CYTOGENETIC ASPECTS OF CHRONIC MYELOID LEUKEMIA
Nel 1960 NOWELL e HUNGERFORD159 dimostrarono per primi, nella leucemia mieloide cronica (LMC), la presenza di una peculiare anomalia cromosomica (a cancer cell marker) caratterizzata dalle ridotte dimensioni di uno dei piccoli cromosomi acrocentrici appartenenti al gruppo G (fig. 1). Tale riduzione, che interessava le braccia lunghe di uno dei piccoli cromosomi, veniva pressochg contemporaneamente con/ermata da BAIKIE e Coll. 8 ed in seguito documentata da numerosi altri AA. 14, 26, 78, 93, 130, 207. L' alterazione cromosomica venne denominata cromosoma di Filadelfia in omaggio alla sede ove venne e/fettuata la scoperta e, pill sinteticamente, Ph i, secondo la dizione abbreviata della nomenclatura concordata a Denver (1960).

Per lungo tempo questo cromosoma stato considerato come un cromosoma del paio 21 del gruppo G poich3, secondo COURT BROWNe TOUGH52: <>.Deponeva inoltre a/ayore dell'associazione al paio 2I il /atto che il Ph l possedesse satelliti ben visibili quali sono quelli del paio 21 rispetto a quello 22. Gli studi autoradiogra/ici non avevano a questo riguardo portato elementi sostanziali ai fini differenziativi per l'incostanza, tranne in rare eccezioni 17s, delle modalita replicative a livello di questo gruppo di cromosomi 92, 190, 195 Oggg dopo gli studi in fluorescenza e l'applicazione delle tecniche di banding appare con]ermata l"appartenenza del cromosoma Ph I al paio cromosomico pih Iungo del gruppo G.
HUNGERBORD159 in 1960 were the first to show, in chronic myeloid leukemia (CML), the presence of a peculiar chromosomal anomaly ('a cancer cell marker') characterized by the reduced dimensions of one of the small acrocentric chromosomes belonging to G group (fig. 1 ). This reduction which concerned the long arms of one of the small chromosomes, was confirmed almost at the same time by BAIKIE et al. 8 and later on documented by many other authors 14, 26, 78, 93, 130,20v The chromosomal alteration was cailed Philadelphia chromosome as a compliment to the locality where the discovery was made and, more synthetically Ph I, according to the abridged nomenclature agreed at Denver (1960). This chromosome was for a long time considered as a chromosome of pair 2 1 of G group seeing that, according to COURT BROWN and TOUGHS2: 'in good preparations of cells containing phIchro mosome two of the three normal acrocentrics are appreciably smaller than the remaining one'. Furthermore, in favour of association to pair 21 was the fact that Ph I had quite visible satellites, as are those of pair 21 in respect to 22. Autoradiographic studies in this respect have led no substantial elements for differentiating purposes owing to the changeableness, save for rare exceptions 1us, of the replication modalities at level of this group of chromosomes 92, 190, 195. Actually, after the studies in fluorescence and the application of banding techniques, the belonging of Ph ~ chromosome to the longest chromosomal pair of G group seems to be confirmed.

Dopo le prime sporadiche segnalazioni, attualmente l'esame di Paste casistiche cliniche 74. 205, 216, 219 ha consentito una migliore precisazione delle caratteristiche del cromosoma Ph I.
After the first sporadic information, at present the examination of a large number of clinical cases 74, 205,216,219 has made it possible to obtain a better specification of the characteristics of Ph ~ chromosome. a) It would appear to be present in medullary cells, in peripheral blood cells

b) Cellule eritroidi e megacariociti sarebbero anch'essi portatori dell'anomalia cromosomica. In questo senso deporrebbero le ricerche di TRUJILLO e OHNo 21o con la documentazione spettrofotometrica di residui di emoglobina tra i cromosomi di metafasi ben distese contenenti il Ph l. Altre documentazioni indirette riguardano il rapporto numerico tra le cellule attivamente dividentesi osservate in preparazioni citologiche e metafasi contenenti il cromosoma Ph I ~ol, 14o, oppure la documentazione di granuli di ferro dispersi tra i cromosomi, evidenziati con la tecnica citochimica di Perls 5~ o ancora, infine, la dimostrazione autoradiogra]ica della incorporazione di SgFe in meta/asi contenenti il Ph 1 177. L'evidenza dell'anomalia nelle cellule megacariocitarie sarebbe anch'essa indiretta e documentata dalla ]requente reperibilitd del Ph l in duplicato in meta]asi tetraploidi o in numero superiore in meta/asi poliploidi, sia a livello midollare, sia a livello di versamenti sierosi associati alla LMC ~5, 28 (figg. 2, 3, 4, 5 e 6).
b) Also erythroid cells and megakaryocytes would appear to be carriers of the chromosomal anomaly. This seems to be the result of the researches made by T~IJJILLO and OHNO 210 with a spectrophotometric documentation of hemoglobin residues among the chromosomes of well spread metaphases which include Phk Other indirect documentations concern a numerical ratio between actively dividing cells, which were observed in cytologic preparations and metaphases containing Ph I chromosome 101, 140, or else documentation of iron granules scattered amongst chromosomes, evidenced by means of the cytochemical technique of Perls so, or also finally an autoradiographic demonstration of the incorporation of SgFe into metaphases containing Ph Im. The evidence of an anomaly in megakaryocytic cells would also seem to be indirect and documented by a frequent tracing of Ph ~in duplicate in tetraploid metaphases or in higher number in polyploid metaphases, both at a medullary level and in the serous effusions associated with CML is, 28(figs 2, 3, 4, 5 and 6).

Anche nei fibroblasti midollari ottenuti per coltura di /rustoli midollari di pazienti affetti da LMC il Ph I 8 assente. Secondo MANIATIS e Coll. 15ocib escluderebbe la presenza dell'anomalia nelle cellule mesenchimali midollari indifferenziate e la eonfinerebbe ad elementi ematopoietieamente pii~ differenziati quali l'emocitoblasto mieloide e le cellule da esso derivate. d) L'anomalia cromosomica sarebbe presente nel midollo ed anche nel sangue periferico prima di ogni evidenziazione clinica. Tall dati si riferiscono ai concetti oggi ben noti di reduplicazione

These reports show that there is only a myelopoietic participation to the morbid proliferation , s2, 69. ph i is also missing in fibroblasts obtained through the culture of bone marrow particles of patients suffering from CML. According to MAmATIS et al. ~sothis would exclude the presence of the anomaly in non-differentiated medullary mesenchymal cells and would limit it to more hematopoietically differentiated elements such as myeloid hemocytoblast and derived cells. d) Chromosomal anomaly would seem to be present in the marrow and also in peripheral blood before any clinical evidence. These data refer to concepts of cellular reduplication today well 241

