Abstract Purpose. The authors sought to evaluate the feasibility, diagnostic accuracy and safety of urological biopsy performed using a flexible alligator forceps. Materials and methods. Twenty-seven patients with suspected urothelial malignancy underwent retrograde biopsy using a 7-F biopsy forceps (Cordis, Miami, FL, USA). Mild sedation was guaranteed by an anaesthesiologist. The final diagnosis was confirmed by cytohistological data and subsequently by pathology findings at surgery. Lesions with benign histopathology were closely monitored for at least 12 months. Results. The technical success rate was 92.6%. The high diagnostic accuracy was related to the positive correspondence between histological outcomes and surgical results or follow-up observations. As regards sample site, the procedure was less successful in calyceal lesions than in lesions located in the upper urinary tract, with a technical success of 71.43%. Twenty patients had asymptomatic haematuria in the early hours after the procedure; ten of them had mild dysuria. No one require medication, and no major complications occurred. Conclusions. Urological forceps biopsy is a safe and easy procedure. It provides a relatively high level of accuracy in the diagnosis of lesions of the upper urinary tract. Riassunto Obiettivo. Scopo del nostro lavoro è stato valutare fattibilità, accuratezza diagnostica e sicurezza della biopsia urologica eseguita con un biotomo flessibile tipo alligator. Materiali e metodi. Ventisette pazienti con il sospetto di patologia uroteliale maligna, sono stati sottoposti a biopsia per via retrograda, utilizzando il biotomo da 7 F (Cordis, Miami, FL, USA). L’anestesista ha eseguito una blanda sedazione. La diagnosi finale è stata confermata dai reperti patologici chirurgici e precedentemente dai risultati citoistologici. Le lesioni con risultato isto-patologico benigno sono state tenute sotto stretto follow-up per almeno 12 mesi. Risultati. Il successo tecnico è risultato del 92,6%. L’alta accuratezza diagnostica era dovuta alla corrispondenza positiva tra esame isto-patologico eseguito sul prelievo bioptico ed i reperti patologici chirurgici o la stazionarietà/riduzione dimensionale della alterazione durante il periodo di follow-up. Considerando la sede dei prelievi, la performance della procedura è risultata più bassa per le lesioni localizzate nei calici che non per quelle localizzate in altre sedi dell’apparato urinario superiore (AUS), con un successo tecnico, pari al 71,43%. Venti pazienti hanno presentato ematuria asintomatica nelle prime ore dopo la procedura; dieci hanno presentato lieve disuria. Nessuno ha avuto bisogno di specifica terapia. Non è stata osservata nessuna complicanza maggiore. Conclusioni. La biopsia eseguita con il biotomo è una procedura sicura e facile da eseguire. Introduction
Introduzione
Although the incidence of urothelial tumours in the upper urinary collecting system may be low in the general population (one or two cases per 100,000 people annually), it increases in patients with prior or concurrent urothelial tumours in the lower urinary tract, with a reported incidence of 3.9–4.8% [1]. Ureteroscopy is one technique most widely used to investigate upper urinary tract pathology. Several studies in the literature compare the accuracy of radiographic studies, ureteroscopy, biopsy and cytology in predicting the histopathology of upper-urinary-tract transitional cell carcinoma (TCC) [2, 3]. Ureteroscopy was most useful in patients with negative voided urine cytology and positive preoperative radiographic findings [4]. However, ureteroscopic biopsy can be associated with some complications, such as perforation, spillage of tumour cells and ureteral strictures [5, 6]. Forceps biopsy was originally designed for endomyocardial biopsies [7]. It was also used for percutaneous transluminal biliary biopsy through a transhepatic biliary drainage tract and was found to be safe and easy to perform [8]. To the best of our knowledge, this is the first study in which a forceps biopsy device was used to perform biopsies in the upper urinary tract. The pathological grade obtained from biopsy was compared to that of the surgically resected specimens, and all patients undergoing conservative treatment were closely monitored. The aim of the study was to evaluate the feasibility, sensitivity, specificity, diagnostic accuracy and safety of urological biopsy performed using a flexible alligator forceps.
