Real-time spatial compound sonography of Achilles tendon in patients with heterozygous familial hypercholesterolaemia and normal physical examination

Abstract

Riassunto

Purpose. This study was undertaken to assess the prevalence and ultrasound features of Achilles tendon xanthomas (ATX) in patients with heterozygous familial hypercholesterolemia (HFH) and normal physical examination studied with high-resolution ultrasonography (HRUS) and, secondarily, to evaluate the role of real-time spatial compound sonography (CS) in terms of image quality. Materials and methods. Both Achilles tendons of 40 patients with HFH were studied with HRUS and CS. Two experienced radiologists evaluated by consensus the presence of ATX described as (1) tendon thickening and/or (2) focal hypoechoic areas and the quality of images obtained with the two techniques. Results. Ten out of 80 tendons showed thickening (mean: 11.2 mm). Twelve xanthomas 4.1–9.8 mm were identified in 9/80 tendons of five patients. In 5/80 tendons, both tendon thickening and focal hypoechoic areas were observed. There was no difference in the number of xanthomas detected at conventional US or CS. With respect to image quality, the performance of CS was considered significantly higher than HRUS in 72/80 (90%) cases and equal to HRUS in the remaining 8/80 (10%) (p<0.001). Conclusions. CS is an effective tool in the assessment of ATX in patients with HFH and normal physical examination, and provides a better image quality when compared with HRUS.

Obiettivo. Valutare l’incidenza e gli aspetti ecografici degli xantomi del tendine d’Achille (XTA) in pazienti con ipercolesterolemia familiare eterozigotica (IFE) ed esame clinico negativo, studiati con ecografia ad elevata risoluzione (HRUS) e, secondariamente, l’apporto fornito dalla “real-time spatial compound sonography” (CS) in termini di qualità dell’immagine. Materiali e metodi. Entrambi i tendini d’Achille di 40 pazienti con IFE sono stati studiati con HRUS e CS. Due esperti radiologi hanno valutato in consenso sia la presenza di XTA intesi come: a) incremento dello spessore e/o b) presenza di aree focali ipoecogene, sia la qualità delle immagini ottenute con entrambe le tecniche. Risultati. Dieci su 80 tendini erano ispessiti (media: 11,2 mm). In 9/80 tendini di 5 pazienti sono stati identificati 12 xantomi (media: 7,1 mm). In 5/80 tendini si è osservato l’ispessimento associato alla presenza di aree ipoecogene. Il numero di xantomi identificati si è rivelato uguale con HRUS o CS. Riguardo la qualità dell’immagine, la CS è stata valutata significativamente superiore in 72/80 (90%) casi o, nei rimanenti 8/80 (10%), pari all’HRUS (p<0,001). Conclusioni. L’ecografia CS è in grado di individuare XTA in pazienti con IFE ed esame clinico negativo, offrendo una migliore qualità dell’immagine rispetto all’HRUS.

Introduction

Familial hypercholesterolaemia (FH) is an inherited autosomal dominant disorder of lipoprotein metabolism characterised by a defect in the low-density lipoprotein (LDL) receptor gene [1]. This leads to progressively rising LDL cholesterol levels, lipid deposition in the Achilles tendon in the