Il Ph I ~ stato ritrovato a questo riguardo in numerosi casi di diagnosi incerta, in cui successivamente si ebbe lo sviluppo di una conclamata situazione leucemica, come in uno dei primissimi casi da noi segnalati in letteratura 26.
known ('doubling time' according to GALTON85) and clinical picture of the disease. It is well known that the peripheral hematologic picture may have already been anomalous for some years (about five in the case of CML) before the leukemic form is clinically ascertainable on the ground of the most prominent symptoms (leukocytosis, splenomegaly, manifest discomfort, anemia). Ph I was found, in this connection, in many cases of uncertain diagnosis, where subsequently a definite leukemic situation developed, as in one of the very first cases pointed out by us in literature 26.
e) L'anomalia cromosomica pub essere sempre osservata anche nel midollo osseo di pazienti in fase di remissione. dimostrato che in fase di remissione della LMC, parallelamente al miglioramento del quadro morfologico, si viene osservando la scomparsa del Ph r nel sangue periferico, soprattutto in relazione al diminuire degli elementi immaturi o dividentisi ss. Nel midoUo osseo le indagini pih accurate tendono invece a confermare la presenza dell'anomalia eromosomica (Ph I) anche nelle fasi di remissione clinica: la scomparsa del Ph I dovrebbe essere imputata pih frequentemente alla cattiva qualit~ delle preparazioni e all'esiguo numero di metafasi ottenibili in queste condizioni 2~9.Tuttavia quadri midollari con cellule sicuramente prive di Ph I sono stati dimostrati in non poche forme di LMC come, ad esempio, in alcuni casi osservati all' esordio clinico della malattia, in cui le cellule contenenti il Ph ~ erano molto scarse in rapporto alla massa degli elementi mieloidi proliferanti. ~ stato suggerito che in questi casi potessero coesistere due cloni a livello midollare, l' uno con cariotipo normale e l' altro p h i + 33, z~s
e) Chromosomal anomaly may be evidenced also in the bone marrow of patients in remission stage. It is demonstrated that in the stage of CML remission, in parallel to an improvement of the ,norphologic picture, the disappearance of Ph I in peripheral blood is becoming evident particularly in connection with a diminution of immature or dividing elements 3s. As regards bone marrow the most accurate researches tend to confirm the presence of the chromosomal anomaly (Ph ~) also in clinical remission stages: the disappearance of Ph ~ should be more frequently attributed to the bad quality of preparations and to the very reduced number of metaphases obtainable under such conditions 219 However medullary pictures with cells surely without Ph ~ have been demonstrated in some forms of CML like, for instance, in some cases observed at the clinical onset of the disease where cells containing Ph ~were very few in comparison to the mass of proliferative myeloid elements. It has been suggested that in these cases two clones might coexist at medullary level, one with normal karyotype and the other p h i + 33, 215

trovata in gemelli monoovulari di pazienti a//etti da LMC 17, 6i, 9,, 96, i16,m, 2~9,220 0 in /igli nati da genitori con LMC 9. Questi ultimi AA., studiando aggregazioni /amiliari di LMC, trovarono il Ph I helle celluIe midollari di un paziente con LMC la cui madre era morta per la stessa malattia. Tuttavia non poterono identi/icare cellule Phz + nelle cotture di fibroblasti sottocutanei, convaIidando indirettamente per questa via la natura acquisita dell'aberrazione cromosomica. Analogamente il Pff non era presente neUe colture di sangue peri/erico e di fibroblasti di una ragazza con leucemia acuta la cui madre era deceduta per LMC. Esiste tuttavia una segnafazione 2o6, peraltro isolata, in cui il ritrovamento del Ph z in gemelli monoovulari ed in un loro discendente con LMC suggerirebbe come, in alcuni casi, la [orma leucemica possa essere geneticamente determinata.
in monoovular twins of patients suffering from CML 17,61,94,96, n6, 137,219,22o or in children of parents suffering from CML 9. These last mentioned authors, studying family aggregations of CML, found Ph ~ in the bone marrow cells of a patient suffering from CML, whose mother had died of the same disease. However Phi+ cells could not be identified in cultures of subcutaneous fibroblasts, thus indirectly corroborating by this way the acquired nature of chromosomal aberration. Similarly Ph I was absent in cultures of peripheral blood and fibroblasts of a girl suffering from acute leukemia, whose mother had died of CML.

Tra le varie ipotesi prospettate (tab. 1) per chiarire la mor/ogenesi del Ph I, quella che ammette la delezione di una porzione di materiale cromatidico di un autosoma del gruppo G ~ la pi~ semplice e verosimile. RUBKIN e Coll. 1s6 hanno anche calcolato, sulla base di studi spettro/otometricL la perdita di materiale interessata in questa delezione, aggirantesi in media intorno al 40 % ma variabile, evidentemente, da caso a caso.
Among the various hypotheses which have been put forward (tab. 1) so as to clarify the morphogenesis of Ph I, the simplest and most likely one seems to be that which admits the deletion of a portion of chromatidic material of an autosome belonging to G group. RUDKIN et al. '~ have also calculated, on the basis of spectrophotometric studies, the loss of material connected with this deletion, which is about 40 %, but obviously changing from case to case. However the deletion site is still a matter for discussion: whilst in fact the loss of chromatidic material in the telomerle region would lead to the formation of an unstable centric fragment (owing to possible further rearrangements effected by the free chromatidic portions in way of the deletion), a deletion of an intercalary type should keep the stability of the telomeric portion with reproduction of a deficient but stable chromosome 1~4.In any case this last mentioned type of deletion (intercalary)

In ogni caso,

quest'ultimo tipo di delezione (intercalare) dovrebbe verificarsi per e//etto di una duplice delezione in un breve braccio cromatidico quale quello di un cromosoma G e quindi dovrebbe essere un evento assai in/requente in natura 11o Queste difficolt~ interpretative rimangono anche per ammettere che il segmento intercalare deleto vada a traslocarsi su di un altro cromosoma. H s u ,0 ammetterebbe come possibile invece il fatto chela delezione possa ef[ettivamente intervenire qualora il cromosoma G dell'uomo risultasse fornito, come il cromosoma I dell'hamster cinese, di un telomero interstiziale, in modo tale che una rottura unica in questa porzione, preservando una minima porzione telomerica utile, manterrebbe la stabilit?z della regione terminale del cromatide. Recentemente b stata proposta anche la possibilit~ che il Ph I emerga da un processo di inversione pericentrica, essendo stato osservato un Ph I di tipo metacentrico di grandezza eomparabile a quella degli altri cromosomi del gruppo G 29, 191 Secondo alcuni AA. la traslocazione del ]rammento deleto su di un altro cromosoma appare come un'ipotesi non realistica, per il [atto che finora non ~ stata sicuramente osservata. Tuttavia va considerato il ~atto che la traslocazione di porzioni minimali di materiale cromatidico risulta di di[ficile dimostrazione, soprattutto quando vengano interessati cromosomi di grande taglia m. Una traslocazione coinvolgente cromosomi di piccole dimensioni appare invece meglio evidenziabile, come appunto b il caso segnalato da BOTTURA e COOTINHO29 in cui il Ph I risultava come e//etto di una delezione con traslocazione di materiale cromatidico su di un altro cromosoma G: t ( G / G , P f f ) . Infine, secondo un'ultima interpretazione patogenetica, il Ph I potrebbe rappresentare un' unit?t completamente nuova nell'ambito del cariotipo della LAfC, rimpiazzante uno dei piccoli acrocentrici del gruppo G. Se il Pff rappresenta la perdita di un certo numero di geni, ~ improbabile rut-

should take place as a result of a double deletion in a short chromatidic arm such as that of a G chromosome and should therefore be a very infrequent event in nature 110. These difficulties of explanation subsist also for admitting that the deleted intercalary segment moves on another chromosome. H s v 110would instead admit as possible the fact that deletion could really take place if G chromosome of man should be found to possess, as the I chromosome of Chinese hamster, an interstitial telomere so that a single breakage in this section, preserving a minimum useful telomeric portion, would keep the stability of the chromatidic terminal region. Recently, the possibility has also been put forward that Ph I may emerge from a process of pericentric inversion, as a Ph i of metacentric type has been observed which is comparable in size to the other chromosomes of G group 29, 191. According to some authors, the translocation of the deleted fragment on another chromosome seems to be a non realistic hypothesis, seeing that up to now it has never been surely observed. However, the fact has to be considered that a translocation of minimal portions of chromatidic material is difficult to demonstrate, above all when large size chromosomes are involved lsl. On the other hand, a translocation involving small size chromosomes seems easier to be evidenced, as in fact it is the case mentioned by BOTTURA and COUTINHO 29, where Ph I appeared as the consequence of a deletion with translocation of chromatidic material on another G chromosome: t(G/G,phI). Finally, according to a last pathogenetic interpretation, Ph I might represent a completely new unit within the range of CML's karyotype replacing one of the small acrocentrics of G group. If Pht represents the loss of a certain number of genes, it is unlikely however

Up to now the best characterized group of patients with CML Ph ~--- is that of infantile forms, which are however clinically very different from the adult forms (on account of lymphadenopathy, rather frequent infections, added hemorrhagic phenomena). In adults clinical differentiation between patients with and without Ph I is less easy. CARBON~, et al. 36, comparing 13 Ph ~--- patients with 47 p h i + patients found that the only elements differentiating the two populations under examination were represented by a mean value of platelets and leukocytes, lower in Ph I--- individuals. However, on the ground of the available data, we think that it is possible to work out a further cytogenetic classification of CML forms (tab. 2).

b) Forms without splenomegaly: these are still scarcely documented pictures, where it is found difficult to list common basic characteristics. As a matter of fact, the absence or poor consistency of the spleen may be attributed to a particular evolutive model of the morbid form; in some cases they may be initial manifestations of CML where the spleen has not yet reached a considerable size; in other cases we may have instead manifestations which were present for quite a long time and in which a cytostatic treatment, by interrupting a transient proliferative equilibrium appearing till then, would hasten clinical course and prevent a more tardive rise of splenomegaly. The first communications of cytogenetic studies during erythremia related in fact to erythremic circumstances in patients suffering from CML. More recently these manifestations have made it possible to confirm the presence of chromosomal anomaly (Ph I) also in precursors of the red and megakaryocytary series.