Nella popolazione generale, l’incidenza dei tumori uroteliali dell’apparato urinario superiore (AUS), è bassa (uno o due casi per 100000 persone all’anno); essa è maggiore nei soggetti con precedenti tumori del tratto urinario basso, con una incidenza del 3,9%–4,8% [1]. Attualmente la tecnica più utilizzata per lo studio di questo tratto dell’apparato urinario è l’ureteroscopia. In letteratura, sono presenti alcuni lavori che paragonano l’accuratezza di indagini radiologiche, dell’ureteroscopia, della biopsia e della citologia urinaria nel predire l’istopatologia del carcinoma a cellule transizionali (TCC) dell’AUS [2, 3]. L’ureteroscopia è la metodica più utilizzata nei pazienti con citologia urinaria negativa e segni radiografici sospetti per patologia neoplastica [4]. Tuttavia, la biopsia ureteroscopica è associata ad alcune complicanze, come la perforazione, il seeding tumorale e le stenosi ureterali [5, 6]. La biopsia con il biotomo era stata inizialmente progettata per le biopsie endomiocardiche [7]. Inoltre, il biotomo è stato utilizzato per le biopsie biliari percutanee transluminali (previo posizionamento di drenaggio biliare); ed in quest’ultimo caso è risultata essere una tecnica sicura e facile da eseguire [8]. Al meglio delle nostre conoscenze, questo è il primo studio in cui il biotomo viene usato per eseguire biopsie uroteliali dell’AUS. Il grading istopatologico ottenuto dal nostro prelievo bioptico è stato confrontato con quello ottenuto sul pezzo operatorio. Inoltre, i pazienti trattati in maniera conservativa, sono stati tenuti sotto stretto follow-up. Lo scopo del nostro studio è quello di valutare la fattibilità, la sensibilità, la specificità, l’accuratezza diagnostica e la sicurezza delle biopsie uroteliali condotte con il biotomo flessibile alligator type.
Materials and methods

Patients
Pazienti
From February 2007 to September 2010, 27 patients (15 men and 12 women; mean age 71.15 years; range 43-89) with suspected calyceal (n=7), pelvis (n=5), pelvic-ureteral junction (n=4) or ureteral (n=11) lesions (four thickenings and 23 filling defects) underwent transureteral biopsy using the 50-cm, 7-F flexible biopsy forceps (Cordis, Miami, FL, USA) (Table 1). Patients with suspected multiple tumours on imaging studies and/or ureteroscopic inspection were excluded from the study. Urine cytology was obtained at least once prior to the urothelial biopsy in all patients; in particular, in seven patients, urinary cytology identified the presence of atypical cells. Suspicion of an upper urinary tract TCC was the main indication for biopsy. Da febbraio 2007 a settembre 2010, 27 pazienti (15 maschi e 12 femmine; età media 71,15 anni, range 43–89) con sospette lesioni caliceali (n=7), pelviche (n=5), della giunzione pelvi-ureterale (n=4) o ureterali (n=11) [4 ispessimenti parietali focali e 23 difetti di riempimento all’esame uro-tomografia computerizzata (TC)] sono stati sottoposti a biopsia trans-ureterale con biotomo flessibile, 7 F, lungo 50 cm, (Cordis, Miami, Florida, USA) (Tabella 1). Sono stati esclusi i soggetti con sospette multiple lesioni agli esami radiologici e/o all’ispezione ureteroscopica. Tutti i pazienti hanno eseguito l’esame citologico delle urine almeno una volta prima della biopsia uroteliale; in particolare in 7 pazienti sono state identificate la presenza di cellule atipiche. Il sospetto di TCC dell’AUS è stato considerato la principale indicazione alla biopsia. Tutti i casi sono stati discussi durante un meeting multidisciplinare tra radiologo interventista, urologo, oncologo ed anatomo-patologo che hanno concordato per

all of whom agreed to obtain a biopsy using a flexible biopsy forceps under fluoroscopic guidance. The coagulation profile was in the normal range for all patients. Patients undergoing anticoagulant and/or antiaggregating therapy interrupted treatment at least 7 days before the procedure, with the introduction of fractionated heparin when necessary. Written informed consent was obtained from all patients. Baseline imaging Pretreatment imaging consisted of a urological multidetector computed tomography (MDCT) scan (Aquilion 64, Toshiba, Tokyo, Japan) without and after contrast medium administration i.v.. Each MDCT scan was acquired with a thickness of 0.5 mm, voltage of 120 kV and tube current of 250 mA. The contrast-enhanced scans were acquired after injection of 100 ml of iodinated contrast agent (Visipaque 320, GE Healthcare, Princeton, NJ, USA) at an injection rate of 3 ml/s, followed by injection of 40 ml saline at a rate of 2 ml/s. After baseline acquisition, the contrast medium was administered and a venous (60 s) and a late (excretory phase, after 10–20 m) scan were obtained. CT findings were classified as thickness and filling defects on the basis of longitudinal and transverse diameters. Pretreatment procedure All patients arrived in the angiography suite with a ureteral catheter previously placed by the urologist during