controls on the 35th, 70th, 100th day and final control after 130 days. The following drugs were used:
1) placebo, costituito da compresse contraddistinte da una sigla e contenenti ciascuna talco e amido ana g 0,2 (numero 10 casi);
1) placebo, composed of marked tablets each containing talc and starch ana g 0.2 (10 subjects);
2) anoressanti: a) clor/entermina cloridrato, compresse da mg 75 (numero 3 casi); b) dietilpropione cloridrato, compresse da mg 75 (numero 4 casi); c) fentermina, sotto forma di resinato, capsule da mg 15 di principio attivo (numero 4 casi);
2) anorectic drugs: a) chlorphentermine hydrochloride, tablets of mg 75 (3 subjects); b) diethylpropion hydrochloride, tablets of mg 75 (4 subjects); c) phentermine as resinate, capsules of mg 15 of active drug (4 subjects);
3) feniletilbiguanide ad azione istantanea- ritardo in capsule da g 0,05 (numero 10 casi) ;
3) prompt-long acting phenethylbiguanide, in tablets of g 0.05 (10 subjects);
4) dimetilbiguanide, ad azione istantanea-ritardo in capsule da g 0,250 (numero 9 casi).

L'assegnazione del trattamento è stata fatta secondo l'ordine di elencazione dei /armaci, procedendo dal placebo alla dimetilbiguanide, a mano a mano che i pazienti si presentavano alla nostra osservazione ed erano ritenuti idonei; al termine, e cioè dal quinto paziente, veniva ripetuto lo stesso ordine di assegnazione, e cos~ via. L'attivitä terapeutica espIicata dai quattro differenti tipi di trattamento, è stata valutata in base alla riduzione ponderale osservata nei vari gruppi di pazienti esaminati; i criteri seguiti sono statt da not giä riferiti e discussi in un precedente lavoro (STRArA e CoII. 2~). Nella valutazione deIl'attivitä terapeutica dei diversi /armaci considerati, si è anche tenuto conto del loro eventuale effetto anoressante (toIIeranza alla dieta ipocalorica), codificando la « sensazione di /ame » comparsa nei pazienti assoggettati alIa restrizione dietetica e ai diversi tipi di trattamento, come segue: molto intensa, 4 punti; intensa, 3 punti; discreta, 2 punti; lieve, 1 punto. L'assegnazione del punteggio alla sensazione di farne ha reso cos} disponibili delle entitä numeriche in grado di consentire la successiva elaborazione statistica delle variazioni di tale parametro, onde ottenere dei termini piü precisi di confronto tra i vari farmaci studiati (STI~ATA e Coll.'~2). Per avere un quadro piü completo nella formuIazione del giudizio sull'attivitä ed e//icacia dei vari farmaci impiegati, abbiamo anche seguito il comportamento di alcune prove di esplorazione del metabolismo glucidico. Abbiamo cioè esaminato la tolleranza al carico di glucosio per os e l'assimilazione glucidica con il test di Conard, prima e alla fine del trattamento. E' stata infine presa costantemente in considerazione l'eventuale comparsa di assuefazione o di spiacevoli fenomeni collaterali (nei loro caratteri di natura, entitä, gravitä, durata, /requenza ed epoca di comparsa) nel corso dei vari trattamenti. Ogni paziente ammesso alla prova era informato che il farmaco associato alla restrizione dietetica « serviva a ridurre la sua sensazione di farne e a ~avorire la perdita di peso ». Il placebo e gli anoressanti sono statt somministrati in dose di 1 compressa al giorno al mattino, durante la prima colazione; entrambe le biguanidi sono state somministrate 1 h prima dei due pasti principali (pranzo e cena) in dosi singole di una capsula. Le variazioni ponderali sono state rilevate sempre con la stessa bilancia e cercando di controllare i pazienti in condizioni identiche (abiti, ora, evacuazioni) a quelle iniziali. Per la successiva elaborazione dei dati, le modißcazioni deI peso eorporea sono state espresse come riduzione percentuaIe del sovrappeso iniziaIe. La « sensazione di fame» è stata espressa secondo la codificazione precedentemente riferita, con un punteggio variabile da 1 a 4. La

