L'intervento di gastroresezione con anastomosi gastro-duodenale o gastro-digiunale si complica in circa il 5 % dei pazienti con la sindrome post-prandiale tardiva (CAMPANACCI e MAGNANI~). Questa è cos~ defin#a perché caratterizzata da disturbi insorgenti 2-4 h dopo i pasti, specie se ricchi di carboidrati. I pazienti lamentano astenia, senso di lame, sudorazione fredda, tremore, tachicardia, ipotensione fino al deliquio. Questa sintomatologia, per l'evidente analogia con i quadri clinici causati da eccessive dosi di insulina, è stata da tempo attribuita ad un abbassamento della glicemia, sicché oggi viene comunemente indicata come « sindrome ipoglicemica o post-prandiale tardiva» o, impropriamente, come «dumping syndrome tardiva» del gastroresecato. Questo quadro va tenuto distinto dalla vera « dumping syndrome», che insorge nella rase immediatamente post-prandiale ed è riconducibile ad un imper]etto funzionamento dell' anastomosi (FERRI~R lo). La patogenesi deIla sindrome ipoglicemica tardiva del gastroresecato è tuttora oggetto di discussione.

GASTRECTOMY I-IYPOGLYCEMIC SYNDROME. Gastric resection with gastro-
duodenal or gästrojejunal anastomosis is complicated in about 5 % of cases by the late postprandial syndrome (CAMPANACCI and MAGNANI3). It is termed thus because it is characterised by disturbances arising 2-4 hrs after meals, especially after those rich in carbohydrates. Patients complain of asthenia, a sensation of hunger, cold sweats, trembling, tachycardia and hypotension to the point of fainting. This symptom-complex, so obviously akin to the clinical patterns due to insulin overdosage, has long been attributed to a fall in the blood glucose, so that now it is commonly called the 'late hypoglycemic or postprandial syndrome' or improperly the 'late dumping syndrome' of gastrectomized patients. This pattern taust be kept distinct from the true 'dumping syndrome', which occurs in the immediate postprandial period and is due to imperfect function of the anastomosis (FERRIER 10). The pathogenesis of the late hypoglycemic syndrome following gastric resection is still a subject of debate.

Una prima teoria si basava sull'osservazione che nel gastroresecato in rase post-prandiale si verifica una iperglicemia piü precoce e accentuata che nel soggetto normale (lag storage effect), segu~ta da una rase ipoglicemica altrettanto rapida e marcata. Questa ~ase ipoglicemica tardiva veniva attribuita ad una iperinsulinemia causata dal rapido aumento iniziale della glicemia (teoria dell'iperglicemia ipoglicemizzante sostenuta da ROTH e Colld ~, LONGUEVILLE14 e MIROUZE e Coll. 19). Recentemente è stato però osservato che diversi entero-ormonL quali la secretina, la pancreozimina e l'enteroglucagone, esplicano un'azione insulino-stimolante (SaMoLS e MARKS as, DUPRg e Coll. 6'7, UNGER e ColldS). Pertanto, è stata avanzata l'ipotesi che la sindrome ipoglicemica tardiva del gastroresecato sia legata ad un iperinsulinismo provocato non tanto dall'iperglicemia, quanto da una abnorme produzione di questi entero-ormoni (HoLDSWORTH e Coll/~, VEVERBRANTS e Coll.~°). L'obiettivo della nostra ricerca è stato quello di verificare la validitä delle teorie patogenetiche sopra esposte. A tale scopo abbiamo studiato il comportamento della glicemia e della insulinemia dopo carico di glucosio p.o. e i.v. in un gruppo di gastroresecati con sindrome ipoglicemica tardiva. Per conBronto, sono stati studiati anche alcuni soggetti gastroresecati non affetti da sindrome ipoglicemica e alcuni soggetti normali.
One theory was based on the finding that patients who have undergone gastric resection experience hyperglycemia earlier in the postprandial phase and to a more marked degree than is the case with normals (lag storage effect), followed by an equally swift and marked hypoglycemia. This hypoglycemic phase is attributed to hyperinsulinemia due to the rapid initial rise in the blood glucose (the hypoglycemic hyperglycemia theory upheld by ROTH et al. 2~, LONGUEVILLE 14, and MIROUZE et a1.19). Recently, however, it has been obserred that several gut hormones such as secretin, pancreozymin and enteroglucagon exert an insulin-stimulafing action (SAMOLS and MARKS23, DUPRÉ et alfi'7, UN¢ER et al.eS). LIence the hypothesis that the late hypoglycemic syndrome after gastric resection is related to hyperinsulinism induced not so rauch by hyperglycemia as by an abnormal production of these gut hormones (HoLDSWORTH et al. la, V E V E R B R A N T S et a12°). The aim of our study was to check the validity of the above pathogenetic theories. For this purpose we followed the trend of the plasma sugar and plasma IRI levels after oral and i.v. glucose in a series of gastric resection cases with and without the late hypoglycemic syndrome and in some normals.