ureteroscopy. Each patient was kept in a state of moderate sedation through i.v. administration of a combination of midazolam (Ipnovel 15, Roche, Milan, Italy) (0.07–0.08 mg/kg), propofol (Diprivan, AstraZeneca S.p.A., Caponago, Italy) (0.5–2.0 mg/kg/h) and fentanyl (Fentanest; Pharmacia & Upjohn, Milan, Italy) (1–2 μg/kg). Heart rate, electrocardiographic trace, oxygen saturation, respiratory frequency and blood pressure were continuously monitored throughout the procedure. During the entire biopsy session, patients were monitored by an anaesthesiologist. Adequate antibiotic prophylaxis was achieved with administration of 600 mg of prulifloxacin (Unidrox, Angelini, A.C.R.A.F. S.p.A, Rome, Italy) given every 8 h for 24 h, beginning shortly before the procedure. Biopsy equipment and procedure All procedures were performed with fluoroscopic guidance (Philips Allura Xper FD20 (Philips Medical System, Best, The Netherlands), available in our institute. The angiography detector system uses an indirect method of X-ray conversion through caesium iodide (CsI). The detector has a field of view of 38×30 cm and an array of 2,480×1920

l’esecuzione di una biopsia uroteliale con biotomo flessibile sotto guida fluoroscopica. La crasi ematica è risultata essere nel range di normalità in tutti i pazienti. I pazienti in terapia anticoagulante e/o antiaggregante hanno sospeso il trattamento almeno 7 giorni prima della procedura, introducendo eparina frazionata se necessario. Un consenso informato scritto è stato ottenuto per tutti i pazienti. Imaging pre-trattamento L’imaging pre-trattamento è stato ottenuto mediante indagine con TC multistrato addominale (Aquilion 64, Toshiba, Tokio, Giappone), con e senza iniezione di mezzo di contrasto (MdC). Ciascuna scansione TC è stata acquisita ad uno spessore di 0,5 mm, a 120 kV e 250 mA. Le scansioni con MdC sono state eseguite iniettando 100 ml di contrasto iodato (Visipaque 320, GE Healthcare, Princeton, NJ, USA) ad una velocità di 3 ml/s seguito dall’iniezione di 40 ml di soluzione salina ad una velocità di 2 ml/s. È stata eseguita un’acquisizione basale e, dopo la somministrazione intravenosa di MdC, è stata ottenuta un’acquisizione venosa (60 secondi) e una tardiva (fase escretoria dopo 10–20 minuti). I reperti TC sono stati classificati come ispessimenti parietali focali e difetti di riempimento sulla base dei diametri longitudinale e trasverso. Procedure pre-trattamento Tutti i soggetti sono giunti in sala angiografia con un catetere ureterale, precedentemente posizionato dagli urologi durante l’esame ureteroscopico. Ciascun paziente è stato mantenuto in regime di sedazione moderata, ottenuta mediante la somministrazione per via endovenosa di una combinazione di midazolam (Ipnovel 15, Roche, Milano, Italia; 0,07–0,08 mg/kg), propofol (Diprivan, AstraZeneca S.p.A., Caponago, Italia; 0,5–2 mg/kg/h) e fentanil (Fentanest, Pharmacia & Upjohn, Milano, Italia; 1–2 μg/kg). Durante il trattamento sono stati monitorati di continuo la frequenza cardiaca, il tracciato elettrocardiografico, la saturazione dell’ossigeno, la frequenza respiratoria, la pressione sanguigna. I pazienti, per tutta la durata della procedura, sono stati sorvegliati da un medico anestesista. Un’adeguata profilassi antibiotica è stata ottenuta mediante la somministrazione endovena di 600 mg di prulifloxacina 600 mg (Unidrox, Angelini, A.C.R.A.F. S.p.A, Roma, Italia) somministrata ogni 8 ore per 24 ore, iniziando poco prima della procedura. Descrizione della procedura Tutte le procedure sono state eseguite con guida fluoroscopica (Philips Allura Xper FD20, Philips Medical System, Best, Paesi Bassi). Il detettore dell’angiografo utilizza un metodo indiretto di conversione dei raggi X attraverso lo ioduro di cesio (CsI). Il detettore ha un campo di vista di 38×30 cm ed una matrice di 2480×1920 pixels con un pitch di 154 microns per pixel. Tutte le biopsie sono state eseguite utilizzando un

pixels, with a pitch of 154 μm/ pixel. All the biopsies were performed using the 50-cm, 7-F flexible biopsy forceps (Cordis). Through the ureteral catheter, a 0.035-in. J-tip hydrophilic guidewire (Glidewire, Terumo Medical Corporation, Somerset, NJ, USA) was introduced in the ureteral lumen. The catheter was then replaced first by a 4- or 5-F vascular catheter (Cordis Europe, N.V., LJ Roden, The Netherlands) to perform ascending pyelography using radiopaque contrast medium (Visipaque 320, GE Healthcare) and subsequently by a 7-F vascular introducer (Cordis Europe), with an appropriate length to reach the site of the biopsy, shown as a filling defect during pyelography. A 50-cm, 7-F flexible biopsy forceps (Cordis) was introduced through the vascular introducer (Fig. 1a,b). Because of its flexibility and adjustability, it allowed for several histological samples to be obtained (typically three to five passes), depending on the subjective quality of the material obtained as assessed by visual inspection.