form of xanthomas and, although still the subject of investigation, a significant increase in mortality due to coronary disease [2, 3]. FH may manifest in the heterozygous form (HFH), with an incidence of 1/500 individuals in the general population, or in the homozygous form, with an incidence of 1/1,000,000 [4]. About 10 million persons worldwide are estimated to suffer from familial hypercholesterolaemia, which is correctly diagnosed in only 10% [5]. Common clinical criteria for a diagnosis of HFH include an early history of coronary atherosclerosis, increased total cholesterol levels and the presence of tendon xanthomas, usually affecting the Achilles tendons, the extensor tendons of the hand and elbow, and the patellar tendons [6]. In view of its anatomical–structural characteristics (site and size), the Achilles tendon is easy to evaluate. Ultrasonography (US) in particular is a reliable method for measuring the tendon thickness and identifying possible lipid deposition, both in the form of xanthomas, which appear as focal or confluent hypoechoic areas, and as a diffuse and heterogeneous tendon thickening with or without calcification, even when the physical examination is completely normal [7, 8]. Recently, a new sonographic technique has been introduced – real-time spatial compound sonography (CS) – which relies on the possibility of electronically steering the US beam generated by the array of transducer elements in order to obtain three to nine, in part overlapping, scans of an object from multiple view angles [9]. Application of this technique to the sonographic study of various organs and systems, especially the musculoskeletal system, has led to improved image quality compared with conventional sonography as a result of a reduction in background noise, spurious echoes and other acoustic artefacts, such as anisotropy [10–13]. The primary aim of this study was to evaluate the incidence and sonographic patterns of Achilles tendon xanthomas (ATX) in patients with HFH and clinically normal tendons studied with high-resolution ultrasound (HRUS). The secondary aim was to assess the contribution of CS technology in terms of image quality.

Materials and methods Patients After obtaining approval by our institution’s ethics committee, over a period of 2 years, we recruited 40 consecutive patients with genetically proven HFH. All patients (19 men and 21 women, age range 11–74 years, mean age 46.9 years) underwent HRUS and CS of both Achilles tendons after providing their written informed consent. No patient had either clinically detectable abnormalities of Achilles tendon or clinical history of tendinitis or previous Achilles tendon injuries. Examination technique All examinations were performed by two expert radiologists (over 15 years’ experience in US) on an ATL HDI 5000 (ATL, Bothell, WA., USA) scanner with a 7- to 12-MHz lin-

Si stima che circa 10 milioni di persone in tutto il mondo sono affette da ipercolesterolemia familiare, di cui solo il 10% correttamente diagnosticato [5]. I criteri clinici usualmente presi in considerazione nella diagnosi di IFE sono costituiti, soprattutto, da una storia di precoce aterosclerosi coronarica, dall’aumento della concentrazione di colesterolo totale e dalla presenza di xantomi tendinei, questi ultimi più frequentemente apprezzabili in corrispondenza dei tendini d’Achille, dei tendini estensori delle mani, del gomito e dei tendini patellari [6]. In considerazione delle sue caratteristiche anatomo-strutturali (sede, dimensioni), il tendine d’Achille può essere facilmente esaminato e, in particolare, l’ecografia risulta un metodo affidabile per misurare il suo spessore ed individuare l’eventuale accumulo di lipidi, sia sotto forma di veri e propri xantomi, evidenziabili come aree ipoecogene focali o confluenti, sia come diffuso e disomogeneo ispessimento tendineo, accompagnato o meno dalla presenza di calcificazioni, anche quando l’esame obiettivo locale sia del tutto negativo [7, 8]. Recentemente, è stata introdotta una nuova tecnica ecografica – real time spatial compound sonography (CS) – la quale sfrutta la possibilità di angolare elettronicamente il fascio ultrasonoro generato dalla linea di elementi trasduttori al fine di effettuare diverse (da tre a nove) scansioni, in parte sovrapposte, di un oggetto da diversi angoli di vista [9]. L’applicazione di tale tecnica allo studio ecografico di vari organi ed apparati, con particolare riferimento al sistema muscolo-scheletrico, ha mostrato una migliore qualità dell’immagine rispetto all’ecografia convenzionale, principalmente grazie alla riduzione del rumore di fondo, degli echi spuri e di altri artefatti acustici, quali l’anisotropia [10–13]. L’obiettivo principale di questo lavoro è stato quello di valutare l’incidenza e gli aspetti ecografici degli xantomi del tendine d’Achille in pazienti con IFE ed esame clinico dei tendini negativo, studiati con ecografia ad elevata risoluzione (HRUS). Quale obiettivo secondario, è stato valutato l’eventuale apporto fornito dalla tecnologia CS in termini di qualità dell’immagine.