SPEEI) e LAWLER 1~ attribuirono alla presenza di un si//atto cariotipo la possibilit~t di un decorso clinico estremamente protratto in questi pazienti. Already in 1963 TOUCH et al. ~ supposed that patients with XO,PhIq - cells might belong to a well defined subgroup of CML. SPEED and LAWLER196ascribed the possibility of an extremely extended clinical course as regards these patients to the presence of a karyotype of such a nature. Finally DE GROUCHY et al. s6 emphasized the fact that patients having this karyotype had no children. It was possible to infer from the whole of these data the existence of a CML variant characterized by an extended course and by infertility of males 169 As the absence of the Y chromosome seems to be confined solely at bone marrow level, it is very difficult to give an explanation on this ground as to the poor fertility of such patients, although phenomena of non-disjunction in the mitosis of these patients or a frequent loss of Y chromo-

On the other hand, the same material gathered up to now might prove insufficient for drawing general conclusions and the absence of children in these patients could be merely accidental ,93 As far as the second characteristic, that is an extended course of the disease and the comparatively high benignity of the same in these patients, it has been supposed that the clone of XO,phI+ cells may derive from clone XY,PhX+ and it may have taken a certain proliferative advantage in comparison with its original strain. The extended course of the disease in these cases would thus be interpreted not as due to the activity of clone XO,Ph~+, but to a competitive repression as regards proliferation of clone XY,Ph x+ .
3) Forme con Ph I associato ad altre anomalie cromosomiche In questi casi il cromosoma Ph I pub essere associato sin dall'inizio della LMC con altre aberrazioni cromosomiche, oppure pitt/requentemente pub essere associato, nel decorso evolutivo della/orma leucemica, ad altre anomalie cromosomiche. Di questo secondo argomento verrgl ri/erito al riguardo dei cariotipi della LMC nella ]ase di subacutizzazione, mentre qui verranno prese in considerazione le /orme presentanti aberrazioni cromosomiche in aggiunta al Ph 1 sin dall'inizio del decorso clinico. I cromosomi abnormi presenti in questi pazienti possono intervenire in metafasi aneuploidi e poliploidi ~"57, 81, o3, lo9, uT, 1~. Nel paziente descritto da GOH ~3 vi erano due linee cellulari entrambe contenenti il Ph I e a 47 cromosomi, l'una caratterizzata daUa presenza di un cromosoma soprannumerario nel gruppo C e l'altra nel gruppo D. In questo paziente non vi era alcuna evidenza di acutizzazione della/orma a distanza di 1 I mesi dallo studio citogenetico. II caso ri/erito da HousToN e CollJ ~ possedeva invece una linea cellulare pseudodiploide contenente un marker metacentrico. Anche questo paziente non era an-

We shall speak on this subject in connection with karyotypes of CML in the acute phase, whilst here will be taken into consideration only forms showing chromosomal aberrations in addition to Pht even from the beginning of the clinical course. The abnormal chromosomes present in these patients may intervene in aneuploid or polyploid metaphases 1, sT, sl, 93, 109,147,18s. In the patient described by Gort 93 there were two cellular lines both containing Ph I and with 47 chromosomes, one being characterized by the presence of a supernumerary chromosome in C group and the other in D group. Eleven months after the cytogenetic study in this patient there was no evidence of an exacerbation of the form. Instead the case reported by HOUSTON et al. 109 had a pseudodiploid cellular line containing a metaeentric marker. This patient also had not yet

4) Forme mieloblastiche acute ab initio con Ph I Esiste ormai in letteratura una casistica ben documentata comprovante la presenza del cromosoma Ph I in situazioni leucemiche acute di tipo mieloblastico ab initio. Queste ]orme documenterebbero la possibilitA che il Ph ~ in condizioni particolari, soprattutto se associate ad altre modificazioni dell'assetto cromosomico, possa talora presentarsi in situazioni non strettamente connesse con la LMC. ii possibile che questi casi rappresentino forme di LMC precedentemente misconosciute o con decorso atipico. HIRSCHHORNlo6 ammetterebbe, sulla base di proprie osservazioni, che le cellule P h i + possano rappresentare elementi dotati di vantaggio selettivo nell' ambiente in cui crescono, conducenti ad una situazione quale quella rappresentata dalla LMC, ma non ad una ~ vera ~ leucemia (true leukemia). Il disordine mieloproli]erativo sarebbe rappresentato dalla condizione clinica esprimentesi come LMC che condurrebbe, a seconda della sensibilitA individuale, allo sviluppo della ]orma acuta, cio~ della ~ vera ~ leucemia. In questo senso depongono anche i risultati di HUANGe Coll. 111,secondo cui la capacitA di clonizzare sarebbe particolarmente elevata per le linee cellulari aneuploidi e pi?~ bassa per le linee euploidi e pseudodiploidi. Tale capacit~ sarebbe inoltre del tutto indipendente dal
4) Acute myeloblastic /orms ab initio with Ph 1 There is by now in literature a well documented number of cases evidencing the presence of the Ph I chromosome in acute leukemic situations of a myeloblastic type which are ab initio. These forms would support the possibility that Ph I under particular conditions, above all if associated with other modifications of the chromosomal order, may sometimes appear in situations which are not strictly connected with CML. It is possible that these cases represent CML forms which were previously unidentified or had an atypical course. HIRSCHHORN 106 admits, on the ground of his own observation, that p h i + cells may represent elements possessing a selective advantage in the environment where they grow, and leading to situations such as that represented by CML, but not a true leukemia. The myeloproliferative disorder would be represented by a clinical condition expressing itself as CML which would lead, in accordance with individual sensitivity, to a development of the acute form, i.e. 'true' leukemia. Same conclusions are reached by the findings of HUANC et al. m according to whom the capacity of cloning would be particularly high as regards aneuploid cellular lines and lower as regards euploid and pseudodiploid lines. Furthermore, such capacity would be entirely

Recentemente, WHANG-PENG e Coll. 21ehanno condotto uno studio di revisione su 103 pazienti non selezionati af~etti da leucemia mieloblastica acuta. Circa il 70 % dei pazienti possedeva un cariotipo normale, mentre il 30 % risultava possedere linee aneuploidi. Sebbene nei pazienti con cariotipi aneuploidi non siano state ritrovate anomalie cromosomiche univoche, tuttavia il gruppo G appariva coinvolto pik ~requentemente degli altri gruppi. Quattro casi avevano il dato anamnestico della pregressa esposizione a radiazioni: di questi, 3 possedevano un cariotipo normale ed 1 possedeva un cromosoma Ph I nelle preparazioni da midollo osseo. Un altro pazienre, peraltro senza passato positivo per esposizione a radiazioni, aveva anch'esso un cromosoma Pht nelle preparazioni midoUari. WHANG-t)ENGe Coll. 21esuggeriscono di considerare queste forme come rare crisi blastiche di LMC piuttosto che ]orme di leucemia mieloblastica acuta con Ph l, in accordo con l' osservazione che esse rappresentano condizioni di LMC in cui la linea cellulare mieloblastica appare prima che si siano mani[estate le caratteristiche cliniche ed ematologiche della ]orma leucemica cronica conclamata. E) FORMEPh ~--1) Forme infantili Si tratta di mani/estazioni assai simili, dal lato morfologico, a quelle classiche della LMC e presentanti, dal lato clinico, alcune peculiaritft, come ad esempio la coesistenza di adenopatie, la presenza di eruzioni cutanee, una spiccata trombocitopenia associata talora a monocitosi e lin/ocitosi, e mani/estanti in/ine una spiccata resistenza alla terapia con busul~ano o al trattamento radioterapico. Dati pift recenti dimostrerebbero altres} caratteristiche laboratoristiche particolari, come una elevazione del livello di

