Second-generation sonographic contrast agents in the evaluation of renal trauma

Abstract

Riassunto

Purpose. The purpose of this study was to define the indications, diagnostic accuracy and limitations of second-generation sonographic contrast agents in the evaluation of patients with renal trauma. Materials and methods. Between March 2004 and April 2005, 277 patients with blunt abdominal trauma were evaluated. Twenty-eight out of 277 patients had renal lesions, the severity of which was graded according to the organ injury severity scale of the American Association for the Surgery of Trauma (AAST). All the patients enrolled in the study had minor trauma and were evaluated with baseline ultrasound (US), contrast-enhanced US after injection of a second-generation contrast agent (SonoVue) and, if positive, with multiphasic multidetector computed tomography (MDCT). Results. Five out of 28 traumatic parenchymal lesions with perirenal fluid collection were identified at baseline US. All 28 renal parenchymal lesions, with or without perirenal or retroperitoneal haematoma, were identified at contrast-enhanced US. Multiphase MDCT confirmed all the cases that were positive at contrast-enhanced US and demonstrated the integrity of the urinary tract in the delayed phase. Conclusions. Our experience confirmed the diagnostic accuracy of second-generation sonographic contrast material both for diagnosis and for appropriate patient management. In particular, contrast-enhanced sonography proved to be a reliable technique for the evaluation and follow-up of low-grade renal injuries. Its main advantage is reduced radiation exposure, as fewer MDCT examinations are needed, whereas its limitation is the high cost of the technique if used in unselected patients.

Obiettivo. Identificare le indicazioni, l’accuratezza diagnostica ed i limiti dell’ecografia con mezzo di contrasto di II generazione nella valutazione del paziente con trauma renale. Materiali e metodi. Sono stati valutati, tra marzo 2004 ed aprile 2005, 277 pazienti con trauma addominale chiuso. Ventotto/277 pazienti presentavano lesioni renali la cui gravità era stata definita in rapporto all’Injury Severity Scale dell’Associazione Americana per la Chirurgia del Trauma. I pazienti considerati, tutti con trauma minore, sono stati sottoposti ad esame ecografico basale e dopo iniezione di MdC ecografico di II generazione (SonoVue, Bracco) ed ad esame TC multidetettore multifasico in caso di positività. Risultati. Cinque/28 lesioni parenchimali traumatiche con raccolta fluida perirenale sono state identificate all’esame ecografico basale. Ventotto/28 lesioni parenchimali renali, con o senza ematoma perirenale o retroperitoneale, sono state identificate all’esame ecografico con MdC. L’esame TC multidetettore multifasico ha confermato tutti i casi positivi all’esame eco-contrastografico ed ha inoltre definito, all’acquisizione tardiva, la situazione della via escretrice urinaria. Conclusioni. La nostra esperienza ha confermato l’accuratezza diagnostica dell’ecografia con MdC di II generazione sia nella diagnosi della lesione che per la corretta gestione del paziente. In modo particolare l’ecografia con MdC risulta tecnica affidabile per la valutazione del trauma renale di basso grado e nel followup dello stesso riducendo il ricorso a ripetuti esami TC con riduzione della dose radiante. I limiti sono dati dagli alti costi della tecnica se condotta su popolazione non selezionata.

The current diffusion of sonographic contrast agents has extended the indications of ultrasound (US) imaging [1]. In particular, second-generation contrast agents have not only

enhanced the diagnostic potential of US in detecting and characterising liver lesions [2, 3] but also enabled the identification and grading of traumatic lesions affecting other abdominal organs, especially the spleen and liver [4]. The aim of this paper is to define the indications, diagnostic potential and limitations of US with second-generation contrast agents in renal injury on the basis of our experience in a second-level emergency care centre.

Lo scopo del presente contributo è quello di definire le indicazioni, le potenzialità diagnostiche ed i limiti dell’esame ecografico con mezzo di contrasto di II generazione nel trauma renale, in base alla nostra esperienza maturata in un DEA di II livello.