Materiale e metodi Pazienti Durante un periodo di due anni, ottenuta l’approvazione da parte del comitato etico, 40 pazienti consecutivi (19 uomini e 21 donne, età 11–74 anni, media 46,9 anni) affetti da IFE geneticamente accertata, sono stati sottoposti, previa acquisizione del consenso informato in forma scritta, ad ecografia ad alta risoluzione (HRUS) e CS di entrambi i tendini 563

ear-array probe and real-time compound spatial imaging (CS) technology. Once set, the technical parameters remained unchanged throughout the sonographic examination. The Achilles tendons were scanned axially and longitudinally with the patients in the prone position and holding their ankles flexed over the edge of the couch with the tendon at maximum extension to facilitate contact between the probe and the tendon [7]. Special attention was paid to keeping the probe parallel to the tendon. Tendon thickness was determined on transverse and sagittal scans by measuring the anteroposterior diameter at the point of maximum thickness [14]. For tendons that did not appear thickened, measurements were made 2-cm cranially from the distal insertion. Each study consisted of the acquisition of four images, two axial and two longitudinal, obtained with conventional US and CS techniques, respectively. The images had identical settings with the exception of grey-scale gain, which was optimised for the imaging technique being used. All images were stored on the scanner’s hard disk and then sent as raw data to a PC connected to the scanner by means of an Ethernet connection. Overall examination time was 5–7 min. On-site analysis The morphological criteria used for diagnosing xanthoma were: (1) tendon thickening intended as maximum anteroposterior diameter greater than 7.1 mm, a value by at least two standard deviations (SD) higher than the normal sonographic values (5.7±0.7 mm), and (2) evidence of uni- or multifocal hypoechoic lesions, the largest diameter of which was also measured. In addition, tendon echostructure was classified into homogeneous or heterogeneous, and the presence of calcification was assessed. Structural abnormalities were classified as (1) no lesions, (2) uni- or multifocal hypoechoic lesions and (3) diffuse alterations [4, 6, 15, 16]. Off-site analysis The digital images stored on the workstation were retrospectively reviewed by two expert radiologists not involved in the sonographic examinations and blinded to the patients’ clinical data and examination technique. The reviewers were presented the images in random order, with all technical and personal data masked. For each image, they were asked to evaluate overall image quality in terms of the presence of artefacts, such as: 1. Anisotropy, which manifests when the US beam is not perfectly perpendicular to the tendon fibrils, leading to the formation of hypoanechoic blind spots 2. “Coherent wave interface” or “speckle”, caused by excessive background noise, which gives a grainy appearance to an otherwise homogeneous region, reducing image contrast and spatial resolution 3. Presence of spurious echoes or clutter, which hinders detection of lesions smaller than 10 mm 4. Lateral acoustic shadows, the degree of evidence of normal fibril structures and possible lesions (visibility and sharpness in relation to the adjacent fibril structures) [9, 12]. The reviewers consensually expressed their judgement on 564