WHANG-PENG et al. ~16 have recently carried out a reviewing study on 103 non-selected patients suffering from acute myeloblastic leukemia. Approximately 70 % of the patients had a normal karyotype, whilst 30 % were found to possess aneuploid lines. Although univocal chromosomal anomalies have not been found in patients having aneuploid karyotypes, nevertheless G group appeared to be more frequently involved than the other groups. Four cases had the anamnestic datum of previous exposure to radiations; out of these, three had a normal karyotype and one had a Ph I chromosome in the bone marrow preparations. Another patient, who however had no positive history of exposure to radiations, also had a Ph I chromosome in bone marrow preparations. WHANG-PENG et al. 2t6 suggest to consider these forms as rarely occurring blastic crises of CML rather than forms of acute myeloblastic leukemia with Ph I, in agreement with the remark that they represent CML conditions where the myeloblastic cellular line is manifest before the appearance of the clinical and hematologic characteristics of the overt chronic leukemic form. v.) Ph I -
FORMS
1) Infantile forms These are manifestations quite similar, from a morphological point of view, to the typical ones of CML and showing, from a clinical point of view, some peculiarities such as, for instance, the coexistence of adenopathies, the presence of rash, a marked thrombocytopenia connected sometimes with monocytosis and lymphocytosis, and finally showing a marked resistance to a busulphan therapy or to radiotherapic treatment. In addition, more recent data would show special laboratory characteristics such as

2) Forme a polinucleati maturi (tipo Emil-Weil e S~) Si tratta di casi relativamente rari caratterizzati dalla presenza nel sangue periferico di forme quasi esclusivamente mature della serie granulocitaria. II livello di ]osfatasi alcalina leucocitaria risultato /requentemente elevato in queste condizioni 185. I reperti citogenetici in queste forme morbose non sono univoci nella letteratura: put prevalendo i casi Ph ~ - , esistono segnalazioni confermanti la presenza di un cromosoma ph 1 8~.
2) Forms with mature polynucleated (Emil-Weil and S~e type) These are relatively rare cases, characterized by the presence in peripheral blood of almost exclusively mature forms of granulocytic series. Under these conditions the rate of leucocyte alkaline phosphatase has been foundj to be frequently high 18s. Cytogenetic reports in these morbid forms are not univocal in literature; although Ph I - are prevalent, there are communications which confirm the presence of a Ph ~ chromosome ~

I sostenitori di quest' ultima interpretazione banno soprattutto insistito negli ultimi tempi su di un inserimento delle due mani/estazioni (leucemia ad eosinofili e collagenopatia) in un unico quadro morboso a genesi autoimmune, estrinsecantesi, a seconda deUe caratteristiche, ora con una mani/estazione prevalente, ora con l'altra 19, m. The problem of Ph i - patients with CML often makes a clinically difficult diagnosis. From the cytogenetic point of view such problem evidences situations which are still to be better outlined in connection with the possibility that phL-- patients actually represent a special CML subgroup. Apart from some cases in which the haematologic picture probably concealed a myelofibrotic situation m or where other myeloproliferative situations came into being 3~. ~s9, there are well documented observations of CML of adults which are characterized by the absence of Ph ~ chromosome 74, ~39,20s,215 Case reports differ in this respect as regards some data reflecting in part classification methods of patients and are in part a reflect of the non-homogeneity of observations often based on hospitals in large towns where as a rule complex or atypical cases are studied. Many discussions are made as to whether the male sex is prevailingly affected and as to the age in Ph I - forms, but in many case reports also infantile leukemic pictures are included (under 7 years) so that in some of them 2~ distribution according to the age is practically the same between p h i + and Ph x - groups. The CML denomination is found in this sense to be appropriate only to Ph I + forms, whilst Ph ~ - forms having a more rapid development (survival from one to two years) should rather be named 'subacute myeloid leukemia'. However EZDINLI et al. 74 have been able to note that if a valuation of survival time in Ph I - forms is made, together with a larger fraction of subjects who die during the first year of the clinical manifestation there is a second smaller fraction whose clinical course can be superimposed to that of P h i + patients. Such a twofold distribution suggests the inclusion in this subgroup of at least two more distinct populations, one still having the characteristics of typical CML and the other having a definitely rapid course and characteristics in agreement with the above suggested definition of 'subacute myeloid leukemia' and, considering the prevalent age in which it appears 'acute leukemia o~ the elderly' 74.

A scopo illustrativo verranno qui prese in considerazione aherazioni di tipo qualitativo e alterazioni di tipo strutturale. In alcuni pazienti (circa 1/3) la tras[ormazione acuta non comporta alcuna alterazione qualitativa del cariotipo, persistendo il Pff come l'unica alterazione dimostrabile 142,143, 219. Anomalies of structural kind do not consist simply in loss or addition of apparently normal chromosomes (C trisomy) 123 but are often represented by markers such as large acrocentrics or submetacentrics, as is often the case in other types of turnouts 16. In these cases all chromosomal groups are involved, but as it may be expected, those which are most frequently affected are C and G groups 70, 165-172,204, 215. Among structural anomalies one should also include the special phenomenon of the finding of one or more Ph I chromosomes in blastic cells of CML in subacute phase. Generally speaking, these chromosomes are present in cellular lines containing 47 or 48 chromosomes, but many other karyotypes have been reported. Ph I may be present in triplicate 174.192 or reduplicated (diplo-Ph x according to HAMPEL 102)or, more frequently, duplicated in relapsed forms and in terminal blastic phases 55, 64, 7o, 72. 101,120,124, 125,1~, 127,128,132,135,170,183,199.2oo.This phenomenon might be connected with the general law according to which the induction of a chromosomal anomaly favours, owing to the lesser stability of

La duplicazione peraltro b stata osservata anche al di /uori di crisi blastiche vere e proprie, come nel corso di un caso di leucemia mieloblastica acuta ab initio e nel corso di [orme di LMC in/ase di remissione 62, 194 Sulla base dei risultati acquisiti, PEDERSEN166,167 ritiene di poter a//ermare che, nel corso dello sviluppo della LMC, le cellule iperdiploidi Phi+ sono seIezionate a spese di cellule di altro tipo e che il trattamento citostatico, in particolare, favorisce questa selezione, probabilmente per il fatto che le cellule iperdiploidi sono relativamente resistenti all'azione dei /armaci somministrati. La selezione delle cellule iperdiploidi potrebbe essere interpretata a questo riguardo come espressione deIl'avvantaggiamento di cellule pi~ immature della serie mieloide, durante la progressione della LMC. DE GROUCHYe Coll. ~ hanno dal canto loro elencato tre possibili modelli di evoluzione clonale della [orma mieloide cronica: un primo modello definito dall'acquisizione successiva di cromosomi soprannumerari, un secondo modello caratterizzato dalla perdita di cromosomi in associazione o no all'acquisizione di altri cromosomi, infine un terzo modello definito dalla presenza di riarrangiamenti strutturali. In un'analisi pi~ recente, su 24 casi di LMC evoluti clonalmente 11 obbedivano allo schema di un modello preciso caratterizzato dalla presenza di un cromosoma Pff , dalla perdita di un crornosoma del gruppo 17-I8 e dall'acquisizione di un cromosoma marker mediocentrico della grandezza di un cromosoma C. Nella maggior parte delle cellule esaminate vi era un'alta probabi-