Materials and methods Between March 2004 and April 2005, we evaluated 277 patients with baseline US and contrast-enhanced US after the administration of a second-generation contrast agent (SonoVue, Bracco, Italy). The study population included patients with minor blunt abdominal trauma with or without haematuria, patients who were stable but had undergone parasurgical renal procedures during the previous 12 h (lithotripsy, renal artery stenting, postbiopsy monitoring) and patients with acute abdomen, haematuria, and minimal but sudden drop in blood-cell count. Our study did not include polytrauma patients, as these are always assessed by totalbody multidetector computed tomography (MDCT) as a first-line examination. For paediatric patients or patients under the age of 18 years, the parents’ informed consent was obtained before proceeding to contrast-enhanced US. In all cases, baseline US was integrated by the contrast study performed on a dedicated scanner (Sequoia, Siemens, Germany), with low mechanical index and contrast-specific software [coherent contrast imaging (CCI) 222 patients; cadence contrast pulse sequence (CPS) 55 patients]. Patients were administered a bolus of 2.4 ml of second-generation contrast material (SonoVue) followed by about 10 cc saline solution via peripheral vein. The examination consisted of an assessment of the liver parenchyma, followed by the splenic and renal parenchyma and by the supra- and submesocolic peritoneal recesses, and generated both static images and dynamic clips lasting around 20 s each taken at various times during the examination, depending on the findings. There were no early or late allergic reactions to the intravenous contrast material, which appeared to be very well tolerated by patients and fairly easy to use for the operators, who had received specific training in its correct preparation. Patients with positive US findings underwent MDCT (Light Speed Ultra, GE, USA) before and after the intravenous administration of iodinated contrast material, with a specific multiphase acquisition protocol that allowed correct staging of the lesion and assessment of the integrity of the urinary tract. In addition, our study evaluated contrast-enhanced US in the follow-up of renal injuries using the same examination protocol as used for the initial assessment. The lesions were classified into five grades of increasing severity according to the American Association for the Surgery of Trauma (AAST) classification: grade 1, subcapsular haematoma; grade 2, superficial renal lacerations with perirenal haematoma; grade 3, renal lacerations deeper than 1 cm without extension into the collecting system or evi582

Materiali e metodi Nel periodo compreso tra marzo 2004 ed aprile 2005 sono stati valutati con esame ecografico basale e dopo somministrazione di MdC ev di II generazione (SonoVue, Bracco, Italia) 277 pazienti con trauma addominale chiuso minore, con o senza ematuria, pazienti stabili, ma sottoposti nelle 12 ore precedenti a procedure parachirugiche in ambito renale (litotrissia, posizionamento di stent vascolari in arteria renale, monitoraggio post-bioptico) e che avevano manifestato un quadro di addome acuto, ematuria, minima ma improvvisa riduzione dell’emocromo. Non sono stati considerati nel presente studio i pazienti politraumatizzati in quanto sempre valutati, in prima istanza, con esame TC multidetettore total body. Per la valutazione di pazienti pediatrici o comunque minori si procedeva all’esame eco-contrastografico previo consenso dei genitori. L’esame ecografico basale è stato sempre integrato da quello contrastografico adoperando apparecchio ultrasonografico (Sequoia, Siemens, Germania) dedicato, con tecniche a basso indice meccanico, software specifico in modalità CCI (Coherent Contrast Imaging, 222 pazienti) e CPS (Cadence Pulse Sequence, 55 pazienti) dopo somministrazione, tramite accesso venoso periferico, di una quantità di MdC di II generazione (SonoVue, Bracco, Italia) pari a 2,4 ml in unico bolo seguito da circa 10 cc di soluzione fisiologica con valutazione prima del parenchima epatico, poi di quello splenico ed infine di quello renale e dei recessi peritoneali sovra e sottomesocolici con documentazione iconografica sia statica che con clips della durata di circa 20 secondi ognuna, eseguite in momenti variabili, in rapporto al reperto diagnostico emerso. Non si sono mai verificate reazioni allergiche immediate o tardive dopo somministrazione del MdC ev che è apparso molto ben tollerato dai pazienti ed abbastanza maneggevole da usare dagli operatori preventivamente formati alla corretta preparazione dello stesso. I pazienti che presentavano un quadro positivo venivano sottoposti ad esame TC multidetettore (Light Speed Ultra, GE, USA) prima e dopo somministrazione di MdC iodato ev, con protocollo di acquisizione specifico, multifasico, al fine di stadiare in modo corretto la lesione e di documentare l’integrità o meno della vie escretrice urinaria. Nel nostro studio, inoltre, abbiamo testato la metodica ultrasonografica contrastografica nel follow-up delle lesioni traumatiche re-