MATERIAL AND METHODS
Il nostro studio è stato compiuto su I6 pazienti giä sottoposti a gastroresezione e su 10 soggetti normali di controIlo. I dati clinici relativi ai pazienti gastroresecati sono riportati nella tab. 1. Nessun paziente aveva ]amiliaritä diabetica, né presentava segni clinici di diabete. Undici erano di sesso maschile e 5 di sesso femminile e l'etä era compresa tra i 32 e i 65 anni. La gastroresezione era stata effettuata in 10 pazienti per ulcera duodenale, in 5 per ulcera gastrica, in i per doppia ulcera (gastrica e duodenale). In 11 pazienti era stata eseguita una

gastroresezione con anasxomosi gastro-digiunale termino-laterale (Billroth II), nei rimanenti 5 una gastroresezione con anastomosi gastroduodenaIe termino-terminale (Billroth I). L'intervento operatorio risaliva ad un'epoca variabile tra I e 13 anni. I soggetti gastroresecati sono stati opportunamente selezionati da una piü ampia casistica e distinti in due gruppi: il primo comprendente 7 pazienti con sindrome ipoglicemica tardiva (pazienti nn. 1-7), il secondo 9 pazienti esenti da disturbi ipoglicemici (pazienti nn. 8-I6). Tutti i soggetti gastroresecati sono stati sottoposti ad esame radiologico del tubo digerente con pasto baritato, per valutare il funzionamento dell' anastomosi. Durante il periodo della sperimentazione tutti i pazienti hanno segu~to una dieta normale, con apporto glicidico giornaliero di almeno 250 g. Le prove venivano eseguite al mattino, a digiuno, con paziente a riposo dalla sera precedente. Sia i soggetti gastroresecati che i controlli sono stati sottoposti a carico orale di glucosio ( O G T T ) (100 g disciolti in 200 ml di acqua). I prelievi di sangue venoso sono stati effettuati ai tempi O, 30, 60, 90, 120, 180 e 240 min.

These gastrectomized cases were sdected from a larger series and divided into two groups: the first comprised 7 patients with the late hypoglycemic syndrome (patients nos. 1 to 7) and the second 9 patients free from hypoglycemic disturbances (patients nos. 8 to 16). All these patients were subjected to X-ray examination of the digestive tract after a barium meal in order to assess the function of the anastomosis. During the trial period all patients were on a normal diet containing a daily ration of at least 250 g of carbohydrates. The tests were taken first thing in the morning after overnight fast and rest. Both the gastrectomized patients and the controls were given an oral glucose tolerance test (OGTT) (100 g in 200 ml of water). Venous blood samples were taken at times 0, 30, 60, 90, 120, 180, and 240 min. The hepa605

Sul plasma eparinato di ciascun carnpione sono state determinate la glicernia (rnetodo della glucoso-ossidasi) e l'insulinernia [IRI: rnetodo radioirnmunologico di Hales e Randle modificato (CREPALDIe Coll.*)]. Nei 7 pazienti del prirno gruppo e in 4 del secondo (nn. 9, 10, I1, 13) è stato effettuato anche un carico di glucosio per via i.v. (IVGTT), somrninistrando 20 g di glucosio (50 rnl di una soluzione glucosata al 40 %) in 3 min. La glicernia è stata deterrninata ai ternpi O, 3, 5, 10, 20, 30, 40, 50 e 60 min; l'insulinemia ai ternpi O, 3, 5, 10, 20, 30 e 60 min. Le risposte glicemiche ed insulinerniche dopo carico glicidico p.o. e i.v. sono stare valutate anche con il rnetodo planimetrico, che consiste nel rnisurare su carta rnillimetrata le aree delimitate dalle curve glicerniche ed insulinemiche, assurnendo corne linea di base quella corrispondente al valore glicernico ed insulinemico a digiuno (SELTZER e CollY).

RISULTATI
RESULTS
The 7 patients of the first group and 4 of the second (nos. 9, 10, 11, 13) were also given an i.v. glucose tolerance test (IVGTT) with 20 g of glucose (50 ml of a 40 % glucose solution) in 3 min. Plasma glucose was assayed at times 0, 3, 5, 10, 20, 30, 40, 50, and 60 min and IRI at times 0, 3, 5, 10, 20, 30, and 60 min. The plasma glucose and IRI responses to oral and i.v. glucose were evaluated also by the planimetric method, i.e. by measuring on graph paper the areas bounded by the blood glucose and IRI curves, taking the fasting values as baseline (SEtTZER et al}5).