These aberrations may presumably appear by chance, although in CML one of the most frequent way is represented by the non-disjunction of Ph I with subsequent duplication in daughter-cells. Whether duplication of Ph I actually represent the cause of blastic crisis or is simply an epiphenomenon, is still a controversial problem. However duplication has been observed also outside blastic crises, like in the course of a case of ab initio acute myeloblastic leukemia and in the course of CML forms in remission phase 62, 194 On the basis of the acquired results PEDERSEN 166, 167affirms that during the
development of CML the Phi+ hyperdiploid cells are selected at the expense of cells of any other type and that cytostatic treatment favours particularly this selection, probably owing to the fact that hyperdiploid cells are resistant to the action of the administered drugs. In this connection the selection of hyperdiploid cells could be considered as an expression of the advantage during the progression of CML of cells more immature of the myeloid series. DE GROUCHYet al. s7have listed three possible models of clonal evolution of chronic myeloid form: a first model identified by a successive acquisition of supernumerary chromosomes, a second model characterized by a loss of chromosomes, either or not associated to the acquisition of other chromosomes and finally a third model defined by presence of structural rearrangements. In a more recent analysis, 11 out of 24 clonally evolved CML cases followed the scheme of a clear model characterized by the presence of a Ph I chromosome, the loss of a 17-18 chromosome group and the acquisition of a middle-centric marker chromosome as large as a C chromosome. L' associazione di una LMC Phi+ con altre situazioni morbose ~ stata da tempo segnalata, seppure in un numero relativamente scarso di osservazioni. Pi~ [requentemente, questa associazione si verifica per il concomitare oil sovrapporsi di un'altra situazione emopatica, ma il processo leucemico pub accompagnarsi anche a manifestazioni morbose di altra natura, spesso congenita, come la sindrome di Down e la sindrome di Klinefelter. Soprattutto interessante ~ l'associazione della LMC con altre situazioni emopatiche per le possibili implicazioni concernenti il livello dell' alterazione cromosomica, la sede e la contemporaneit~ o meno dell'insorgenza. JOSEPH e Coll. m hanno riferito due casi di LMC complicati, dopo rispettivamente 6 e 4 anni di decorso daUa loro evidenziazione clinica, da linfoadenopatie istologicamente classificate come lin/omi maligni, e nelle quali era dimostrabile il cromosoma Ph ~. In accordo con altri casi riportati in letteratura, ma non seguiti citogeneticamente, il reperto clinico caratteristico di queste /orme era dato dalla rapidit~ dell'evoluzione dopo la comparsa dei linfomi. ~ possibile pensate che il trattamento chemioterapico precedentemente eseguito abbia predisposto il paziente leucemico ad una seconda proliferazione maligna, tra l'altro maggiormente resistente al trattamento. Un'ipotesi alternativa suggerirebbe che la stimolazione operata dagli agenti chemioterapici abbia potuto spingere le cellule staminali ~ totipotenti ~ a differenziarsi secondo una linea lin/omatosa o reticolare maligna piuttosto che verso la serie granulocitica. ~ infine possibile, in questi casi, interpretare le masse tumorali linfoadenopatiche come ~ crisi blastiche extramidollari ~, piuttosto che come veri linfomi, in quanto costituite
The association of Phi+ CML with other morbid situations has been already reported even if in a comparatively limited number of observations. This association occurs more frequently on account of the coexistence or the superimposing of another hemopathic situation, but a leukemic process may go together also with morbid manifestations of another often congenital character such as Down's syndrome and Klinefelter's syndrome. Association of CML with other hemopathic situations is interesting above all for the possible implications concerning the level of chromosomal alteration, the site and contemporary or non-contemporary inception. JOSEPH e t al. 117 have reported two cases of CML complicated respectively after 6 and 4 years from their clinical evidence by lymphoadenopathies histologically classified as malignant lymphomas and where Ph I was demonstrable. In agreement with other cases reported in literature but which were not cytogenetically followed, the characteristic clinical pattern of these forms was given by the rapidity of the evolution after appearance of lymphomas. One could think that the previously performed chemiotherapeutic treatment predisposed the leukemic patient to a second malignant proliferation more resistant to the treatment. An alternative hypothesis suggests that the stimulation brought about by chemiotherapeutic agents may induce the 'totipotent' staminal cells to differentiate according to a malignant lymphomatous or reticular line rather than towards the granulocytic series. Finally it is possible, in these cases, to interpret lymphoadenopathic tumorous masses as 'extrameduUary blastic crises' rather than true lymphomas, as these are formed of cells so highly indifferentiat-

da cellu[e cos~ altamente indifferenziate da rendere impossibile, aU'esame istologico, la loro vera origine. Esse potrebbero in questo senso, come documenterebbe il marker Ph I, precedere o vicariare crisi blastiche a livello peri/erico o nel midollo osseo. Il problema dell'associazione della LMC con il morbo di Hodgkin ~ stato a/]rontato recentemente da ~ZDINLI e Coll. 73 nell'ambito di una discussione pii~ ampia riguardante l'incidenza delle leucemie in generale nel corso della malattia hodgkiniana. Nei vari casi riconsiderati, l'occorrenza di una /orma leucemica ~ in genere posteriore aU'insorgenza della malattia di Hodgkin. In particolare ~ stato sottolineato il reperto di alterazioni cromosomiche caratteristiche di ogni tipo di leucemia (Phl, markers, aneuploidia) a livello midollare, di contro all' assenza di ogni alterazione nel midollo di soggetti affetti soltanto dalla forma hodgkiniana. Questi dati sono assunti dagli AA. come testimonianza del fatto che le due malattie in questione rappresentano entit~ totalmente distinte. Poich3 d'altra parte oltre alla malattia di Hodgkin l'unico dato in comune in questi pazienti era la pregressa irradiazione, si pub arguire come l' occorrenza della LMC possa rappresentare nient'altro che il sovrapporsi alla malattia di base di una leucosi indotta da radiazione. Il tempo medio di comparsa, nella casistica esaminata, ~ di 7 anni, il che in buon accordo con i dati di GALTON,5 sul doubling time delle forme di LMC 12" ~3. Il fatto che tali forme di associazione si siano rese pi~ evidenti negli ultimi anni, ~ in accordo, secondo l' opinione degli AA. sopra citati, con l'attuale introduzione di cicli radioterapici assai intensi e con il tempo di sopravvivenza oggi pih protratto di questi malati. L'associazione mieloma-LMC ~ gi~ stata fatta oggetto di diverse segnalazioni, ma solo recentemente indagata con criteri citogenetici. They could in this sense, as Ph I marker evidences, precede or vicariate blastic crises at peripheral level or in the bone marrow. The problem of the association of CML with Hodgkin's disease was recently examined by EZDINLI et al. 7s in the frame of a more general discussion concerning incidence of leukemia as a whole in the course of Hodgkin's disease. In the various cases which were reconsidered, a leukemic form occurs generally later than the inception of Hodgkin's disease. In particular, stress has been laid on the report of chromosomal alterations which are characteristic of all type of leukemia (Ph ~, markers, aneuploidy) at medullary level, as against the lacking of marrow alterations in patients suffering only from the HodgkinJan form. These data are taken by the authors as documenting the fact that the two examined diseases represent totally distinct entities. As the only factor in common in these patients besides Hodgkin's disease was a previous irradiation, it may be inferred from this that the presence of CML may represent nothing else but the superimposing of a leukosis induced by radiation to the basic disease. The average time of appearance is, in the range of examined cases, of 7 years, which is quite in agreement with GALTON'S 85data on the 'doubling time' of CML forms 12, is. The fact that these forms of association have become more evident in recent years is in agreement, in the opinion of the above mentioned authors, with the present introduction of very intense radiotherapic cycles and with the today more prolonged survival time for these patients. The myeloma-CML association has already been the theme of various communications, but it has only recently been investigated according to cytogenetic criteria. In a case reported by RITZMANN et al. 182 there was a coexistence of CML and monoclonal gammopathy (IgG, k type), the case being cyto279