The treatment was prescribed according to the enumerative order of the drugs from placebo to dimethylbiguanide, as the patients came under out observation and were considered suitable; at the end, that is to say, from the 5th patient, the same prescription order was repeated, and so on. The therapeutic activity showed by the 4 different treatments has been evaluated on the grounds of the weight Ioss observed in the various groups of patients; the criteria taken into consideration in this respect have been discussed previously (STtATA et al.e2). In the evaluation of therapeutic activity of the four different treatments their possible anorectic effect (tolerance to the hypocaloric diet) was also taken into account, codifying the « hunger sensation » in the patients undergoing the dietetic restrictions and the various treatments as follows: very strong, 4 points; strong, 3 points; moderate, 2 points; mild, 1 point. The scoring of the « hunger sensation » supplied us quantitative data, which statistically evaluated gave in more accurate terms a comparison between the various drugs employed (STRATA et al.2"). For a more complete evaluation picture of the activity and effectiveness of the various drugs, some tests of glucose metabolism were made. The oral glucose tolerance test and the peripheral glucose uptake through the Conard test, both at the beginning and at the end of the treatment, were examined. Finally we constantly considered the possible formation of habit or occurrence of disagreable side effects (in their nature, entity, gravity, duration, incidence and onset time) in the course of the different treatments. Each patient submitted to the test was informed that the drug associated with the dietetic measures was intended to reduce bis « hunger sensation » to favour a weight loss. The dose of placebo and anorectic drugs was of 1 tablet daily to be taken at breakfast; both biguanides were administered 1 h before the two main meals (lunch and supper) in single doses of one capsule or tablet. The body weight variations were always taken in controlled conditions (clothes, time of the day, evacuations). The variations of body weight were calculated as percentage reduction of the starting overweight.
The <~hunger sensation » was reported according to the previously mentioned codification, with a variable score from 1 to 4. Gly-

glicemia, dopo carico di glucosio per os (1 g/kg di peso corporeo) e test di assimilazione glucidica (secondo CONARD e CoIl. ~), controllati, come si è detto, inizialmente al termine del trattamento, è stata determinata con il metodo di Hagedorn-Jensen.

RESULTS
Nella tab. 1 sono ri/erite le variazioni dell' eccesso ponderale (V. M. ± I3. S.) ottenute negli obesi con i 4 trattamenti (placebo, anoressanti, /en/ormina e met/ormina) in combinazione con la restrizione dietetica. Dall'esame della tabella si può notare che in tutti i gruppi di pazienti si verifica una costante perdita di peso, giä ben evidente al primo controllo dopo 35 giorni di trattamento, e che questa continua in modo progressivo e quasi lineare (fig. 1) fino al controllo finale a 130 giorni. L'entitä del fenomeno tuttavia è lievemente diversa nei vari gruppi; la riduzione percentuale del sovrappeso è in/atti un po' piü accentuata nei casi
Tab. 1 shows the variations (M. V. + S . D . ) observed in our obese subjects during treatment (placebo, anorectic drugs, p h e n f o r m i n and metformin) combined with dietetic measures. I t will be seen from this table that a constant weight loss in all the groups is already present at the first control after 35 days of treatment and it is progressive and almost linear (fig. 1) till the last control after 130 days.
However, body weight loss is slightly different in the various groups; percentage reduction of overweight is, in fact, more marked in the patients undergoing