d’Achille. Nessun paziente presentava anormalità clinicamente rilevabili del tendine d’Achille né storia clinica di tendinite o di pregresse lesioni tendinee in sede achillea. Tecnica d’esame Gli esami sono stati condotti da due esperti radiologi (più di 15 anni d’esperienza di ecografia) con un apparecchio ATL HDI 5000 (ATL, Bothell, WA, USA) dotato di sonda lineare 7–12 MHz e di tecnologia real-time compound spatial imaging (CS). Una volta regolati, i parametri tecnici sono stati mantenuti invariati per tutta la rimanente durata dell’indagine ecografica . I tendini d’Achille sono stati esaminati mediante scansioni sui piani assiale e longitudinale, col paziente in posizione prona e caviglie flesse al di fuori del lettino da esame con il tendine in massima estensione per facilitare il contatto tra la sonda ed il tendine [7]. Particolare attenzione è stata posta nel mantenere la sonda perpendicolare al tendine. Lo spessore del tendine è stato misurato mediante scansioni trasversali e sagittali, misurando il diametro antero-posteriore in corrispondenza del punto di maggiore ispessimento [14]. In tendini che non presentavano ispessimento, le misurazioni sono state effettuate 2 cm cranialmente all’inserzione distale. Ogni studio è stato completato con l’acquisizione di quattro immagini, due sul piano assiale e due su quello longitudinale, ottenute rispettivamente con metodica ecografica convenzionale e con tecnica “compound”. Le immagini presentavano identica regolazione dei parametri tecnici fatta eccezione per il guadagno in scala di grigi, ottimizzato dall’operatore in relazione alla tecnica usata. Tutte le immagini sono state archiviate sul disco rigido in dotazione all’unità ecografica e poi inviate, sotto forma di dati grezzi, ad un personal computer (PC) collegato all’ecografo attraverso una connessione in standard Ethernet. Il tempo complessivamente richiesto per l’effettuazione dell’esame è stato di 5–7 minuti. Analisi “on-site” I criteri morfologici utilizzati dagli esaminatori per la diagnosi di xantoma sono stati: 1) l’incremento dello spessore tendineo ovvero quando il suo diametro massimo antero-posteriore risultava maggiore di 7,1 mm, valore corrispondente ad almeno 2 deviazioni standard in più rispetto ai valori ecografici normali (5,7±0,7 mm) e 2) il riscontro di lesioni ipoecogene uni o multifocali misurate nel loro diametro maggiore. È stata, inoltre, valutata l’ecostruttura tendinea suddivisa in omogenea ed eterogenea nonché la presenza di calcificazioni. Le anomalie strutturali sono state classificate come: 1) assenza di lesioni; 2) lesioni ipoecogene uni o multifocali; 3) alterazioni diffuse [4, 6, 15, 16]. a five-point scale, and the degree of evidence of structural abnormalities was consequently rated as: 1. Definitely present 2. Probably present 3. Nonassessable 4. Probably absent 5. Definitely absent In particular, reduction of anisotropy was defined as the correct visualisation without interruptions of the tendon fibres, whereas reduction of the other artefacts was described in terms of reduced speckle and clutter. The improvement in image quality was related to the ability to visualise normal tendon fibres and/or differentiate them from possible lesions [12]. Statistical analysis was done using the t test for paired samples, with statistical significance set at p<0.05.

Results Sixty-six out of 80 tendons showed no significant changes in size (anteroposterior diameter: 4.2–6.9 mm, mean: 5.5 mm) or structure. Fourteen out of 80 (17.5%) tendons in seven (6.2%) patients (four men, three women, age: 24–57 years, mean: 47.3 years) showed dimensional and/or morphostructural alterations (Table 1). In detail, ten (12.5%) tendons in five patients (three men, two women, age: 24–57 years, mean: 46.4 years) were enlarged (8.3–15 mm, mean: 11.2 mm). Nine tendons in five patients (two men, three women, age: 43–57 years, mean: 50.8 years) showed 12 focal hypoechoic areas classified as xanthomas (largest diameter: 4.1–9.8 mm, mean: 7.1 mm), five of which were located on the left and seven on the right. Of these, four patients had bilateral involvement of Achilles tendons, with unifocal (Fig. 1) or multiple xanthomas, whereas the remaining case had unilateral involvement with multiple xanthomas (Fig. 2). Five out of 80 tendons (6.2%) in three patients exhibited only increased anteroposterior diameter (10–15 mm, mean 12.4 mm). In five out of 80 (6.2%) tendons in three patients, the increase in anteroposterior diameter (8.3–12 mm, mean 10 mm) was associated with focal hypoechoic lesions (4.8–8.4 mm, mean 6.4 mm). In two patients the tendons were thickened and calcified bilaterally (Fig. 3). The number of alterations identified by HRUS and CS was equal. The degree of evidence of structural abnormalities in the 80 Achilles tendons studied with HRUS and CS is shown in Table 2. As regards the presence of sonographic artefacts, CS was considered significantly better than conventional US in 72/80 (90%) cases and equivalent to conventional US in the remaining eight cases (10%) (p<0.001) (Fig. 4).