Gastroresecafi con sindrome ipoglicemica tardiva Comportamento della glicemia - La curva glicemica media dopo carico orale di glucosio nei 7 pazienti gastroresecati con sindrome ipoglicemica (fig. 1 e tab. 2) è caratterizzata da un'acme di 176 ± 25 m g / I O 0 ml a 30 min, segu}to da una rapida diminuzione della glicemia fino a raggiungere, alla 3 « e alla 4 a h, i valori di 46 e 45 m g / l O 0 ml rispettivamente. La media delle aree glicemiche è di 564 ± 92 m m ~, mentre la media delle acmi glicemiche è di 197 ± 23 m g / 100 ml. Esaminando le singole curve glicemiche, è possibile rilevare che nei pazienti nn. 1, 5, 6 e 7 l'acme glicemica è particolarmente elevata, con una curva a tipo «lag storage effect ». In tutti i pazienti di questo gruppo la glicemia è scesa, tra la 3 a e la 4 a h, a valori in/eriori a 45 m g / 100 ml. In coincidenza con questa /ase ipoglicemica sono comparsi disturbi, piü o meno rilevanti, simili a quelli avvertiti dagli stessi pazienti 2-4 h dopo i pasti normali. Nei pazienti nn. 2 e 7 vi è stata

The mean plasma glucose area was 564 ----- 92 m m 2 and the mean plasma glucose peak was 197 ± 23 mg/100 ml. As will be seen from the plasma glucose curves, in patients nos. 1, 5, 6, and 7 the peak was very high with a 'lag storage effect'. In all the patients of this group the plasma glucose fell between the 3rd and 4th hrs to below 45 m g / 100 rel. During this hypoglycemic phase the patients had more or less severe disturbances similar to those they experienced 2-4 hrs after normal meals. Patients nos. 2 and 7 even lost conscious-

anche perdita di coscienza, che si è risolta solo dopo l'iniezione i.v. di glucosio.

Comportamento dell'insulinemia - La curva insulinemica media nei 7 gastroresecati con sindrome ipoglicemica (fig. 2 e tab. 2) è caratterizzata da una protratta elevazione fino ad un massimo di 128 e 127 ~U/ml, rispettivamente a 60 e 120 min. Questa risposta si differenzia nettamente da quella osservabile nei soggetti normali, sia per gli elevati livelli di insùlinemia che per la sua maggiore durata. L'area insulinemica media dopo carico orale di glucosio risulta di 1.030 + 249 mm ~, mentre la media delle acmi insulinemiche è di 178 ± 43 BU/ml. Per stabilire una correlazione tra lo stimolo glicidico e la risposta insulinemica dopo il carico di glucosio, sono stati caleolati in ciascun paziente il rapporto tra l' area insulinemica e l' area glieemica, espresse in mm ~, ed il rapporto tra l'acme insulinemica e l'acme glicemica. Questi rapporti (indici insulinogenici) risultano, nei gastroresecati con sindrome ipoglicemica tardiva, rispettivamente di 1,76 ± 0,25 e di 0,91 ± 0,22.
Trend of plasma I R I - The mean plasma IRI curve in the 7 gastrectomy cases with the hypoglycemie syndrome (fig. 2 and table 2) was marked by a prolonged rise to a peak of 128 and 127 BU/ml at 60 and 120 min respectively. This response differed sharply from that observed in normals both in the height of the IRI levels and the longer duration. The mean plasma IRI area after oral glucose was 1,030 ± 249 mm e and the mean IRI peak was 178 ± 43 BU/ml.

Comportamento della glicemia - Nei 9 gastroresecati esenti da disturbi ipoglicemici, il carico orale di glucosio ha dimostrato in 6 casi (nn. 8, 9, 10, 11, I2 e 13) un normale comportamento della glicemia (fig. 1 e tab. 3), mentre in 3 casi (nn. 14, 15 e 16) ha evidenziato una ridotta tolleranza ai carboidrati (fig. 1 e tab. 4). In questi ultimi 3 pazienti, la curva glicemica media è a tipo di diabete chimico, con acme di 296 -+- 56 mg/lO0 ml ed un valore di 180 ± 21 mg/lO0 ml alla 2 a h. Nel gruppo dei 6 gastroresecati con normale tolleranza glicidica, la media delle aree glicemiche (tab. 3) è di 415 + 85 mm ~, mentre la media delle acmi glicemiche è di 124 -+" 6 mg/lO0 rel.
Trend oB plasma glucose - In the 9 post-gastrectomy cases free from hypoglycemic disturbances the oral glucose tolerance test elicited a normal plasma glucose trend (fig. 1 and tab. 3) in 6 cases (nos. 8, 9, 10, 11, 12, and 13) and impaired tolerance (fig. 1 and tab. 4) in 3 cases (nos. 14, 15, and 16). In the latter 3 patients the mean plasma glucose was of the chemical diabetes type with a mean peak of 296 ± 56 mg/100 ml and a value of 180 ~ 21 mg/100 ml after 2 hrs. In the 6 cases with normal glucose tolerance the mean plasma glucose area (tab. 3) was 415 ± 85 mm 2 and the mean plasma glucose peak 124 ± 6 mg/ 100 ml.

To establish a correlation between the glucose stimulus and the IRI response to glucose loading, we calculated for every patient the ratio between the IRI area and the plasma glucose area, expressed in mm 2, and the ratio between the IRI peak and the plasma glucose peak. In the gastrectomized cases with the late bypoglycemic syndrome these ratios (insulinogenic indices)were 1.76 ~ 0.25 and 0.91 ± 0.22 respectively.