trattati con anoressanti e placebo rispetto a quelli trattati con le biguanidi; la differenza ai vari tempi di controllo, rispetto al gruppo trattato con placebo, non risulta comunque mai signißcativa all'indagine statistica, Gli effetti esplicati dal placebo, gli anoressanti e le biguanidi, sulla « sensazione di /ame » comparsa nei pazienti dopo la restrizione dietetica, sono riassunti (V. M. ± D . S . ) nella tab. 2 e graficamente rappresentati nella fig. 2. Come si può osservare dalla tabella e ancor meglio dal grafico, inizialmente tutti i tipi di trattamento determinano un'evidente riduzione della « sensazione di /ame ». Dopo le prime cinque settimane di tratt.amento (primo controL lo), quasi tutti i pazienti che, come si è detto, erano stati all'inizio informati che le compresse da assumere servivano a ridurre la sensazione di fame e a favorire la perdita di peso, hanno ri/erito infatti una netta riduzione dell'appeti586
placebo and anorectic drug treatments, when compared with patients treated with biguanides; the difference at the various controls in the time compared with the placebo treated group is not, however, statistically significant. The effects of placebo, anorectic drugs and biguanides on the « hunger sensation » which appeared in the patients after the dietetic restriction, are summarized (M. V. ± S.D.) in tab. 2 and graphically represented in fig. 2. Initially all the treatments caused an evident decrease in « hunger sensation ». After the first 5 weeks of treatment (first control) nearly all the patients initially informed, as it was said, that tablets to be taken were intended to reduce the « hunger sensation » and to favour the weight loss, reported, in fact, a n e t reduction in the appetite, such as to make possible a satisfactory tolerance of the dietetic restrictive measures. In the anorectic drugs treated group the effect was

Nel gruppo dei trattati con anoressanti il fenomeno si è reso piü palese, però non in modo significativamente diverso dagli altri gruppi (tab. 2). In 70 ° giornata invece, mentre l'azione degli anoressanti si accentua ulteriormente, permane pressochè invariata quella delle biguanidi, tanto da non differire in modo significativo dal gruppo precedente (0,2 > P > 0,1; fig. 2), e si riduce chiaramente quella del placebo a livelli statisticamente significativi nei confronti degli anoressanti (0,02 > P > 0,01; tab. 2). A1 successivo controllo, dopo circa 3 mesi di trattamento (100 giornj, anche i pazienti in terapia con biguanidi hanno denunciato un netto aumento della sensazione di farne, analogamente al gruppo trattato con placebo; al contrario, i casi trattati con anoressanti hanno continuato regolarmente a riferire una chiara riduzione del loro appetito, si da permettere una buona tollerabilitä della restrizione dietetica imposta. Si può pertanto presumere che la riduzione della sensazione di farne, osservata con le biguanidi nelle prime ~asi di trattamento, sia riferibile ad un effetto placebo, che si esaurisce in mo-

more evident but not significantly different from the other groups (tab. 2). O n the 70th day, on the contrary, the action of anorectic drugs was further enhanced, the effect of the biguanides was alrnost the same hut not such as to differ significantly from the previous group (0.2 > P > 0.1; fig. 2) while the placebo effect was clearly reduced at statistically significant levels in comparison with anorectic drugs (0.02 > P > 0.01; tab. 2). At the further control (100 days) also the patients undergoing the biguanides-therapy reported a net increase in the « hunger sensation » similar to that of the placebo-treated group; contrarily patients treated with anorectic drugs constantly referred a net reduction in the appetite as such and a satisfactory tolerance of the imposed dietetic restrictions was possible. Therefore we can assume that the reduction in « hunger sensation » reported with biguanides in the first stages of treatment is to be considered a placebo effect exhausted in a relatively short time, likewise happened in the placebo-treated group. After 130 days these effects became more evident so that in patients submitted to placebo and biguanides the « hunger sensation » increases again

do relativamente breve, analogamente a quanto si è veriflcato nel gruppo trattato con solo placebo. Dopo 130 giorni il /enomeno si rende ancor piü evidente, tanto che, nei pazienti in trattamento con p]acebo e biguanidi, la sensazione di fame ritorna quasi ai valori iniziali, mentre persiste significativamente ridotta nei pazienti trattati con anoressanti (tab. 2; fig. 2). Il comportamento medio della curva glicemica da carico per os ( O G T T ) e del coefficiente di assimilazione glucidica ( I G T T ) nei 4 gruppi di obesi, prima e dopo 130 giorni di trattamento, è riportato nella tab. 3 e nelle figg. 3 e 4. La curva glicemica, chiaramente alferata all'inizio, subisce evidenti modificazioni con tendenza alla normalizzazione con i vari trattamenti. The mean behaviour of the oral glucose tolerance test (OGTT) and of the intravenous glucose tolerance test (IGTT) in the 4 groups of obese patients before and after 130 days of treatment, is reported in tab. 3 and fig. 3 and 4. OGTT at the beginning is clearly abnormal, and it undergoes evident improvement approaching a normalization after the 4 different treatments. dente a 90 min, ma soprattutto a I20 (P < 0,001) e 180 min (fig. 3 ) ] è però praticamente della stessa entitä nei quattro gruppi di pazienti; non risultano perciò mai differenze significative rispetto a! gruppo trattato con solo placebo ( tab. 39. Anche il coeßiciente di assimilazione glucidica, decisamente ridotto inizialmente, denuncia un chiaro e significativo aumento dopo 130 giorni di terapia (fig. 4), però è sostanzialmente analogo nei 4 gruppi di obesi (tab. 3) e tale da escludere influenze legate ad un tipo di trattamento piuttosto che ad un altro. Pur senza m.ai raggiungere la normalitä, il coefficiente di assimilazione glucidica migliora in/atti allo stesso