Discussion The presence of ATX is considered virtually pathognomic of HFH [5]. ATX are extremely rare in the other forms of hyperlipidaemia, such as type III hyperlipoproteinaemia, cerebrotendinous xanthomatosis, apolipoprotein B hyperproduc-

nate da due esperti radiologi, non coinvolti negli esami ecografici e tenuti all’oscuro dei dati clinico-anamnestici della tecnica utilizzata per ogni esame. Le immagini sono state presentate in ordine casuale ed è stata nascosta ogni informazione riguardo ai parametri tecnici e/o ai dati anagrafici. Agli esaminatori è stato chiesto di valutare, per ciascuna immagine, la qualità complessiva in relazione alla presenza o meno di artefatti, quali: 1. l’anisotropia, che si manifesta quando il fascio ultrasonoro non è perfettamente perpendicolare alle fibrille tendinee causando la formazione di areole ipo-anecogene di mancata visualizzazione; 2. “coherent wave interface” o “speckle”, riconducibile ad un eccessivo rumore di fondo che conferisce un aspetto granulare ad una regione altrimenti omogenea, riducendo il contrasto e la risoluzione spaziale dell’immagine; 3. la presenza di echi spuri o “clutter”, che rende difficoltoso evidenziare lesioni di dimensioni inferiori a 10 millimetri; 4. le ombre acustiche laterali e altri artefatti acustici, nonché al grado di evidenza delle normali strutture fibrillari che di eventuali lesioni (visibilità e chiarezza rispetto alle strutture fibrillari tendinee adiacenti) [9, 12]. I due radiologi hanno espresso, in consenso, la loro valutazione graduandola su una scala di cinque livelli ed il grado di evidenza delle anomalie strutturali è stato conseguentemente distinto in: 1. sicuramente presente; 2. probabilmente presente; 3. non valutabile; 4. probabilmente assente; 5. sicuramente assente. In particolare, la riduzione dell’anisotropia è stata definita come la corretta rappresentazione, senza soluzione di continuità, delle fibre tendinee, mentre la riduzione degli altri artefatti è stata descritta in termini, soprattutto, di riduzione dello “speckle” e del “clutter”. Il miglioramento della qualità dell’immagine è stato rapportato alla capacità di visualizzare le normali fibre tendinee e/o differenziare queste ultime da eventuali lesioni presenti [12]. L’analisi statistica è stata effettuata utilizzando il test t di Student per campione doppio con livello di significatività statistica fissato a p<0,05.

Risultati Sessantasei/80 tendini non hanno presentato, all’indagine ecografica, significative alterazioni dimensionali (diametro antero-posteriore: 4,2–6,9 mm, media: 5,5 mm) né morfostrutturali. Quattordici/80 (17,5%) tendini in 7 (6,2%) pazienti (quattro maschi, tre femmine, età: 24–57 anni, media 47,3 anni) hanno mostrato alterazioni dimensionali e/o morfostrutturali (Tabella 1). tion and hyper-b-sitosterolaemia [6, 8, 17]. ATX are accompanied by an enlargement of the tendon caused not only by fat deposition but also by oedema and inflammation, which may lead to achillodynia, as reported at least once in their lifetime by 30% of patients with HFH, and, rarely, to tendon rupture [16, 18]. The recent discovery of effective pharmacological treatments has placed much emphasis on the early diagnosis of HFH, above all to prevent atherosclerosis, which has a particularly poor prognosis when the coronaries are involved due to the increased risk of fatal ischaemic events [2, 3, 19]. Xanthomas, however, which are often the first clinical sign of HFH and usually manifest in the fourth decade of life, are often missed on clinical examination, whereas they are perfectly identifiable on US. Because of this and the fact that the technique is widely available and has a good cost–benefit ratio (even in terms of biological costs), US has become the most commonly employed imag566