Nel gruppo dei 3 gastroresecati con curve diabetiche, la media delle aree glicemiche (tab. 4) è di 1.289 ± 319 mm ~, mentre la media delle acmi glicemiche è di 296 ± 56 mg/lO0 rel. Nessuno di questi 9 pazienti ha presentato ipoglicemia nella rase tardiva del carico glicidico, né, tanto meno, disturbi a questa ri/eribili.

Comportamento dell'insulinemia - Nei 6 gastroresecati con normale tolleranza glicidica, la risposta insulinemica è normale (fig. 2 e tab. 3). Dei 3 gastroresecati con ridotta tolleranza glicidica, 2 (nn. 14 e 15) presentano una risposta molto ridotta, mentre il terzo (n. I6) mostra un comportamento normale (fig. 2 e tab. 4). Nei 6 gastroresecati con normale tolleranza glicidica (tab. 3), la media delle aree insulinemiche è di 334 ± 42 mm ~, mentre quella delle acmi è di 58 ± 5 BU/ml. La media dei rapporti tra le aree insulinemiche e le aree glicemiche è di 0,95 ± 0,19, mentre quella dei rapporti tra le acmi insulinemiche e le acmi glicemiche è di 0,46 +- 0,04. Nel gruppo dei 3 gastroresecati con ridotta tolleranza glicidica (tab. 4), la media delle aree è di 286 +- 51 mm ~, mentre quella delle acmi insulinemiche è di 46 ± 19 ~tU/ml. La media dei rapporti tra le aree insulinemiche e le aree glicemiche è di 0,23 ± 0,03, mentre quella dei rapporti tra le acmi è di 0,14 ± 0,03.

Of the 3 cases with lowered glucose tolerance, two (nos. 14 and 15) presented a rauch reduced response whereas the third (no. 16) showed a normal trend (fig. 2 and tab. 4). The mean IRI area in the 6 cases with normal tolerance (tab. 3) was 334 ± 42 mm 2 and the mean peak was 58 ± 5 BU/ml. The mean ratio of IRI area to plasma glucose area was 0.95 ± 0.19 and the mean ratio of insulin peak to plasma glucose peak was 0.46 ± 0.04.

Nella tab. 5 sono con]rontati i risultati del carico orale di glucosio nei var2 gruppi di pazienti esaminati. Il gruppo A comprende 7 pazienti gastroresecati ton sindrome ipoglicemica tardiva, il gruppo B 6 pazienti gastroresecati senza sindrome ipoglicemica e con normale tolleranza glicidica, il gruppo C 3 pazienti gastroresecati senza sindrome ipoglicemica e con ridotta tolleranza glicidica, il gruppo D I0 soggetti normali di controllo.
Tab. 5 compares the results of oral glucose loading in the various groups of patients considered. Group A comprises 7 gastrectomized patients with the late hypoglycemic syndrome, group B 6 gastrectomized patients without hypoglycemic syndrome and with normal glucose tolerance, group C 3 gastrectomized patients without hypoglycemic syndrome and with lowered glucose tolerance and group D 10 normal controls.

None of these 9 patients showed evidence of hypoglycemia in the late phase of the test not did they complain of any disturbances that could be attributed to low blood sugar.
In the 3 gastrectomized patients with reduced glucose tolerance (tab. 4) the mean area was 286 ± 51 mm 2 and the mean IRI peak 46 ± 19 9U/ml. The mean of the ratlos of IRI areas to plasma glucose areas was 0.23 ± 0.03 and that between the peaks 0.14 ± 0.03.