Attraverso l’introduttore vascolare, è stato introdotto il biotomo flessibile 7 F, lungo 50 cm (Cordis, Miami, Florida, USA) (Fig. 1). Grazie alla sua flessibilità, il biotomo consente di eseguire più prelievi istologici, in relazione al giudizio qualitativo visivo del materiale prelevato da parte dell’operatore. Generalmente sono stati eseguiti da 3 a 5 prelievi.
Outcomes
Il successo tecnico della procedura è definito come il corretto posizionamento del device nella zona della lesione. Le lesioni sono state classificate sulla base dei reperti istopatologici in due gruppi: gruppo 1, costituito dalle lesioni in cui il materiale bioptico è risultato sufficiente per esami istopatologici e quindi per giungere ad una diagnosi definitiva di lesione benigna o maligna (per esempio TCC, tessuto normale uroteliale, etc.); gruppo 2, costituito dalle lesioni in cui il materiale prelevato non è stato sufficiente per giungere ad una diagnosi definitiva. Technical success was defined as correct positioning of the biopsy device within the lesion. Lesions were classified on the basis of histopathological reports: group 1, indicating an adequate sample for histopathological analysis to yield a definitive diagnosis of a benign or malignant lesion (e.g., TCC, normal ureteral tissue, etc); group 2, indicating that the sample was insufficient to yield a definitive diagnosis. Biopsy results from group 1 were compared with the pathological diagnosis on the surgical specimens or with

the imaging findings during close follow-up (of at least 12 months). Lesions were defined as benign when the histological report showed a definitive diagnosis of benignity or when the lesion exhibited a decrease in size or no morphological change during imaging follow-up. Filling defects unreachable by forceps were considered as possible malignancies and were consequently subjected to a close followup (of at least 12 months), but they were not considered for our analysis. On the basis of this categorisation, sensitivity, specificity and diagnostic accuracy of the procedure were evaluated. Complications were classified as major or minor in accordance with the classification system of the Society of Interventional Radiology [9].

Una lesione è stata definita come benigna quando l’esame istologico ha riportato una diagnosi definitiva di benignità o quando, nel corso del follow-up, è risultata dimensionalmente ridotta o invariata. I difetti di riempimento non raggiungibili dal biotomo sono stati considerati come possibili maligni e di conseguenza sottoposti ad un periodo di stretto follow-up (almeno 12 mesi), ma non sono stati considerati per la nostra analisi. Sulla base di queste considerazioni, sono state valutate la sensibilità, la specificità e l’accuratezza diagnostica della procedura. Le complicanze sono state classificate in maggiori e minori, in accordo con il sistema di classificazione della Società Internazionale di Radiologia Interventistica [9].
Results Risultati Technical success was obtained in 92.6% (25/27) of patients. In two (7.4%) patients, the biopsy could not be performed because pyelography did not detect the precise site of the filling defect shown at CT urography. In both patients, the filling defects had a transverse diameter <1 cm and were considered as possible malignancies and underwent close follow-up. Histological outcomes of the biopsies are summarised in Table 1. All lesions sampled (25/27, 92.6%) had either a surgical histological correspondence (n=21), or the close follow-up demonstrated stability or reduction in size (n=4). All urothelial biopsies performed with the forceps had a positive correspondence with histological examination or with the follow-up. With regard to biopsy site, the procedure proved to be less precise in the calyceal lesions than in the remaining sites, with a value of technical success of 71.43%. In all cases in which the biopsy was performed, the tissue sampled was sufficient for histological examination. There were no major complications related to the biopsy procedures; some patients (n=20) had asymptomatic haematuria in the early hours (up to 24 h) after the procedure, and others (n=10) had mild dysuria. No patient required any medication.