In 9/80 tendini di 5 pazienti (due maschi, tre femmine; età: 43–57 anni, media: 50,8 anni) sono state identificate 12 aree focali ipoecogene classificate come xantomi (diametro maggiore: 4,1–9,8 mm, media: 7,1 mm), cinque delle quali localizzate a sinistra e sette a destra. Di questi, quattro pazienti hanno mostrato il coinvolgimento bilaterale di entrambi i tendini, con localizzazioni xantomatose singole (Fig. 1) o multiple, mentre nel rimanente caso il coinvolgimento era solo monolaterale ma con localizzazioni xantomatose plurime (Fig. 2). Cinque/80 (6,2%) tendini in 3 pazienti hanno presentato esclusivamente un incremento del diametro antero-posteriore (10–15 mm, media 12,4 mm). Il numero di alterazioni identificate si è rivelato uguale con US o CS. Il grado di evidenza delle anomalie strutturali degli 80 tendini d’Achille esaminati con HRUS e CS è indicato nella Tabella 2. Per quanto attiene la presenza o meno di artefatti ecografici, la CS è stata valutata significativamente superiore in 72/80 (90%) dei casi o, nei rimanenti 8 (10%) casi, pari all’ecografia convenzionale (p<0,001) (Fig. 4).

La presenza di xantomi del tendine d’Achille è considerata virtualmente patognomonica di IFE [5]. Di contro, gli xantomi del tendine d’Achille sono estremamente rari nelle altre forme di iperlipidemia quali l’iperlipoproteinemia di tipo III, la xantomatosi cerebrotendinea, l’iperproduzione di apolipoproteina B e l’iper-b-sitosterolemia [6, 8, 17].Gli xantomi del tendine d’Achille sono accompagnati da incremento dimensionale del tendine, causato non solo dal deposito di lipidi, ma anche da fenomeni di edema e flogosi, che possono condurre ad achillodinia, riferita almeno una volta nella vita dal 30% dei pazienti con IFE, e, seppur raramente, a rottura tendinea [16, 18]. Recentemente, la scoperta di terapie farmacologiche efficaci ha posto grande enfasi sulla diagnosi precoce di IFE, soprattutto al fine di prevenire i fenomeni aterosclerotici, particolarmente infausti quando a localizzazione coronarica, a causa dell’aumentato rischio di eventi ischemici mortali [2, 3, 19]. Tuttavia, gli xantomi, usualmente presenti a partire dalla quarta decade di vita e pur costituendo spesso la prima manifestazione clinica di IFE, non di rado risultano misconosciuti all’esame clinico, mentre risultano 567

ing modality for the assessment of ATX [4, 8, 14, 15]. Traditionally the first-line imaging technique, conventional radiography in the lateral projection and with adequate exposure settings allows determination of tendon thickness and detection of calcification only but remains a projective technique that exposes the patient to ionising radiation [20]. This last limitation is shared by computed tomography (CT) which, while providing accurate measurement of tendon size thanks to the intrinsic contrast between Achilles tendon and the surrounding fat, is unable to measure density values that differ substantially in patients with normal and elevated cholesterol levels [16]. Magnetic resonance imaging (MRI) is only able to detect changes in shape and size and a nonspecific “pointed” appearance but cannot demonstrate lesions unequivocally referable to tendon xanthomas [20–22]. In agreement with the literature, our experience confirms that in a small but nonnegligible series of patients with clinically normal Achilles tendon, US can demonstrate significant tendon alterations, such as thickening, calcification or the presence of xanthomas [7, 8, 15, 19]. The smaller proportion of patients with HFH and ATX seen in our study (6.2%) compared with previous reports (29%–33.1%) is in part related to our study sample not having appreciable tendon abnormalities on clinical examination [6, 19, 23]. In addition, the limited number of patients included and the study design did not allow us to confirm or refute the epidemiological data derived from larger series, according to which subjects with HFH and ATX tend to be older males with higher total and LDL cholesterol levels than those without tendon xanthomas [19]. CS is an ultrasonographic technique that has improved the definition of sonographic images and that has been reported to have diagnostic value, for example, in musculoskeletal imaging and in particular in rotator cuff studies 568