Infine ci sembra interessante segnalare che in nessun caso, con i vari Jarmaci impiegati, sono stati osservati fenomeni collaterali di rilievo: sia la tollerabilitä locale (tubo gastro-enterico) che generale, è stata sempre ottima, tanto con i tre anoressanti che con le biguanidi (/en/ormina e metformina).
Lastly we wish to point out that with the various drugs no important side effect has been observed in any patient with the different drugs employed. Both local and general tolerance was always excellent, both using anorectic drugs and biguanides (phenformin and metformin).

Dall'esame dei risultati della sperimentazione da noi condotLa, si può concludere che le biguanidi (fenetil- e dimetil-biguanide ritardo) non influenzano il calo ponderale degli obesi, con normale o ridotta tolleranza agli idrati di carbonio, ottenuto con una dieta standard di 1.200 calorie. Il comportamento del peso corporeo, calcolato come riduzione percentuale dell'eccesso ponderale rispetto al proprio peso teorico, è infatti approssimativamente analogo, e comunque non differente all'analisi statistica a quello osservato con la semplice restrizione dietetica; la riduzione ponderale è invece lievemente inferiore a quella riscontrata con anoressanti noti. Examining the resu!ts of our investigations we can conclude that biguanides (phenethyl- and dimethylbiguanides) have no effect on the body weight loss of obese subjects, with normal or impaired carbohydrates tolerance, submitted to a standard diet of 1,200 calofies. The body weight variations evaluated as a per cent reduction of the overweight as to the ideal weight is, in fact, approximately similar and, anyway, different at the statistical analysis from the one obtained by a simple dietetic restriction; weight reduction is, on the contrary, slightly lower when compared with that achieved with anorectic drugs. i quali hanno dimostrato, in una ricerca « a doppio cieco », un calo ponderale della stessa entitä in due gruppi di obesi sottoposti ad una dieta ipocalorica (1.000 calorie) e trattati rispettivamente con placebo e fenformina per un periodo di 15 settimane. Analoghi risultati sono anche derivati da ricerche su soggetti diabetici obesi, quando le variazioni del peso corporeo da fenformina sono state valutate comparativamente con l'impiego di placebo e di una dieta controlLata (ABRAMSON e ARKY 1). Queste conclusioni non confermano alcuni, ed in particolare i primi dati della letteratura, in base ai quali era stata prospettata la possibilitä di un efficace impiego delle biguanidi nel trattamento degli eccessi ponderali. E' da rilevare però che tali osservazioni si rfferiscono in genere allo studio di soggetti diabetici obesi a dieta non controllata (LIEBERMEISTER e Col1.15; SCHWARTZ e Coll. 2o) e per di piü senza un confronto con sostanze inerti o altri farmaci (PATEL e STOWEtS 17). Gli effetti sono stati valutati sol~anto in riferimento al dosaggio del /armaco, ed è stata dimostrata la mancanza di un rapporto dose/riduzione ponderale (PATEL e STOWERS17). Non vi è dubbio che la validitä di quei rilievi sia quanto mai discutibile, perchè inficiati da un'impostazione metodologica non corretta e non aderente alla complessitä del problema sperimentale. E' sufficiente ricordare - - in altra sede abbiamo piü dettagliatamente ri]erito sui possibili errori di valutazione dei /armaci anoressanti (STRaWA e Coll. ~2) __ che basta richiamare l'attenzione del paziente sul senso di appetito e sul peso corporeo, perchè questi spontaneamente riduca l'apporto calorico. Nel caso particolare della /enformina, interviene anche il /attore « tolleranza del medicamento », e si comprende come sia effettivamente di/ficile sceverare quanto è da attribuire alle azioni secondarie - - disturbi gastroenterici - - del farmaco. E' presumibile che la stessa azione anoressante della ]enformina, da alcuni AA. (UNGER e Coll.~~; BINGLE

