Opioid dependence: case report A 29-year-old man developed opioid dependence while receiving pentazocine [dose not stated]. The man had sickle cell anaemia, and received a pentazocine injection at age 16 years for a painful vaso-occlusive crisis. He experienced a high in addition to pain relief. The frequency of his crises worsened and he self-medicated with pentazocine. Gradually, he administered daily injections of pentazocine even when pain crises had resolved. He developed tolerance, craving and withdrawals unrelated to pain symptoms. The man underwent outpatient detoxification. This was unsuccessful; however, he remained abstinent from injections while receiving dextropropoxyphene. When he experienced a sickle cell crisis, he would relapse to pentazocine use. Inpatient management was advised; however, he did not comply [outcome not stated]. Author comment: "To demonstrate pentazocine dependence in a patient with sickle cell anaemia."