Il successo tecnico è stato ottenuto nel 92,6% dei pazienti (25 dei 27 pazienti). In 2 pazienti (7,4%) non è stato possibile eseguire la biopsia perché la pielografia non ha consentito di identificare la precisa sede del difetto di riempimento documentato all’esame uro-TC; in entrambi i pazienti i difetti di riempimento avevano un diametro trasverso inferiore ad 1 cm e sono stati considerati come possibili lesioni maligne e pertanto sottoposti a stretto follow-up. I risultati istologici delle biopsie sono riassunti in Tabella 1. Tutte le lesioni sottoposte a biopsia (25/27, 92,6%) hanno avuto una corrispondenza istologica chirurgica (n=21) oppure il follow-up ha mostrato stabilità o riduzione delle dimensioni dell’alterazione (n=4). La biopsia uroteliale eseguita con il biotomo ha trovato in ogni caso una concordanza chirurgica o durante il follow-up. Considerando la sede del prelievo dei campioni, la procedura è risultata essere meno precisa nelle lesioni caliceali, con un valore di successo tecnico pari al 71,43%. Nei casi in cui è stato possibile eseguire la biopsia, il materiale prelevato è sempre risultato sufficiente per l’esame istopatologico. Non si è verificata nessuna complicanza maggiore. Alcuni pazienti (n=20) hanno presentato ematuria asintomatica nelle prime ore successive alla procedura (entro 24 ore) e lieve disuria (n=10); nessun paziente ha necessitato di terapie specifiche.
Discussion CT urography is effective in depicting upper urinary tract tumours in patients undergoing surveillance because of a history of urothelial tumour. Endoluminal filling defects have a positive predictive value (PPV) of 50% in the pelvicalyceal system and 87.5% in the ureter. Moreover, urothelial thickening is an important imaging sign of urothelial tumour, especially in the pelvicalyceal system, with a PPV of 87.5% vs. 64% along the ureter [10]. Therefore, histological confirmation is often required for correct diagnosis,

treatment planning and prognosis. Voided urine cytology adds a significant cost without any diagnostic benefit in the workup of low-risk patients with asymptomatic microscopic haematuria [11] and is used only as a predictive factor in association with imaging findings [4]. Exfoliated cell cytology in combination with biopsy grade is recommended as part of the evaluation of upper urinary tract TCC selected for endoscopic management [12]. Endoscopic brush cytology is a specific method for detecting TCC of the upper urinary tract but does not appear to be successful in diagnosing dysplasia or in carcinoma in situ (CIS). As with urinary cytology in general, the technique is less effective in diagnosing low-grade (I and II) lesions [13]. Ureteroscopy and ureteroscopic biopsy is the gold standard for evaluating upper urinary tract lesions [14]. The main risks of ureterorenoscopic examination are considered to be dissemination of malignant cells and ureteral injury. Ureterorenoscopic dissemination of malignant cells is a controversial issue. The few reports describing this complication suggest it is associated with high irrigating
un PPV del 64% lungo l’uretere [10]. Di conseguenza è necessaria una conferma istologica per definire una corretta diagnosi, impostare un eventuale iter terapeutico e stabilire una prognosi. Nei pazienti a basso rischio con ematuria microscopica asintomatica, la citologia urinaria rappresenta un costo aggiuntivo, senza aggiungere alcun beneficio diagnostico [11]; oggi è usata solo come fattore predittivo, in associazione con i rilievi dell’imaging [4]. La citologia urinaria in associazione con il grading istopatologico, è raccomandata nella valutazione del carcinoma a cellule transizionali dell’AUS in pazienti selezionati per la procedura endoscopica [12]. La citologia endoscopica tramite brushing è specifica per l’identificazione dei TCC dell’AUS. Il brushing sembra non essere efficace nella diagnosi di displasia o in quella di carcinoma in situ (CIS). Come per la citologia urinaria in generale, esso risulta inoltre meno efficace nella diagnosi delle lesioni a basso grado (I e II) [13]. Attualmente l’ureteroscopia e la biopsia ureteroscopica rappresentano il gold standard per la identificazione delle lesioni dell’AUS [14]. pressure during biopsy [15]. However, high irrigating pressure is necessary during treatment and biopsy in order to achieve a clear view. No guidelines have been published concerning ureteral examination in upper urinary tract tumours, and the accuracy of ureterorenoscopic examination appears to depend on patient selection. Matsumoto