These AA. observed, in a « double blind » experiment, a similar body weight reduction in two groups of obese pätients undergoing a hypocaloric diet (1,000 calories) and at the same time placebo and phenformin for 15 weeks. The same results were also observed in diabetic obese subjects when comparatively evaluated with those obtained by placebo and controlled diet (ABRAMSON and ARKY 1). These conclusions do not, however, confirm some results, and particularly the first ones reported in the literature, on the grounds of which a possibility of an effective use of biguanides in the treatment of overweight was pointed out. But it is to be noted that those observations generally refer to obese diabetic subjects on uncontrolled diets (LIEBERMEISTER et al.~~; SCH\VARTZ et al. 2°) and furthermore without comparison with placebo or other drugs (PATEL and STOWeRS 17).
As pointed out in a previous paper by us on the possible errors in the evaluation of anorectic drugs (STRATA et al.22), it is often sufficient to call the patients' attention to their sense of apperlte and to their body weight to have a spontaneous reduction in their caloric supply. In the case of phenformin, the reduction of caloric intake may also sometimes be ascribed to the gastro-enteric troubles provoked by the drug. The anorectic activity of phenformin, described by some AA. (UNGER et al.24; BINGLE and JAMES 2; PATEL and STOWERS 1«) and indicated as a cause of weight loss and even observed by us in a non-controlled previous trial (Cu-

e JAMES 2; PATEL e STOWERS 1~) indicata come responsabile della riduzione ponderaIe, ed anche da noi stessi osservata in ricerche preliminari non controllate (CucuRACHI e ColL 8), sia da ascrivere a questi effetti secondari, peraltro segnalati sempre come spiacevoli, nel trattamento ipoglicemizzante del diabete. L'impiego, da noi effettuato, di biguanidi-ritardo, con effetti secondari, cioè nettamente attenuati in rapporto alla liberazione del /armaco a livello del tubo gastroenterico, ci ha permesso di rilevare che queste sostanze non possiedono una specifica azione anoressante. Le variazioni del senso di appetito degli obesi da noi trat~ati con le biguanidi corrispondono a quelle osservate con il placebo, mentre sono nettamente differenti da quelle rilevate con anoressanti noti. E' da ritenere pertanto quanto mai improbabile un' azione anoressante delle biguanidi e non ci sembra, in base ai dati della Ietteratura che, nelle stesse casistiche con risultati positivi, la riduzione ponderale possa essere ascritta a questo effetto. In/atti CLARKe e DUNC&N a non hanno riscontrato differenze nel peso medio finale tra soggetti che avevano presentato o no anoressia in corso di trattamento con biguanidi. Che l'effetto dimagrante delle biguanidi si esplichi con un meccanismo pik complesso di interferenze metaboliche, rimane a nostro avviso una semplice ipotesi di lavoro. L'osservazione che la riduzione del peso corporeo sia piü evidente nei diabetici obesi che nei semplici obesi (WE5LER ~e), e cioè in quei soggetti in cui è sicuramente alterato il metabolismo glucidico, non rappresenta elemento di prova in favore della tesi enunciata. Infatti nello stesso obeso, almeno in/ase statica, sono quasi sempre dimostrabili quelle alterazioni del meaabolismo glucidico (ridotta assimilazione del glucosio, iposensibilitä all'insulina, risposta iperinsulinemica allo stimolo glucidico) sulle quali proprio si dovrebbe esplicare la azione delle biguanidi. Our use of long-acting biguanides with less side effects allowed us to observe that these substances have no specific anorectic action. The variations in the appetite of out patients treated with biguanides were similar to those obserred with placebo, while clearly different from those obtained with anorectic drugs. Therefore we do not believe in a true anorectic effect of biguanides. On the other hand, CLARKE and DUNCAN 6 have not observed any difference in the mean final body weight between subjects with or without anorexia during biguanide treatment.
The fact that the body weight reduction of biguanides occurs with a more complex mechanism, such as metabolic interferences, is, in our opinion, a simple working hypothesis. Observations that body weight reduction is more evident in diabetic obese subjects than in simple obese ones (WELLER 26) is not evidence in favour of the forrnulated thesis. In fact, in the same obese subjects, at least in a static stage, those alterations in glucose metabolism (decreased glucose tolerance, low insulin sensitivity, insulin oversecretion in response to hyperglycemia) on which biguanides are supposed to act, are nearly always present, The demonstration itself, that phenformin reduces insulin oversecretion (GRODSKY et al.la; BOSHELL et al. a) or that phen593