L'esistenza di metafasi contenenti il Ph I e cromosomi grandi metacentrici in associazione dimostrerebbe d'altronde come nello stesso ceppo proli[erativo si siano innestate due anomalie appartenenti a disordini proliferativi diversi, richiamando anche per questa via l'interessamento di cellule staminali comuni, come giA segnalato in caso di associazioni multiple di disordini ematologici 42. In maniera analoga verrebbe a richiamarsi ad un coinvolgimento di cellule reticolari comuni l' associazione tra LMC e reticolosarcoma, descritta da LIBRE e Mc

genetically characterized by various stemlines, one of which showed both the Pht and a chromosomal aberration of submetacentric marker type formerly described as typical of WaldenstriSm forms (MW chromosomes). It is possible that this association is casual or that we have here a combined myeloplasmocytary proliferation, but it is also possible that a process of monoclonal plasmacellular answer to the myeloid proliferation is operating, or finally that a situation of homeostatic paresis (immunological deficit) may have favoured the appearance of the leukemic clone. The existence of metaphases containing Ph I and associated with large metacentric chromosomes would show, on the other hand, that in the same proliferative strain are inserted two anomalies belonging to different proliferative disorders, thus provoking in this way the intervention of common staminal cells, as already pointed out in case of multiple associations of hematological disorders 42. Analogously, the association between CML and reticulosarcoma described by LIBRE and Mc FARLAND 148 can be explained as an involvement of common reticular cells.

Proprio in rapporto al problema della interdipendenza tra Ph I e radiazioni va inserito il caso riferito da PROPP e LlzzI 176riguardante un paziente affetto da leucemia linfoblastica acuta in cui potd essere documentata la presenza di un cromosoma Filadelfia-simile. II paziente in questione era un radiologo che per 12 anni aveva praticato fluoroscopie e che sulla base di questa attivitA poteva aver sviluppato un quadro di leucemia contenente il Ph I. In queste condizioni il problema interpretativo si riallaccia a quello pih generale delle associazioni tra LMC e altre emopatie, in cui possono essere coinvolte stem cells diverse, e cio~ filiere proliferative diverse da quella mieloide, come sino ad ora ammesso per le forme Ph z +.
ce of CML in which many members showed, in the absence of any pathological situation, a Philadelphia-like chromosome 160,21s Also some conditions strictly connected with protracted therapeutic treatments with cytostatic drugs 168,chloramphenicol *~, lysergyde 97 or connected with exposure to X-rays 88, 90have allowed the demonstration of Philadelphialike chromosomes. Many of these data which were ascertained prior to the introduction of fluorescence techniques, should today be revaluated owing to the possibility that, in some cases, it may not be a question of Ph ~ chromosomes but rather of deleted Y or other chromosomal fragments (fig. 7). However in some cases, as for instance during radiations the appearance of Ph I chromosomes is no longer considered exceptional, as is shown by the studies of various Jat~anese authors on the appearance of leukemic forms in survivors of the Hiroshima atomic explosion or in subjects exposed to radioactivity for occupational reasons. We have to mention the case reported by PROPP and LlzzI 176concerning a patient suffering from acute lymphoblastic leukemia where the presence of a Philadelphia-like chromosome could be evidenced just for the problem of interdependence between Ph I and radiations. The patient in question was a radiologist who had been practising fluoroscopies for 12 years and, as a consequence, might have developed a Ph I leukemic form. Consequently, the explanatory problem is linked to the more general problem of associations between CML and other hemopathies where different stem cells may be involved, that is to say proliferative series different from the myeloid as has been accepted up to now for p h i + forms.

del cariotipo ~ intervenuta recentemente dopo gli studi di CASVERSSONe Coll. 3a, 39, 40 e di ZECH 224, basati sull'impiego di fluorocromi come la chinacrina e la chinacrina mostarda, in grado di legarsi in corrispondenza dell' acido desossiribonucleico ( A D N ) cromosomico secondo siti pre/erenziali. Queste osservazioni hanno consentito l'identificazione selettiva delle diverse coppie cromosomiche in base alla presenza di strie trasversali piil o meno brillanti in corrispondenza dei cromatidi caratterizzabili per la /requenza con cui esse sono riproducibili. Con l'impiego della chinacrina risulta generalmente agevole l'identificazione visuale del cromosoma As, di uno dei cromosomi D e dell'Y no. Con l'uso della chinacrina mostarda ~ inoltre possibile dimostrare una netta differenza tra le coppie G21 e G22. In condizioni normali si dimostra chela coppia 22 ~ generalmente pi~ pi~ piccola di dimensioni rispetto alla coppia 21 (secondo la classificazione concordata a Denver e ripresa a Chicago). Con lo studio in fluorescenza dopo chinacrina la coppia pi~ piccola dimostra una banda fluorescente estremamente brillante in corrispondenza della regione prossimale delle braccia lunghe, mentre la coppia pi~ grande presenta una colorazione pik paIlida. L'esame comparato in fluorescenza di cariotipi di soggetti mongoli (trisomia 21) ha portato alla dimostrazione che il cromosoma soprannumerario appartiene alia coppia pi~ piccola, presentando anch' esso una banda [ortemente fluorescente in corrispondenza della regione prossimale delle braccia lunghe. Nel caso della LMC invece il cromosoma Ph I viene ad associarsi alla coppia con fluorescenza debole e quindi alla coppia opposta a quella della sindrome di Down 163 (fig. 8). La diretta conseguenza di queste osservazioni ~ innanzitutto il [atto che i cromosomi Ph ~ e l'extracromosoma G del mongolismo, una volta attribuiti alla medesima coppia cromosomica (Gzl), appaiono oggi corrispondere a coppie diverse del gruppo G e, pik precisamente, al paio pi~ lungo (G2D e al paio pi~ corto (G22) rispettivamente. the karyotype has been recently effected after the studies by CASPERSSONet ah as, 39,4o and ZF.CH224based on the use of fluorochromes such as quinacrine and mustard quinacrine linked to the chromosomal desoxyribonucleic acid (ADN) according to preferential sites. These observations have made possible the selective identification of various chromosomal pairs according to the presence of more or less brilliant transversal bands of chromatids which are characterized by the frequency by which they are reproducible. Using quinacrine the visual identification of As chromosome, of a D chromosome and of Y is0 is generally easy. Using mustard quinacrine it is possible to demonstrate a clear difference between GEl and GEE pairs. In normal conditions it is shown that pair 22 is generally smaller than pair 21 (according to the classification agreed in Denver and confirmed in Chicago). By fluorescence study after quinacrine the smaller pair shows an extremely brilliant fluorescent strip in the proximal region of long arms, whilst the larger pair shows a less intense staining. A comparative fluorescence examination of karyotypes of mongolian subjects (2 1 trisomy) demonstrated that the supernumerary chromosome belongs to the smaller pair and that it shows a strongly fluorescent strip in the proximal region of long arms. Instead in the case of CML Ph I chromosome is associated to the weakly fluorescent pair and therefore to the pair opposed to that of Down's syndrome 163 (fig. 8).
A direct consequence of these observations is firstly that Ph I chromosomes and G extrachromosome of mongolism which were formerly ascribed to the same chromosomal pair (Gzl) now appear to correspond to different pairs of G group and, more precisely, to the longer pair (G2,) and to the shorter pair (G22) respectively. tenenza a coppie cromosomiche separate ~ stata suffragata anche dall'utilizzazione delle cosiddette tecniche di banding che consentono, mediante procedimenti vari di denaturazione dell'ADN 4, 4s, 63, 84, 223, di ottenere la dimostrazione di bande eterocromatiche ipercolorantesi con il Giemsa e caratteristiche di ogni cromosoma. Anche con questa tecnica il cromosoma soprannumerario nella sindrome di Down risulta appartenere alla coppia cromosomica breve deI gruppo G dotata di una banda iperscura nella regione prossimale delle braccia lunghe m, mentre il cromosoma di Filadelfia viene ad essere associato al paio lungo presentante una colorazione pallida con regione scura in corrispondenza del centromero (figg. 9 e 10). L'appartenenza a diverse coppie cromosomiche neU' ambito del gruppo G sarebbe ulteriormente dimostrata da alcuni studi autoradiografici ns che dimostrerebbero come il Ph I appartenga al paio replicantesi precocemente, mentre l' extracromosoma nel mongolismo appartenga al paio replicante pih tardivamente. In base aUa classificazione adottata a Denver e a Chicago (fig. 11) e alle caratteristiche di fluorescenza e di banding, la sindrome di Down associata al paio cromosomico pih piccolo dovrebbe essere pertanto identiflcata come trisomia 22. Tuttavia, per riconoscimento comune aUa IV Conferenza per la Standardizzazione in Cito~enetica (Pari# I970), si ~ convenuto too di attenersi alia classificazione del pattern fluorescente dato da CASVERSSONe Coll. 38 e di classificare il cromosoma 21 come il cromosoma associato alla sindrome di Down e come pih piccolo del cromosoma 22. Questa revisione classificativa faciliterebbe anche il compito di ridenominare una sindrome da tempo definita come ~ trisomia 21 ~, quale appunto il mongolismo. Per definizione, il cromosoma Ph 1 verrebbe quindi ad appartenere alla coppia opposta, e cioO la 22. Questi dati, in parte inaspettati, banno riproposto il problema dei rapporti tra cromosoma Filadelfia e fos]atasi al-