dence of urinary extravasation; grade 4, lacerations involving the renal cortex, medulla and the collecting system, injuries to the main renal artery, thrombosis of a segmental renal artery without parenchymal laceration; grade 5, lacerations that completely shatter the renal parenchyma, which appears subverted with devascularisation, disruption of the renal artery, traumatic renal artery occlusion [5].

Results In the 277 patients studied, 45 of whom had traumatic gross haematuria, baseline US identified five cases of renal parenchymal injury with associated perirenal haematoma. Integration with the contrast-enhanced study identified 28 cases of renal parenchymal injury associated or unassociated with perirenal and/or retroperitoneal haematoma (Fig. 1a, b). All cases of positive findings on contrast-enhanced US were confirmed by the following MDCT study, which provided very similar images (Fig. 1c, d). Compared with the baseline study, contrast-enhanced US also allowed better evaluation of the extent of perirenal effusion, and its findings were fully confirmed by MDCT (Fig. 2). In one case of a young patient with postbiopsy perirenal haematoma, MDCT was not considered indispensable, as the pain had regressed fairly rapidly, and the patient was haemodynamically stable. The patient was instead monitored by contrast-enhanced US performed 12 and 48 h after the initial assessment and a further baseline examination 1 week later, with resolution of the pathological findings. The lesions identified on contrast-enhanced US were classified into three main groups based on a comparison between contrast-enhanced US, AAST classification and MDCT findings: (1) minor injury: presence of perirenal haematoma without evidence of renal parenchymal alteration 3 min after the i.v. administration of contrast (8 cases), essentially corresponding to AAST grade 1; (2) moderate injury: perirenal haematoma with parenchymal laceration not extending to the renal hilum (14 cases), corresponding to AAST grades 2 and 3; (3) severe injury: characterised by a deep laceration extending to all parenchymal components with involvement of the hilar region, a condition only in part corresponding to AAST grades 3 and 4, as the method does not allow us to establish whether or not the collecting system is intact (6 cases).

Discussion and conclusions Our study shows that contrast-enhanced US has a high level of diagnostic accuracy in the assessment of renal trauma. It should be noted that the technique offers additional advantages over conventional US (typically, absence of ionising radiation, fast examination times, and ready accessibility), in that it allows accurate evaluation of heavily vascular parenchymas such as the spleen, the liver and the kidneys. This enables the identification of lacerations-contusions and

Le lesioni identificate sono state classificate in cinque diversi gradi di gravità crescente secondo quanto definito dall’AAST: grado 1, ematoma subcapsulare; grado 2, lacerazione superficiale renale con ematoma perirenale; grado 3, lacerazione renale profonda (superiore al centimetro) senza interessamento del sistema escretore e evidenza di stravaso urinario; grado 4, lacerazione interessante la struttura cortico-midollare ed il sistema escretore, coinvolgimento dell’arteria renale principale, trombosi di un’arteria segmentaria senza evidenti lacerazioni parenchimali; grado 5, grave lesione destruente il nefroparenchima che appare sovvertito con devascolarizzazione, distruzione dell’arteria renale, occlusione port-traumatica dell’arteria renale [5].