posto solo teorico per l'utile impiego di questi Jarmaci negli obesi. Infatti, se le biguanidi fossero effettivamente in grado di modificare quelle situazioni metaboliche, peraltro tipiche dell'obesitä (CucURACHI e Coll. S), noi dovremmo almeno assistere alla normalizzazione del quadro metabolico glucidico. In effetti, dopo 18 settimane di trattamento con biguanidi, abbiamo osservato un netto miglioramento della tolleranza agli idrati di carbonio e d e l coefficiente di assimilazione glucidica, ma non differente da quello riscontrato in soggetti obesi sottoposti alla restrizione dietetica sola o associata a farmaci anoressanti. Del resto i dati della letteratura (LIEBEr~MEISTER e Coll.~~; GRODSKY e Co!l.~'~; BUTTERFIELD e Coll.~'; WILANSKY e Coll. 2~; BOSHELL e ColI. ~) sono piuttosto contrasLanti in merito all'influenza delle biguanidi sulla curva gIicemica da carico orale di glucosio. Inoltre, ed il rilievo ci sembra piü interessante, il coefficiente di assimilazione glucidica presenta lo stesso grado di miglioramento nei vari gruppi di soggetti sottoposti a diverso trattamento. Si tratta comunque di effetti che saranno da noi piü dettagliatamente esaminati in altra sede. Vogliamo qui solo richiamare l'attenzione sul /atto che nell'obeso una riduzione ponderale, per quanto modesta, si accompagna ad una normalizzazione della tolleranza agli idrati di carbonio, ma non del ridotto 594

LOW ~)] do not necessarily support the use of this drug in obesity. In fact, if biguanides were effectively able to improve those metabolic conditions typical of obesity (CucuRACHI et. al. s) at least a normalization in glucose metabolism should be observed. In effect, after a 18 weeks biguanides-treatment we observed a n e t improvement in glucose tolerance and in peripheral glucose uptake, but no difference when compared with that obtained in obese subjects submitted to dietetic restrictions combined with placebo or anorectic drugs. Several reports in literature (LIEBERMEISTER et al.~5; GRODSKY et al. la; BUTTERFIELD et al.4; WILANSKY et al.27; BOSH~LL et al. a) are somewhat contrasting about the biguanides effects on the oral glucose tolerance test. Furthermore, and this remark seems more interesting to us, the improvement in peripheraI glucose uptake is similar in the various groups of subjects who underwent the 4 different treatments.
However, these effects will be examined more in detail by us in another occasion. Hefe, we only intend to point out the fact that obese patients' body weight reduction, although mild, is associated with a normalization in carbohydrates tolerance but not in the low peripheral glucose assimiIation, which is