perfettamente evidenziabili all’indagine ecografia, che per questa ragione, oltre che per la larga diffusione ed il favorevole rapporto costo (anche biologico)/beneficio, è l’indagine di diagnostica per immagini maggiormente impiegata per la valutazione del tendine d’Achille allo scopo di individuare gli xantomi [4, 8, 14, 15]. Infatti la radiologia convenzionale, storicamente impiegata come prima tecnica di studio, consente, nella proiezione laterale e con adeguati parametri di esposizione, unicamente la determinazione dello spessore tendineo e l’individuazione di eventuali calcificazioni del tendine, rimanendo pur sempre una tecnica proiettiva ed esponendo il paziente a radiazioni ionizzanti [20]. Quest’ultimo limite è condiviso anche dalla tomografia computerizzata (TC), la quale, pur fornendo misurazioni accurate delle dimensioni del tendine achilleo grazie all’intrinseco contrasto esistente tra il tendine stesso ed il circostante tessuto adiposo, non è in grado di misurare valori di densità che differiscano sostanzialmente in pazienti normo o ipercolesterolemici [16]. La risonanza magnetica, infine, non è in grado di evidenziare, al di là delle alterazioni morfodimensionali e un aspecifico aspetto “puntato”, vere e proprie lesioni ascrivibili con certezza a xantomi tendinei [20–22]. In accordo ai dati di letteratura, la nostra esperienza conferma l’osservazione che l’ecografia, in una piccola ma non trascurabile percentuale di pazienti con tendine d’Achille valutato come del tutto normale all’esame clinico, è in grado di evidenziare significative alterazioni tendinee, quali ispessimento, calcificazioni o ancora la presenza di veri e propri xantomi [7, 8, 15, 19]. La minore percentuale di pazienti con IFE e lesioni xantomatose achillee riscontrata nel nostro studio (6,2%) rispetto a quanto riportato in letteratura (29%–33,1%) è da mettere in relazione, almeno in parte, al gruppo di pazienti selezionato, che non presentava apprezzabili alterazioni tendinee all’esame clinico [6, 19, 23]. Inoltre, la popolazione numericamente limitata e la tipologia del nostro studio non consentono di confermare i dati epidemiologici acquisiti su campioni di più vaste proporzioni, che indicano come i soggetti con IFE e con xantomi del tendine d’Achille tendano ad essere di sesso maschile, più anziani e a presentare livelli di colesterolo totale e LDL più elevati rispetto ai pazienti senza xantomi [19]. La CS è una tecnica ultrasonografica la cui applicazione ha condotto ad un miglioramento della definizione dell’immagine ecografica, che alcuni studi hanno riportato essere utile anche ai fini diagnostici, ad esempio in ambito muscoloscheletrico ed in particolare nel distretto anatomico della cuffia dei rotatori [9, 12, 13, 24]. La CS acquisisce contemporaneamente fino a nove immagini dello stesso oggetto da differenti punti di vista. Tali immagini, una volta elaborate tramite un processo di pesatura digitale, vengono poi rappresentate in un’unica immagine finale di sommazione [9, 24]. Questa tecnica, diversamente dall’ecografia b-mode convenzionale nella quale ogni immagine è ottenuta da un solo angolo di insonazione, effettuando la media dei segnali ricevuti, è in grado di ridurre gli artefatti, migliorando sensibilmente il rapporto segnale rumore e la risoluzione dell’im-