et al. [4] indicated that ureterorenoscopy is the most appropriate examination and provides superior accuracy in patients who have positive radiographic findings and negative voiding cytology. This biopsy forceps was originally designed for endomyocardial biopsies [7, 16, 17] and consists of three parts: a three-pull ring handle, cutting jaws and a coiled shaft. The literature contains several reports on the use of the biopsy forceps for histological sampling of the biliary tree [18], and Jung et al. [8] considered percutaneous transluminal forceps biopsy through a transhepatic biliary drainage tract safe and easy to perform. Additionally, the procedure provides relatively high accuracy in the diagnosis of malignant biliary obstructions. To the best of our knowledge,
l’ureteroscopia è un argomento controverso. I pochi studi che hanno descritto questa complicanza, l’hanno associata all’alta pressione di lavaggio durante la procedura [15]. Tuttavia, l’alta pressione è necessaria durante la procedura al fine di ottenere una migliore visione delle vie urinarie. Non ci sono linee guida riguardanti l’esame ureterale nella patologia tumorale dell’AUS e l’accuratezza dell’esame ureteroscopico sembra dipendere dalla selezione dei pazienti. Diversi studi in letteratura descrivono l’uso del biotomo per la biopsia dell’albero biliare [18]; Jung et al. [8] hanno definito la biopsia percutanea transluminale con biotomo attraverso un drenaggio biliare transepatico una metodica sicura e semplice. ours is the first series in which the forceps biopsy device has been used in the upper urinary tract. The procedure can be considered safe and easy to perform. The biopsy method proposed offers two indisputable advantages: the possibility of obtaining histological samples while visualising the precise site of the lesion, and the opportunity to obtain tissue cores and consequently a histopathological grading. Correct and diagnostic samples were obtained from the filling defects and urothelial thickenings documented on CT urography, especially if they were located in the renal pelvis, pelvic-ureteral junction and along the ureter. Considering the site of the biopsies, the procedure proved to be less precise in the calyceal lesions than in other sites, with a technical success of 71.43%. The main limitation of our study is the small number of patients; further research with a large patient population and longer follow-up periods will be needed to precisely evaluate diagnostic accuracy and establish a possible correlation between accuracy and lesion location. Another is the lack of comparison with other biopsy methods. Further comparative studies with a prospective randomised design might establish the best biopsy system for urothelial biopsies. In conclusion, our preliminary results are encouraging, and the forceps device can be proposed as an innovative approach for urothelial biopsy.
la serie di pazienti da noi presentata è la prima in cui il biotomo è utilizzato per prelievi bioptici uroteliali dell’AUS. La procedura può essere considerata sicura e facile da eseguire. Il metodo bioptico presentato presenta due indiscutibili vantaggi: la possibilità di eseguire prelievi istologici vedendo l’esatta sede della alterazione e l’opportunità di ottenere frustoli di tessuto e di conseguenza un grading istopatologico. Un campionamento bioptico corretto e diagnostico è stato eseguito nei difetti di riempimento e negli ispessimenti uroteliali focali documentati all’esame uro-TC, soprattutto se questi erano localizzati nella pelvi renale, a livello della giunzione pelvi-ureterale e lungo l’uretere. Prendendo in considerazione la sede del prelievo, la procedura è risultata essere meno precisa nelle lesioni dei calici renali rispetto a quelle delle altre sedi, con un successo tecnico del 71,43%. Il principale limite dello studio presentato è il basso numero dei pazienti; pertanto, ulteriori studi con un maggior numero di pazienti e periodi di follow up più lunghi saranno necessari per una esatta valutazione dell’accuratezza diagnostica e per stabilire una correlazione tra questa e la localizzazione della lesione. Un altro limite è rappresento dal mancato confronto con altri metodi bioptici. Ulteriori studi comparativi prospettici randomizzati potrebbero essere in grado di stabilire definitivamente il sistema migliore per le biopsie uroteliali. Tuttavia, in conclusione, i risultati preliminari descritti sono incoraggianti ed il biotomo flessibile può essere considerato un approccio innovativo per le biopsie uroteliali.