La glicemia a 30 min nel gruppo A è significativamente maggiore che nei gruppi B e D (lag storage effect), mentre a 180 e 240 min è significativamente inferiore rispetto agli altri gruppi in esame (/ase ipoglicemica). L'area glicemica nel gruppo A è significativamente maggiore che nel gruppo D, mentre l'acme glicemica è significativamente maggiore rispetto ai gruppißeD. La risposta insulinemica al carico orale di glucosio, valutata sia come area che come massima elevazione, risulta nel gruppo A significativamente maggiore rispetto a tutti gli altri gruppi. La media dei rapporti tra area insulinemica ed area glicemica e tra acme insulinemica ed acme glicemica (indici insulinogenici) risulta nel gruppo A maggiore rispetto a tutti gli altri gruppi esaminati. È importante notare che non si rilevano differenze significative nel comportamento dei gastroresecati con normale tolleranza glicidica (gruppo B) rispetto ai soggetti normali di controllo (gruppo D). Altamente significative sono le differenze tra i gastroresecati con ridotta tolleranza glicidica (gruppo C) e gli altri gruppi. I valori glicemicL l'area glicemica e l'acme glicemica sono piü elevati, mentre la risposta insulinemica è significativamente ridotta. Il valore particolarmente basso del rapporto tra le aree insulinemica e glicemica e tra le acmi insulinemica e glicemica (rispettivamente di 0,23 ± 0,03 e di 0,14 ± 0,03) riassume questa situazione, che è l' opposto di quella osservata nei gastroresecati con sindrome ipoglicemica tardiva (gruppo A). The plasma glucose at 30 min was significantly higher in group A than in groups B and D (lag storage effect) and at 180 and 240 min significantly lower than in the other groups considered (hypoglycemic phase). The plasma glucose area of group A was significantly larger than that of group D and the plasma glucose peak significantly higher than those of groups B and D. The plasma IRI response to the oral glucose tolerance test, whether evaluated in terms of area or peak, was significantly greater in group A than in all the other groups. The mean ratlos of IRI area to blood glucose area and of IRI peak to blood glucose peak were higher in group A than in any of the other groups (insulinogenie indices). It is noteworthy that there were no significant differences of trend between the gastrectomized patients with normal glucose tolerance (group B) and the normal controls (group D). The differences between the gastrectomized cases with lowered glucose tolerance (group C) and the other groups were highly significant. The plasma glucose levels, the plasma glucose area and the plasma glucose peak were higher and the IRI response significantly reduced. The very low ratlos of IRI area to plasma glucose area and of IRI peak to plasma glucose peak (0.23 ± 0.03 and 0.14 ± 0.03 respectively) sum up this situation, the reverse of that found in the post-gastrectomy cases with the late hypoglycemie syndrome (group A). Hypoglycemia after oral glucose loading in gastrectomized patients has already been reported by several workers (GILBERT and DUNLOP 11, ADLERSBERG and HAMMERSCHLAG 1, CONN and GELTZER 3). This hypoglycemia has been regarded by the majority as due to an abnormal rise in plasma IRI (ROTH et al. 22, HOLDSWORTH et al. 13, FERRIER 10, MUGGEO et ald°). In a few post-gastrectomy cases with the hypoglycemic syndrome a change in the plasma IRI response to glucose loading has not been observed (SussMAN et al. 26, HOLDSWORTH et a1.13). These cases of hypoglycemia without hyperinsulinism are thought to be caused by failure on the part of the liver to regulate the blood glucose in the late phase of glucose loading (HOLDSWORTH and BESSER 12).
In our gastrectomized patients with the late hypoglycemic syndrome an abnormal IRI rise was observed in 5 cases after the oral glucose test while 2 showed a delay in the response. The mean of the plasma IRI areas, expressing approximately the total quantity of

linemiche, che esprimono approssimativamente la quantitä totale di insulina liberata dal pancreas durante il test, risulta in questo gruppo di pazienti significativamente maggiore di quella di tutti gli altri gruppi esaminati. Anche la media delle acmi insulinemiche è significativamente piü elevata rispetto a quella degli altri gruppi. Valutando il comportamento dei singoli pazienti, non sempre è possibile stabilire una relazione, dopo carico orale di glucosio, tra l'iperinsulinismo, l'ipoglicemia e la gravitä del quadro clinico. In base ai nostri dati, è però possibile con]ermare che la sindrome ipoglicemica tardiva del gastroresecato è riconducibile ad un iperinsulinismo. L'interpretazione di questo iperinsulinismo è controversa. Nel gruppo di gastroresecati con ipoglicemia tardiva la curva glicemica media è caratterizzata da un'acme piü elevata rispetto ai gastroresecati senza sindrome ipoglicemica e rispetto ai soggetti normali di controllo. Questa iperglicemia, segu~ta però da un rapido ritorno ai valori normali, non esprime un dffetto dell' utilizzazione perfferica del glucosio (come potrebbe ]ar pensare il termine di «lag storage effect », coniato da MACLEANlõ nel 1926), ma è dovuta ad un rapido assorbimento di questo da parte dell'intest&o. Curve glicemiche di questo tipo sono stare descritte da numerosi AA. in pazienti gastroresecati (CAMPANACCI e MAGNANI ~, R O T H e Colld ~, H O L D S WORTH e Coll. is, F E R R I E R «0, McKIDDIE e Coll. 17) e da noi sono stare osserrate in 4 pazienti (nn. I, 5, 6 e 7) gastroresecati con sindrome ipoglicemica. Anche l'area glicemica è, nel gruppo di gastroreseeati con ipoglicemia, maggiore rispetto a quella dei gastroresecati senza sindrome ipoglicemica e d e l gruppo di controllo. Tutti questi dati sembrano perciò con/ermare che l'iperglicemia iniziale sia la causa delI'iperinsulinismo, come sostenuto da ROTH e Coll. ~~ e da LONGUE-