Risultati Nei 277 pazienti valutati, di cui 45 con macroematuria posttraumatica, l’ecografia basale ha identificato 5 casi positivi per lesione parenchimale renale con associato ematoma perirenale. La successiva integrazione con esame eco-contrastografico ha identificato 28 casi positivi per lesione renale parenchimale associata o meno ad ematoma perirenale e/o retroperitoneale (Fig. 1a, b). Tutti i casi positivi all’eco-contrastografia sono stati confermati dalla successiva valutazione TCms con quadri anche iconograficamente sostanzialmente sovrapponibili (Fig. 1c, d). Si è meglio definita anche l’entità del versamento perirenale adoperando l’ecografia con MdC rispetto all’esame ecografico basale con ottimale conferma da parte dell’esame TC (Fig. 2). In 1 caso, di paziente minore, con ematoma perirenale post-biopsia con un quadro algico regredito abbastanza rapidamente e comunque stabile emodinamicamente, non si è ritenuta indispensabile la valutazione con esame TC ed è stato monitorato con due successivi esami eco-contrastografici a 12 e 48 ore di distanza dalla prima valutazione, e successiva, ulteriore, valutazione con esame ecografico basale ad una settimana con risoluzione del quadro patologico. I quadri eco-contrastografici rilevati sono stati classificati, in base alla nostra esperienza, in tre gruppi principali di lesioni in rapporto ad una comparazione integrata tra esame eco-contrastografico, classificazione AAST e successivo quadro TC: lesione di “grado minore”, presenza di ematoma perirenale senza evidente alterazione parenchimografica renale dopo circa tre minuti dall’somministrazione di MdC ev (8 casi) e corrispondente sostanzialmente al grado 1 della classificazione AAST; lesione di “grado medio”, ematoma perirenale con lacerazione parenchimale che non raggiunge l’ilo renale, corrispondente sostanzialmente ai gradi 2 e 3 della classificazione AAST (14 casi); lesione di “grado alto”, caratterizzata da lacerazione profonda coinvolgente tutte le varie componenti parenchimali con interessamento della regione ilare condizione corrispondente in certo senso parzialmente al grado 3 e 4 in quanto la metodica non ci permette di definire l’integrità o meno del sistema escretore (6 casi). 583

haematomas, which appear as more or less irregularly hypoechoic and avascular areas. In some cases, it also visualises active bleeding as a hyperechoic blush of contrast material secondary to the interruption of a vascular structure (Fig. 3a, b; clip 3) [4, 6–9]. A further advantage of contrast-enhanced US compared with conventional US is the better and more precise depiction of peritoneal and retroperitoneal fluid collections that results from improved definition of enterocolic wall structures, bladder and abdominal organs [10]. Nonetheless, there are also some limitations to the technique that have to be borne in mind: cost of the contrast material, need for scanners with dedicated software, need for operators familiar with this type of examination, and longer examination times. Moreover, contrast-enhanced US does not demonstrate lesions to the urinary tract but only provides 584

Dalla nostra esperienza emerge l’elevata accuratezza diagnostica dell’eco-contrastografia nella valutazione del trauma renale. Vi è da precisare che tale tecnica presenta alcuni ulteriori vantaggi rispetto a quelli tipici dell’esame ecografico classico (assenza di radiazioni ionizzanti, rapidità di esecuzione, facilità di accesso) che sono rappresentati da buona valutazione dei parenchimi fortemente vascolarizzati come milza, fegato e reni. Questo permette un’ottimale identificazione dei focolai lacero-contusivi e degli ematomi che si presentano come aree più o meno irregolarmente ipoecogene-avascolari. Permette anche di identificare, in taluni casi, il sanguinamento attivo come immagine di “blush” iperecogeno secondario alla soluzione di continuo di una struttura vascolare (Fig. 3) [4, 6–9]. Altro vantaggio dell’eco-contrastografia rispetto all’ecografia basale è rappresentato da una migliore e più precisa identificazione dei versamenti retro ed intraperitoneali in rapporto ad una migliore defini-

indirect evidence of injuries to the renal vascular pedicle by depicting areas of renal infarction. As regards the first point, in our experience, the use of contrast-enhanced US appears unjustified in all cases of minor injury without haematuria. The findings in our patients indicate that contrast-enhanced US was more likely to be positive in the following conditions, listed in order of decreasing frequency: in the presence of traumatic gross haematuria, gross haematuria or microhaematuria following renal biopsy or lithotripsy performed during the previous 12 h, traumatic micro-haematuria and normal urinalysis. For this reason, we have adopted the following approach in the workup of minor trauma: baseline US for stable patients with normal urinalysis, followed by contrast-enhanced US after obtaining consent to the use of the contrast agent, only in the event of a diagnostic doubt. In patients with a minor trauma and gross or microhaematuria or those who have undergone potentially harmful procedures within the previous 12 h, baseline US is always followed by the contrast examination. This approach allows a more accurate screening of patients not only for renal lesions but also for commonly associated injuries to other organs. In addition, it allows considerable savings in terms of both financial and staff resources. CT, preferably with a multidetector technique, remains the gold standard imaging test in the polytrauma patient, as it allows assessment of the renal parenchyma and vasculature and the urinary tract [5, 6, 11–14]. Sonography with secondgeneration contrast material should be regarded as a technique that allows only definitely positive cases to be selected