also supported by the use of the so-called 'banding' techniques which make it possible, by means of various processes of DNA denaturation 4, 43, 63, 84,223to obtain a demonstration of intense staining with Giemsa of heterochromatic bands characteristic of each chromosome. Also with this technique the supernumerary chromosome in Down's syndrome is found to belong to the short chromosomal pair of G group with a very dark band in the proximal long arm region 1~1,whilst the Philadelphia chromosome is then associated to the long pair showing a pale staining with dark region connected with the centromere (figs 9 and 10). The belonging to different chromosomal pairs within G group may be further demonstrated by some autoradiographic studies 17s which would show that Ph 1 belongs to the early replicating pair, whilst the extrachromosome in mongolism would seem to belong to the later replicating pair. On the ground of the classification agreed in Denver and Chicago (fig. 11) and the characteristics of fluorescence and banding, Down's syndrome associated with the smaller chromosomal pair should therefore be identified as 22 trisomy. However by general agreement at the IV Conference on Standardization in Cytogenetics (Paris 1970) it was established 100 to conform to the classification of fluorescent pattern given by CASPERSSON et al. 38 and to classify 21 chromosome as the chromosome associated with Down's syndrome and as smaller than 22 chromosome. This revised classification would also facilitate the task for recalling a syndrome which for a long time has been defined as '21 trisomy', as is just the case for mongolism. By definition, Pht chromosome would thus belong to the opposite pair, i.e. 22. These partly unexpected data have once again raised the problem of relationships between Philadelphia chromo-

The fact that Ph I chromosome is present in a very high percentage of cells in nearly all cases of CML suggests that it is strictly connected with the pathogenesis of this morbid form. I f it simply represents an epiphenomenon of the leukemogenesis or a 'by-product', it would be less constant. It is possible that leukemogenous agents may induce the malignancy and Ph I in a given cell as separated but concomitant phenomena, or else it is possible that their action is at first located at the level of a G chromosome which subsequently interferes in the malignant proliferation. Ph ~ hanno un vantaggio selettivo di crescita sulle altre cellule lo, n e gradualmente prendono il sopravvento imponendosi come linea cellulare secondo le modalit~ evolutive della proliferazione clonale. Probabilmente per determinare l'alterazione cellulare caratteristica della LMC sono necessarie una ~s (tab. 6) o poche mutazioni (all'incirca quattro, secondo GALTON85), ma ancora oggi rimane pura congettura stabilire a quale livello interviene una delezione di un cromosoma G ad evocare l'anomalia caratteristica. Sulla base dei rapporti tra reperti citogenetici e citologici nella LMC si era giunti sin dai primi tempi delle indagini cromosomiche al concetto che l'anomalia specifica (Ph I) non era strettamente limitata alla serie granulocitaria, ma interessava verosimilmente anche la serie eritroide e megacariocitaria. Queste osservazioni riportavano in discussione il problema del ~ livello ~> dell'anomalia cromosomica, in quanto una comunanza del Ph I a filiere proliferative diverse conduceva direttamente ad ammettere Pinsorgenza della mutazione cromosomica almeno a livello della cellula staminale. I1 problema citogenetico sotto questo profilo veniva quindi a sovrapporsi alle impostazioni strettamente citologiche cbe gia diversi AA., ed in particolare FESRATA 7sper primo, avevano dato delle forme leucemiche, come espressione di un difetto della cellula staminale. Alcune osservazioni, come quella di FIALKOW e Coll. 7~ riguardante donne eterozigoti per la glucosio-6-fosfato-dei-
cells 10, 11 and are gradually prevailing as a cellular line from the point of view of clonal proliferation evolutive pattern. In order to determine the cellular alteration which is characteristic of CML, one 13 (tab. 6) or few mutations (about four according to GALTON85) are probably required, but it still remains a mere conjecture to establish at what level acts a deletion of G chromosome which brings forth the characteristic anomaly. On the ground of the relationships between cytogenetic and cytological findings in CML even from the beginning of the investigations on chromosomes the opinion had been reached that the specific anomaly (Ph I) was not strictly limited to the granulocytary series, but quite probably involved also the erythroid and megakaryocytic series. These observations once more evidenced the problem of the chromosomal anomaly 'level', because a common existence of Ph I at different proliferative series brought directly to admit the appearance of chromosomal mutation at least at level of staminal cell. From this angle the cytogenetic problem was placed above the strictly cytologic explanations that various authors, particularly FERRATA 7S, had given of leukemic forms as an expression of a defect of the staminal cell. Questi dati tuttavia non hanno trovato attualmente sicure conferme ma anzi, talora, osservazioni in netta contraddizione. ~ verosimile che il Ph 1 possa originare in pih di una singola stem cell e che non necessariamente debba originare a livello della stem cell. Per quanto riguarda la pluralita delle stem cells interessate, depone a favore di questa ipotesi il caso di un paziente affetto da LMC Ph* + , portatore di mosaico 4 6 , X Y / 4 7 , X X Y presente sia nei linfociti che nel midollo osseo, in cui il Ph l era distribuito in entrambe le linee cellulari. In questo caso era evidente la necessita che il Ph I fosse insorto in stem cells diverse. Analogamente, FIALKOW e Coll. 77 e LAWLER e BANGER144hanno esaminato la distribuzione del locus Xg (legato al sesso) in eritrociti di /emmine con LMC P h i + . Poich8 i risultati della distribuzione corrispondevano a quella di fernmine normali, si poteva concludere che la teoria della genesi unicellulare del Ph 1 era errata 0 che il locus Xg sul cosiddetto cromosoma X inattivato, non era inattivato. Anche il semplice esame morfolo#co del Ph I deporrebbe infine a [avore della pluralita delle stem cells interessate alla produzione di questa anomalia. ~ noto che nella stessa preparazione cromosomica le varie metaJasi posseggono cromosomi Ph I relativamente diversi morfologicamente, almeno per quanto riguarda l'indice centromerico. Poich8 non si pub supporre che nello stesso caso la diversa morfologia dei cromosomi Ph I sia l'espressione semplicistica di diversi gradi di spiralizzazione cromatidica, ~ necessario ammettere anche per