Per concludere possiamo affermare che le biguanidi non hanno un'azione dimagrante, o almeno non potenziano gli effetti della semplice restrizione dietetica. Se tali sostanze siano in grado di riportare aUa norma il quadro metabolico dell'obesitä, come lascerebbero prospettare le ricerche di WILANSKY e Coll. 2~, di FALUDI le e di BUTTERFIELD e Coll. t', o nostre preliminari esperienze (CucuRACHI e Co11.8), oppure se la loro azione si esplichi soltanto su particolari situazioni metaboliche, come quella del diabetico obeso o dell'obeso con storia /amiliare di diabete (WELLER 26., ROGINSKY e BARNETT is), e un problema che aarä da noi discusso quando analizzeremo i risultati della sperimentazione su una casistica pih ampia, selezionata e seguita piü a lungo.
To conclude we can state that biguanides have no effect on b o d y weight or, at least, they do n o t increase the effects of the dietetic restriction alone. I t is still uncertain if such drugs are able to normalize the metabolic p a t t e r n of obesity as suggested by several studies ( W I LANSKY et al.27; FALUDI 12; BUTTERFleCD et al. 4) and our previous trials (CucuRACHI et al.S), or if their action is present only in special metabolic conditions, such as diabetic obese tmtients or obese subjects w i t h d i a b e t i c family histories (WELLER26; ROGINSKY and BARNETT lS). This is a p r o b l e m which will be discussed when our experimental results obtained in a larger n u m b e r of selected and longer studied patients will be available.
RIASSUNTO E' stata condotta una ricerca controllata per valutare l'azione delle biguanidi (fenetil- e dimetilbiguanide) sulla riduzione del peso corporeo di soggetti obesi con normale o ridotta tolleranza agli idrati di carbonio. Dopo 130 giorni di trattamento la riduzione dell'eccesso ponderale è analoga a quella ottenuta con placebo e anoressanti. Le biguanidi non esplicano alcun effetto sulla sensazione di farne; non sembrano infine esercitare influenze significative sul miglioramento della tolleranza agli idrati di carbonio, osservato dopo il calo ponderale. RESUME Les AA. ont éffectué une recherche controlée pour la valutation de l'action des biguanides (fenetyl- et dimetyl-biguanide) sur la réduction du poids des sujets obèses avec une tolérance aux carbohydrates normale on réduite. Après 130 jours de traitement la réduction de l'excés pondérale est analogue ä celle obtenue avec un placebo et des substances anoréxiques. Les biguanides n'ont aucun effet sur la sensation de faim; elles ne semblent pas excercer une influence déterminante sur l'amélioration de la tolérance aux carbohydrates, observée après la chute ponderale. RESUMEN Se realizó una investigación controlada para valorar la acción de las biguanidas (fenetil y dimetil-biguanida) sobre la disminución del peso corporal de sujetos obesos con una tolerancia normal o reducida a los hidratos de carbono. Después de 130 dfas de tratamiento, la reducción del exceso ponderal es anäloga a la que se consigue con placebo y con anorexantes. Las biguanidas no ejercen ningün efecto sobre la sensación de apetito; tampoco parecen ejercer influencias significativas sobre el mejoramiento de la tolerancia a los hidratos de carbono, observado después del descenso ponderal. 595
OBESITY TREATMENTWITH BIGUANIDES ZUSAMMENFASSUNG Es wurde eine kontrollierte Untersuchung durchgeführt, um die Wirkung der Biguanide (Phenethylbiguanid, Dimethylbiguanid) auf die Reduktion des Körpergewichtes, von fettleibigen Patienten mit normaler oder reduzierter Kohlenhydrattoleranz einzuschätzen. Nach 130 Behandlungstagen entspricht die Reduktion des Gewichtsüberschusses derjenigen eines Placebo und Appetitzüglers. Die Biguanide entfalten keine Wirkung auf das Hungergefühl, schließlich scheinen sie keinen signifikanten Einfluß auf die Verbesserung der Kohlenhydrattoleranz auszuüben, die man nach der Gewichtsreduktion beobachtet.
SUMMARY The effects of biguanides (phenethyl- and dimethylbiguanide) on the body weight reduction, were evaluated, by a controlled clinical trial, in obeses with normal or impaired glucose tolerance. Biguanides, after a 130 days treatment, caused a loss overweight as well as placebo or anorectic drugs and of the same value; besides they had no appetite suppressant effect. At last, biguanides seem to exert no significant activity on the glucose tolerance improvement, observed in the obese patients after simple body weight reduction.