zione delle strutture parietali enterocoliche, della vescica e dei vari organi endoaddominali [10]. Accanto ai vantaggi vi è da tener conto anche degli svantaggi: elevato costo del mezzo di contrasto, necessità di apparecchi ecografici con software dedicati, abitudine da parte dell’operatore a questa tipologia d’esame, allungamento dei tempi d’esame. L’ecografia con MdC, inoltre, non documenta le lesioni delle vie escretrici urinarie ma indirettamente quelle del peduncolo vascolare renale identificando aree infartuali parenchimali renali. Per il primo punto, dalla nostra esperienza, non sembra giustificato l’uso dell’eco-contrastografia in tutti i traumi minori senza ematuria. Infatti come riportato nei risultati dei pazienti esaminati, la possibilità di una positività dell’esame eco-contrastografico era più alta, con frequenza decrescente, in pazienti con macroematuria post-traumatica, macroematuria o micrometaturia in pazienti sottoposti nelle 12 ore precedenti a procedure bioptiche renali o di litotrissia, microematuria post-traumatica, pazienti con esame delle urine normale. Per questo motivo, attualmente, abbiamo adottato la seguente condotta nell’iter diagnostico nel trauma minore: esame ecografico basale per i pazienti stabili e con esame 585

for CT assessment of the integrity of the urinary tract and vascular pedicle [14–16]. The use of contrast-enhanced US is also fundamental in the follow-up of lesions, as it reduces exposure to ionising radiation in this group of patients who are often young, if not children, and also in view of the fact that renal trauma is frequently managed conservatively [4, 17, 18]. It should be stressed that contrast-enhanced US cannot be used in stable polytrauma patients who require a comprehensive CT study extending to multiple body districts [19, 20]. This is why no high-grade lesions appear in our series: these are critical situations, often with lesions to multiple organs, that necessitate assessment by CT to enable adequate trauma grading. 586

delle urine nella norma, nel caso di dubbio diagnostico si procede, previo consenso all’eco-contrasto. In pazienti con trauma minore con macro- o microematuria, o sottoposti nelle 12 ore precedenti a procedure potenzialmente lesive, l’ecografia basale viene seguita in ogni caso dall’eco-contrasto. Ciò sta permettendo un migliore screening per l’identificazione del paziente con lesione renale ma anche di altri organi che a volte ad esso si associano. Questa condotta inoltre permette un notevole risparmio sia in termini di risorse economiche che gestionali del personale preposto. Resta da ricordare che comunque il gold standard nel paziente traumatizzato resta sempre la TC, meglio se multidetettore, tecnica che permette di valutare sia l’aspetto parenchimale, vascolare ed escretore del rene [5, 6, 11–14]. L’eco-contrastografia con MdC di II generazione deve essere considerata come metodica in grado di avviare alla valutazione TC quei pazienti sicuramente positivi per documentare l’integrità della via escretrice e del peduncolo vascolare [14–16]. L’ecografia con MdC appare metodica fondamentale nel follow-up delle lesioni riducendo così l’esposizione radiante in pazienti spesso giovani, a volte pediatrici, considerando anche che frequentemente la terapia del trauma renale è di tipo conservativo [4, 17, 18]. Si ribadisce inoltre che la metodica non può essere applicata nel soggetto politraumatizzato stabile in quanto questo tipo di paziente deve essere studiato con esame TC comprendente distretti corporei multipli [19, 20]. Per questo motivo, nella nostra casistica, non compaiono casi di lesioni renali di alto grado in quanto si tratta di situazione critiche, spesso associate a lesioni di altri organi, che necessitano di valutazione TC che permette un adeguato bilancio del trauma.