When assessing the trend of individual patients it is not always possible to establish a relation after oral glucose between hyperinsulinism, hypoglycemia and the severity of the clinical picture. On the strength of our data, however, we are able to confirm that the late hypoglycemic syndrome of gastrectomized patients is due to hyperinsulinism. The interpretation of this hyperinsulinism is controversial. The mean plasma glucose curve in the gastrectomized patients with late hypoglycemia is characterized by a higher peak than in gastrectomized without the hypoglycemic syndrome and in controls. This hyperglycemia is, however, marked by a rapid return to normal values; it is not the expression of a defect in peripheral glucose utilization (as the term 'lag storage effect' coined by MaCLEAN1~ in 1926 might suggest) but is due to a rapid absorption of glncose by the intestine. Blood glucose curves of this type have been described by numerous investigators in postgastrectomy patients (CAMPANACCI and MAGNANI2, ROTH et al.22, HOLDSWORTH et al. 1~, FERRIER 10 MCKIDDIE et al. 1~) and we observed them in 4 of our patients (nos. 1, 5, 6, and 7) with the hypoglycemic syndrome. Likewise the plasma glucose area was greater in the cases with hypoglycemia than in the post-gastrectomy cases without hypoglycemia and in the controls. All these data seem to confirm that the initial hyperglycemia is the cause of the hyperinsulinism as maintained by ROTH et al. 22 and by LONCUEVILL~14

ta dipendenza dell'iperinsulinismo dall'iperglicemia. Infatti il carico i.v., mentre provoca un'iperglicemia piü elevata di quella prodotta dal carico orale, determina una risposta insulinemica nettamente inferiore (fig. 5). Se pol la risposta insulinemica viene valutata con il metodo planimetrico, la differenza tra il carico orale e quello i.v. risulta ancora piü evidente (fig. 6). Anche nel soggetto normale la risposta insulinemica al glucosio somministrato per via orale è maggiore di quella ottenibile allorché uguali quantitä di glucosio vengano somministrate i.v. (MclNTYRE e Coll. lô, ELRICK e Coll.S). Se poi la somministrazione del glucosio viene eseguita rapidamente e direttamente nel digiuno con sondino, la risposta insulinemica è ancora piü elevata, tanto da provocare delle vere e proprie crisi ipoglicemiche (MclNTYRE e Coll. 1~, HOLDSWORTHe Coll.lS). Simili ]enomeni possono essere anche provocati dall'infusione intradigiunale di aminoacidi e di acido cloridrico (DuPRÉ e Coll.Q. Tutte queste osservazioni si spiegano tenendo presente che dall'intestino, giä in condizioni normali, vengono elaborate sostanze che controllano la produzione di insulina. L'effetto insulinopoietico della secretina (DUI, RÉ e Coll.~), della pancreozimina (MEADE e Coll.18), della gastrina (UNGER e Colld s) e dell' enteroglucagone (SAMoLS e Colld*) è stato ormai ampiamente documentato. La produzione di questi entero-ormoni avviene normalmente quando il pasto giunge a contatto con la mucosa intestinale ed è regolata, oltre che dalla composizione e dalla quantitä del pasto, anche dalla rapiditä del riempimento intestinale (MCINTYRE e Coll.iQ. Nei gastroresecati con sindrome ipoglicemica si verißcherebbe un massivo e rapido contatto del glucosio con ampf tratti dell'intestino, e ciò comporterebbe non solo un suo piü rapido assorbimento, ma anche un'eccessiva produzione di entero-ormoni ad azione insulino-stimolante. dence of hyperinsulinism on hyperglycemia. Indeed, whilst the IVGTT induced greater hyperglycemia than the OGTT, it induced a distinctly lower insulin response (fig. 5). And when the insulin response is evaluated by the planimetric method, the difference between the IVGTT and OGTT is eren more marked (fig. 6). In normals too the IRI response to the OGTT is greater than to the IVGTT when equal quantities of glucose are administered (McINTYRE et al. 16, F&RICK et al.8). When glucose is administered quickly and directly into the jejunum by tube, the insulin response is so great as to induce genuine hypoglycemic crises (MCINTYRE et al) 6, HOLDSWORTH et ai)3). Similar phenomena can also be induced by intrajejunal infusion of amino acids and of hydrochloric acid (D•PRÉ et alfi).
All these observätions are explained by the fact that even in normal conditions substances that control insulin production are elaborated in the intestine. By now ample evidence has been provided for the insulinopoietic effect of secretin (DupRÉ et ald), of pancreozymin (MEADE et al)8), of gastrin (UNGER et al? s) and of enteroglucagon (SAMOLS et al?4). The production of these gut hormones normally occurs when food comes into contact with the intestinal mucosa and is controlled partly by the composition and size of the meal and partly by the rate of intestinal filling (MclNTYRE et al.16). In the post-gastrectomy hypoglycemic syndrome masses of glucose are thought to come quickly into contact with large segments of the intestine and consequently there is not only faster absorption of glucose but also overproduction of gut hormones possessing insulin-stimulant action. It would be this overproduction of insulin, at the stage when 625