[9, 12, 13, 24]. CS acquires simultaneously up to nine images of the same object from different points of view. After processing by digital averaging, these images are represented as a single summation image [9, 24]. CS in unlike conventional B-mode imaging in which each image is obtained from a single insonation angle; by averaging multiple signals, CS reduces artefacts, substantially improving the signal-to-noise ratio and image resolution [13]. In particular, CS considerably reduces anisotropy and artefacts, such as speckle and clutter [12]. As is known, these aspects are especially important in musculoskeletal imaging, above all in the study of tendons. Only by analysing the tendon fibres perpendicularly is it, in fact, possible to discriminate with any certainty areas of echostructural alteration from the normal fibril network, consisting of the interfaces between the connective tissue septa of the endomysium and the parallel collagen fibre bundles [14, 15]. In particular, the presence of focal hypoechoic areas or a diffusely inhomogeneous pat-

magine [13]. In particolare, l’ecografia CS riduce notevolmente il fenomeno dell’anisotropia ed artefatti quali lo “speckle” ed il “clutter” [12, 25, 26]. Tali aspetti, com’è noto, assumono particolare rilievo in ambito muscolo-scheletrico, soprattutto nello studio delle strutture tendinee. Solo analizzando perpendicolarmente le fibre del tendine è possibile, infatti, discriminare con sicurezza eventuali aree di alterazione ecostrutturale rispetto al normale disegno fibrillare, costituito dalle interfacce tra i setti connettivali dell’endomisio ed i fasci di fibre collagene a decorso parallelo [14, 15]. In particolare, la presenza di aree ipoecogene focali o un aspetto diffusamente disomogeneo vengono considerati, in pazienti con IFE ma senza storia clinica di traumi o tendinite, come altamente suggestivi di xantomi, anche in assenza di conferma istologica [4]. Tuttavia, mentre l’ecografia viene considerata attendibile nella detezione di aree ipoecogene focali in un tendine di per sé iperecogeno e nella stima dell’in569

tern are considered highly suggestive of xanthoma, even in the absence of histological validation in patients with HFH but without a history of trauma or tendinitis [4]. However, although US is reliable for detecting focal hypoechoic areas on a generally hyperechoic tendon and for determining the extent of tendon thickening, it appears to be subject to a greater error margin in the evaluation of echostructural heterogeneity [6]. In our experience, in the study of Achilles tendon by CS provided an image quality that was subjectively considered better than that of conventional US. On the other hand, this better image quality, despite increasing diagnostic confidence, did not translate into a greater rate of detection of ATX. Furthermore, as already suggested for conventional US, CS could be used to monitor regression of xanthomas and/or the reduction in tendon size in patients receiving therapy [27]. There are some limitations to our study. In particular, the assessment of image quality was conducted in a subjective fashion, and a double-blind review of cases would have allowed us to determine inter- and intraobserver variability.

Nella nostra esperienza, la CS ha fornito una qualità delle immagini giudicata soggettivamente migliore rispetto all’ecografia convenzionale per quanto attiene lo studio del tendine d’Achille. D’altra parte, nella serie presa in esame, tale migliore qualità dell’immagine, per quanto utile per una maggiore confidenza diagnostica, non si è tradotta in un più elevato tasso di identificazione di xantomi achillei. Inoltre, l’ecografia CS potrebbe essere utilizzata, come già suggerito per l’ecografia convenzionale, per monitorare la regressione degli xantomi e/o la riduzione delle dimensioni del tendine in pazienti sottoposti a terapia [27]. Il nostro studio presenta dei limiti. In particolare, la valutazione della qualità dell’immagine è stata condotta in maniera soggettiva e una revisione dei casi in doppio cieco avrebbe consentito il calcolo della variabilità intra- ed interosservatore.

Conclusione Conclusion In conclusion, CS sonography proved useful in the evaluation of the Achilles tendon in patients with HFH and normal clinical examination, as it provided early evidence of xanthomas, thereby supplying the clinician with useful information for patient management and optimisation of therapy.

In conclusione, nella nostra esperienza, l’ecografia CS è utile nella valutazione del tendine d’Achille in pazienti con IFE ed esame clinico negativo, nei quali riesce a evidenziare precocemente la presenza di lesioni xantomatose, fornendo al clinico informazioni utili per la gestione del paziente e l’ottimizzazione della terapia.