Up to now these data have not found sure confirmations but, on the contrary, were sometimes strongly contradicted. Ph I may probably originate in more than one stem cell and it must not necessarily originate at the level of the stem cell. As regards the great number of involved stem cells, this hypothesis is supported by the case of a patient suffering from Ph I + CML carrier of a 46,XY/47, XXY mosaic present both in lymphocytes and bone marrow, where Ph I was distributed in both cellular lines. In this case Ph I surely originated in different stem cells. Similarly, FIALKOWet al. rl and LAWLER and BANGER144 investigated the distribution of Xg locus (linked to sex) in erythrocytes of females suffering from CML p h i + . As the distribution data corresponded to that of normal females, we could draw the conclusion that the theory of unicellular genesis of Ph I was wrong and that Xg locus on the so-called inactivated X chromosome was not inactivated. Finally, also a simple morphological examination of Ph I would emphasize the great number of stem cells involved in the genesis of this anomaly. It is a known fact that in the same chromosomal preparation the various metaphases have Ph I chromosomes comparatively different from the morphological point of view, at least as far as the centromesic index is concerned. As one cannot suppose that in the same case the different morphology of Ph I chromosomes is simply an expression of various degrees of chromatidic spiral information, it is

necessary to admit also for this phenomenon the intervention of several stem cells in the genesis of the typical anomaly. As regards the primary cellular seat of Ph ~, the staminal cell cannot be always taken into consideration, but there exist conditions where it is possible to demonstrate that chromosomal anomaly evolved in connection with more mature elements, for instance in cases in which Ph ~ seems to be limited to a single cellular proliferative series (erythroid, granulocytary, megakaryocytary). However, under these conditions cannot be excluded the fact that the primitive stem cell connected with chromosomal anomaly (Ph I) has also produced a closely related Phi-- stem line or else that the selective advantage acquired by positive Ph ~cells remained limited to a single cellular series in comparison with the proliferation of bone marrow elements ~0s

DaU'esposizione sopra e//ettuata risulta l'importanza dell'indagine citogenetiea a livello delle /orme di LMC: essa si ~ arricchita negli ultimi anni di vaste casistiche la cui elaborazione ha consentito non solo una riconsiderazione di concetti gi~l in parte accennati all'inizio degli studi citogenetici (proli/erazione clonale, vantaggio selettivo di linee aberrate lo, 11, 12, 13, 145, 146), ma anche di inquadrare sotto schemi megIio delineati [orme proliferative a mor/ologia eonsimile, eppure a decorso clinico e con implicazioni prognostiche diverse. In questo senso ha significato la distinzione eft /ettuata, almeno nell' ambito delle leucemie dell' adulto, tra /orme Ph I + e/orme Ph L--. Ma lo studio citogenetico ha oggi consentito di ravvisare anche elementi prognostici a significato pi~ generale nel decorso della LMC, come ad esempio il subacutizzarsi della/orma al comparire di un quadro aneuploide di tipo iperdiploide o l'approssimarsi di una remissione in base alla comparsa di meta/asi ipodiploidi.
The above description shows the importance of a cytogenetic research in CML forms; in the course of last years this research was supported by many case reports the elaboration of which not only has made possible the revisal of concepts already partly outlined at the beginning of the cytogenetic studies (clonal proliferation, selective advantage of aberrant lines lo, 11, 12, 13, 145, 146), but also to arrange under better traced schemes proliferative forms having a similar morphology and yet showing a different clinical course, also with different prognostic implications. In this scheme, the distinction made at least within the leukemiae of adults, between PhX+ and Ph I - forms is of considerable importance. But the cytogenetic study has now allowed to perceive also prognostic elements with a more general significance in the course of CML, such as for instance sub-exacerbation of the form at the appearance of an aneuploid picture of a hyperdiploid type or else the approaching of a remission on the ground of the appearance of hypodiploid metaphases. 289

Se dal lato clinico-pratico gli insegnamenti derivati dallo studio citogenetico sono stati numerosi, altrettanto /ruttuose sono state le osservazioni riferibili a fini strettamente biologici: la dissociazione delle coppie cromosomiche interessate neUa LMC e nel mongolismo ha portato a meglio definire il ruolo dei cromosomi G nel controllo di talune attivita enzimatiche quali la fos/atasi alcalina e galattoso- I-fos/ato- uridil-trans/erasi u2 Non meno importanti sono infine le implicazioni inerenti all'interessamento delle stem cells nell'insorgenza dell'anomalia cromosomica: esse hanno dato elementi di supporto alla teoria ferratiana della LMC, anche se ancora oggi, come in ogni altra situazione tumorale, il ruolo patogenetico dell'anomalia in esame rimane indefinito.
I f the acquired k n o w l e d g e through cytogenetic study has been ample from the clinico-practical p o i n t of view, equally advantageous have been the observations attributable to strictly biological purposes: the dissociation of chromosomal pairs involved in C M L and in mongolism has led to a b e t t e r definition of the role of G chromosomes in the control of some enzymatic activities such as alkaline phosphatase and galactose-Iphosphate-uridyl-transferase 212. Finally, no lesser i m p o r t a n t are the implications relating to the i n v o l v e m e n t of stem cells in the appearance of chromosomal anomaly: it have given s u p p o r t i n g elements to the theory of FERRATA regarding C M L even if today, just as in any o t h e r turnout situation, the pathogenetic role of the anomaly u n d e r examination is still undefined.
RIASSUNTO I1 cromosoma di Filadelfia (Ph ~) rappresenta un elemento distintivo (marker) della maggior parte delle forme di leucemia mieloide cronica (LMC). Tuttavia alcuni quadri particolari di LMC, quali quelli dell'infanzia, la forma a polinucleati maturi tipo Emil-Weil, le forme ad eosinofili, sono in genere caratterizzate dall'assenza del cromosoma di Filadelfia. Nell'adulto, il ritrovamento del P h I pu6 essere utilizzato a scopo diagnostico, essendo le forme Ph i _ caratterizzate da un decorso pih severo e da una sopravvivenza mediana minore rispetto alle forme Phi+. Nel corso delle crisi blastiche della LMC il Ph i pus associarsi ad altre anomalie strutturali cromosomiche che testimoniano della evoluzione clonale di alcune linee proliferative avvantaggiate. Malattie diverse dalla LMC possono dimostrare occasionalmente cromosomi simili al Ph:: h stato pertanto proposto di considerare il cromosoma di Filadelfia non come cromosoma distintivo della LMC, ma piuttosto come marker delle sindromi mieloproliferative in generale. Le pifi recenti acquisizioni in base allo studio in fluorescenza dopo colorazione con chinacrina dimostrano che il Ph i appartiene al paio pih lungo dei cromosomi del gruppo G (21), mentre il cromosoma interessato nella sindrome di Down appartiene al paio di cromosomi pih corto (G 22). Tall dati, in accordo anche con gli studi di banding, dimostrano come non esista un rapporto tra queste due aberrazioni, in particolare per quanto riguarda una supposta azione di controllo genetico del1'attivitfi della fosfatasi alcalina (bassa in genere nella LMC ed elevata nella sindrome di Down) localizzabile a livello di geni dei cromosomi del gruppo G. I rapporti tra Ph ~ e stem cells dimostrerebbero infine come il Ph * possa insorgere a livello di cellule molto indifferenziate; rimane invece ancora oggi controversa la sequenza degli eventi patogenetici coinvolti nel determinismo dell'anomalia. SUMMARY Philadelphia chromosome (Ph') represents a marker of the most part of chronic myeloid leukemia (CML). However, some particular patterns of CML such as the infantile ones, the form with mature polynucleates of Emil-Well type, the eosinophilic forms are as a rule characterized by the absence of Philadelphia chromosome. In adults the finding of Ph ~ may be utilized for diagnostic purposes, as Ph:-- forms are characterized by a more severe course and a lesser mean survival in comparison with Phi+ forms. During the blastic crises of CML Ph I may be associated with other structural chromosomal anomalies which prove the clonal evolution of some advantaged proliferative lines. Diseases different from CML may occasionally show Phklike chromosomes; it has been therefore suggested to consider the Philadelphia chromosome not as a CML distinguishing chromosome, but rather as a marker of myeloproliferative syndromes. The most recent acquisitions on the ground of the fluorescence study after quinacrine staining show that Ph i belongs to the longer pair of G group's chromosomes (21), whilst the chromosome involved in Down's syndrome belongs to 290

the shorter pair of chromosomes (G 22). These data, in agreement also with banding studies, show that there is no relationship between these two aberrations, particularly with regard to a hypothetical action of genetic control for the alkaline phosphatase activity (generally low in CML and high in Down's syndrome) which may be located at the level of group G chromosomes genes. Finally, the relationships between Ph ~ and stem cells would demonstrate that Ph I may appear at the level of very undifferentiated cells; on the other hand the succession of pathogenetic events involved in the determinism of the anomaly is still controversial.