intestinal absorption of glucose is already complete and glycogenolysis in the liver is blocked, that produces the hypoglycemic crisis. According to this interpretation, early hyperglycemia assumes the status of joint cause of hyperinsulinism and cannot alone induce hypoglycemia. This rationale of the pathogenesis, upheld also by HOLDSWORTHet al. 18, is corroborated by some data reported in 1967 by SAMOLS and MARKS2a, who found that the OGTT induces an abnormal rise in total plasma glucagon (pancreatic glucagon + enteroglucagon) in gastrectomy patients.
Il carico orale di glucosio ha permesso di individuare, nel gruppo dei 9 gastroresecati senza sindrome ipoglicemica, 3 casi di diabete chimico asintomatico, 2 dei quali presentavano una risposta insulinemica molto ridotta. Questo rilievo con]erma l' elevata prevalenza di diabete nei soggetti sottoposti a gastroresezione, come giä segnalato da EVENSEn 9, GXLBERT e DUNLOP 11, DÉROT e LEQUESNE 5, ROBERTSON 21, TOBE e Coll. ~~, McKIDnlE e Coll. 17 e VALLIN~9 Il diabete del gastroresecato potrebbe rientrare nelle forme da esaurimento insulare a causa dell'iperstimolazione delle isole. In questo senso depongono i rilievi istologici di TOBE e Coll. ~~, che hanno osservato nei gastroresecati da vecchia data aspetti di vacuolizzazione e degenerazione delle cellule Ô pancreatiche.
The oral glucose tolerance test identified 3 cases of symptomless chemical diabetes among our 9 gastrectomized patients without hypoglycemic syndrome; 2 of them had markedly reduced IRI response. This finding confirms the high incidence of diabetes in patients who have undergone gastric resection, as previously reported by EVENSEN9 GILBERT and DUNLOP 11, DÉROT and LEQUESNE õ, ROBERTSON 21, TOBE et al. 27, MCKIDBIE et al. 1~ and VALLIN 29 Post-gastrectomy diabetes might be a form of islet exhaustion due to chronic overstimulation. The histologic findings of TOBE et al. 2z, who observed Ô-cell vacuolation and degeneration in the pancreas of patients having undergone gastric resection long ago, would point in this direction.

La sindrome ipoglicemica tardiva del gastroresecato è secondaria ad un iperinsulinismo causato in parte dall'abnorme aumento post-prandiale della glicemia, ma soprattutto dalla massiva liberazione di entero-ormoni, specie enteroglucagone. In questi pazienti, l'iperinsulinismo può portare ad un progressivo esaurimento dell'insulinopoiesi.
The late hypoglycemic syndrome found in post-gastrectomy patients is secondary to hyperinsulinism caused partly by the abnormal postprandial rise in blood glucose and partly, indeed mainly, by the massive release of gut hormones, especially enteroglucagon. RIASSUNTO Sette pazienti gastroresecati affetti da sindrome ipoglicemica tardiva sono stati sottoposti a carico di glucosio p.o. (100 g) e i.v. (20 g). Per controllo, sono stati esaminäti anche 9 pazienti gastroresecati asintomatici e 10 soggetti normali. I1 carico orale di glucosio ha provocato, nei gastroresecati con sindrome ipoglicemica, una risposta insulinemica maggiore rispetto ai gastroresecati asintomatici e ai normali. Questo iperinsulinismo precede la fase ipoglicemica che si manifesta 3-4 h dopo il carico. Nei gastroresecati con ipoglicernia, la curva glicemica presenta una elevazione piü precoce e marcata rispetto ai gastroresecati asintomatici e rispetto ai normali ( lag storage effect). L'iperinsulinismo non semhra essere però correlabile solo con questa iperglicemia iniziale, in quanto non è evidente alcuna relazione tra livelli glicemici e insulinemici. Inoltre, l'iperglicemia da carico i.v. di glucosio, pur di entitä simile a quella da carico orale, provoca una risposta insulinemica minore, che non differisce da quella ottenuta nei gastroresecati asintomatici. Altri fattori insulino-stimolanti, quali l'enteroglucagone ed altri entero-ormoni, prodotti dall'intestino in misura abnorme, sono, insieme all'iperglicemia, la piü probabile causa della sindrome ipoglicemica tardiva del gastroresecato. RESUME Sept patients, après gastrectomie, souffrant d'un syndrome hypoglycémique retardé ont été soumis ä une charge de glucose p.o. (100 g) et i.v. (20 g). Afin de procéder ä un contrôle, on a examiné également 9 patients, après gastrectomie, sans des symptômes ainsi que 10 sujets normaux. La charge orale de glucose a provoqué dans les patients qui avaient été soumis ä gastrectomie et manifestant un syndrome hypoglycémique, une réponse d'insuline plus importante par rapport aux patients qui avaient subi la gastrectomie sans accuser des symptômes et aux sujets normäux. Cette quantité excessive d'inuline précède la phase de hypoglycémie, la courbe glycémique présente une augmentation précoce et plus accentuée par rapport aux patients ayant subi la gastrectomie sans accuser des symptômes particuliers et par rapport aux sujets normaux (lag storage effect). La quantité excessive d'insuline ne peut cependant se rapporter ä cette hyperglycémie initiale seulement, parce aucune relation entre les niveaux de glycémie et d'insuline n'est évidente. Et encore, l'hyperglycémie du ä la charge i.v. de glucose, tout en étant de la même quantité que celle de la charge orale, détermine une réponse en insuline inférieure qui ne diffère pas de celle obtenue dans les sujets ayant suhi la gastrectomie sans accuser des symptômes. D'autres facteurs stimulants d'insuline tels que entéroglucagone, et d'autres entéro-hormones produits par l'intestin dans une mesure anormale, sont, avec l'hyperglycémie, la cause la plus probable du syndrome hypoglycémique retardée du patient ayant été soumis ä gastrectomie. RESUMEN Siete pacientes con gastrorresecci6n afectos de sfndrome hipoglicémico tardlo, han sido sometidos a carga de glucosa p.o. (100 gr.) e i.v. (20 gr.). Como control se han examinado también nueve pacientes con gastrorresección asintomäticos y diez individuos normales. La carga oral de glucosa, en los pacientes con gastrorresección y slndrome hipoglicémico, ha provocado una reacción insulinémica mayor que en los pacientes con gastrorresección asintomaticos y en los normales. Ese hiperinsulinismo precede la fase hipoglicémica que se manifiesta a las 3, o 4 horas de la carga. En los pacientes con gastrorresección e hipoglicemia, la curva glicémi'ca presenta una elevación mäs precoz y acentuada que en los pacientes con gastrorresección asintomäticos y en los normales (lag storage effect). E1 hiperinsulinismo no parece que se pueda correlacionar sólo con esa hiperglicemia inicial, puesto que no se evidencia reladón alguna entre los niveles glucémicos e insulinémicos. Ademäs, la hiperglicemia por carga de glucosa i.v., aunque de entidad semejante a la de carga oral, provoca una reacción insulinémica menor, que no difiere de la que se obtiene tratando pacientes con gastrorresección asintomaticos. cagon y otros entero-hormones, producidos por el intestino en medida anormal, junto con la hiperglicemia eonstituyen la causa mäs probable de sfndrome hipoglicémico tardfo en los pacientes con gastrorresección.
ZUSAMMENFASSUNG Sieben gastrektomierte Patienten mit spätem Hypoglykämiesyndrom wurden einer Glukosebelastungsprobe unterzogen (100 g p.o.) und (20 g i.v.). Als Kontrollen dienten 9 asymptomatische gastrektomierte Patienten und 10 gesunde Normalpersonen. Bei den gastrektomierten Patienten mit Hypoglykämiesyndrom rief die orale Glukosebelastung einen stärkeren Anstieg des Insulinspiegels hervor als bei den asymptomatischen Patienten und bei den Normalpersonen. Dieser Insuliniiberschuss geht der hypoglykämischen Phase voraus, welche 3-4 h nach der Belastung auftritt. Bei hypoglykämischen Patienten zeigt die Blutzuckerkurve einen früheren und höheren Anstieg im Vergleich zu den symptomffeien Patienten und den Normalpersonen (lag storage effect). Der Hyperinsulinismus scheint jedoch nicht allein dieser anfänglichen Hyperglykämie zuschreibbar zu sein nachdem zwischen Blutzucker- und Insulinspiegel keinerlei Beziehung ersichtlich ist. Ausserdem bewirkt die Hyperglykämie bei i.v. Glukosebelastung, obwohl sie von ähnlicher Grössenordnung ist wie die infolge oraler Belastung beobachtete, eine geringere Insulinreaktion,welche sich von der bei asymptomatischen Gastrektomierten nicht unterscheidet. Ausser der Hyperglykämie sind andere Insulin-stimulierendeFaktoren, wie z.B. Enteroglukagon und andere Enterohormone welche vom Darm in abnormen Quantitäten sezerniert werden, die wahrscheinlichste Ursache des späten Hypoglykämiesyndroms bei gastrektomierten Patienten. SUMMARY Seven post-gastrectomy patients suffering from the late hypoglycemic syndrome were suhjected to oral (100 g) and i.v. glucose (20 g) tolerance tests. Nine symptom-free post-gastrectomy patients and 10 normal subjects were tested as controls. The OGTT induced a more marked plasma IRI response in the post-gastrectomy patients with hypoglyeemia than in both those without and in normals. This hyperinsulinism preceded the hypoglycemic phase, which became manifest 3-4 hrs after loading. The blood glucose curve presented an earlier and more marked rise in the post-gastrectomy patients with the hypoglycemic syndrome compared to patients without this syndrome and to normals (lag storage effect). The hyperinsulinism does not, however, seem to correlate with the initial hyperglycemia only since there was no relation whatsoever between the blood glucose and IRI levels. Further, the hyperglycemia induced by i.v. glucose, though similar in extent to that induced by the oral test, induced a smaller IRI response, not different from that obtained in the symptomless postgastrectomy patients. Other insulin-stimulantfactors, such as enteroglucagon and other gut hormones, produced by the intestine in abnormal quantity, together with hyperglycemia, are the most probable cause of the late hypoglycemic syndrome of post-gastrectomy patients.
