Einleitung Allgemeines Wohlbefinden, k6rperliche und geistige Leistungsf~higk ei t , S t o f f w e c h s e l f u n k t i o n e n , Immunkompetenz, H~matopoese und W u n d h e i l u n g s i n d y o n e i n e r a d ~ q u a t e n E r n ~ h r u n g a b h ~ n g i g (10, 20, 22, 23, 37, 49, 63). N i c h t n u r s c h w e r e , s o n d e r n b e r e i t s l a t e n t e F o r m e n d e r M a n gelern~hrung ffihren zu einer Beeintrfichtigung von Organfunktionen und s t e l l e n d a m i t b e i b e s o n d e r e n B e l a s t u n g e n d e s O r g a n i s m u s , z. B. b e i a k u ten Erkrankungen oder Operationen, eine ernste Gef~hrdung des Patient e n dar. I n z w i s c h e n k o n n t e d u r c h z a h l r e i c h e U n t e r s u c h u n g e n g e z e i g t werden, dab viele Patienten durch konsequente Ern~hrungsbehandlung i n d i e L a g e v e r s e t z t w e r d e n , i h r e K r a n k h e i t l e i c h t e r z u f l b e r w i n d e n (9, 21, 24, 55, 61, 69). D e s h a l b k o m m t d e r D i a g n o s t i k d e s E r n ~ h r u n g s z u s t a n d e s , i n s b e s o n d e r e b e i R i s i k o p a t i e n t e n , g r 5 B e r e B e d e u t u n g z u al s b i s h e r *) Die Arbeit enth~ilt die Ergebnisse aus den U n t e r s u c h u n g e n ffir die Dissertation yon Herrn J. angenommen. Angeregt durch die Befunde von Bistrian und Blackburn (6, 8, 9), w o n a c h 50 % a l l e r P a t i e n t e n i n d e n c h i r u r g i s c h e n u n d m e d i z i n i schen Kliniken in den USA mangelern~hrt zur Aufnahme kommen, e n t w i c k e l t e n w i r i n F o r t f ( i h r u n g f r ( l h e r e r A r b e i t e n m i t F e k l (27, 36) e i n Untersuchungsprogramm mit anthropometrischen und laborchemischen Parametern, die eine Beurteilung der Fettdepots, der Muskelmasse und d e r F u n k t i o n s p r o t e i n e e r l a u b e n (Tab. 3). W e g e n d e r e n g e n B e z i e h u n g e n z w i s c h e n E i w e i f t - E r n ~ a r u n g s z u s t a n d u n d zellul~irer I m m u n a b w e h r (7, 17, 56, 57, 66, 72, 81, 86) h a b e n w i r i n d i e s e s P r o g r a m m e i n e o r i e n t i e r e n d e Untersuchung der Immunkompetenz mitaufgenommen. Nachdem wir m i t d i e s e m S c h e m a m e h r als 500 i n t e r n i s t i s c h e P a t i e n t e n i m R a h m e n d e r A u f n a h m e u n t e r s u c h u n g f i b e r p r f i f t h a b e n (36, 37), soll i n d e r v o r l i e g e n d e n Arbeit die Frage d i s k u t i e r t werden, in w e l c h e m U m f a n g die e i n z e l n e n Parameter unseres Untersuchungsbogens zur Verlaufsbeobachtung bei A n d e r u n g e n d e s E r n ~ h r u n g s z u s t a n d e s , also i n a n a b o l e n o d e r k a t a b o l e n S i t u a t i o n e n , g e e i g n e t s i n d . D a z u w e r d e n d i e D a t e n y o n 15 P a t i e n t e n m i t chronischem Alkoholabusus herangezogen, bei denen durch nutritive und sonstige Therapie innerhalb yon 3 Wochen eine Umkehr yon Katabolie in A n a b o l i e z u e r w a r t e n war. I m V e r g l e i c h d a m i t w e r d e n d i e V e r l a u f s p r o t o k o l l e y o n 15 k a t a b o l e n M a l i g n o m p a t i e n t e n m i t i n f a u s t e r P r o g n o s e diskutiert.
Patientengut und Methoden Bei 15 katabolen Malignompatienten mit infauster Prognose (Gruppe A, Tab. 1) u n d bei 15 Patienten mit chronischem Alkoholabusus, die wegen eines toxischnutritiven Leberparenchymschadens, einer akuten oder chronischen Pankreatitis, eines Ulkusleidens oder einer P n e u m o n i e stationer a u f g e n o m m e n werden muBten (Gruppe B, Tab. 2), wurde am 1., 10. u n d 20. Behandlungstag der Ern~hrungszustand naeh dem U n t e r s u c h u n g s s c h e m a der Tabelle 3 erhoben. Die Patienten der Gruppe B erhielten zus~tzlich zur fiblichen Krankenhauskost 1000 ml einer industriell gefertigten Formuladi~t (Biosorb-Drink| mit 1000 Kalorien u n d 40g biologisch hochwertigem EiweiB. Bei den Patienten der Gruppe A war wegen ihrer schweren G r u n d k r a n k h e i t u n d der infausten Prognose eine Ern~hrungsbehandlung nicht angezeigt. Zur Erfassung des Ern~ihrungszustandes verwandten wir anthropometrische laborchemische u n d immunologische Methoden (Tab. 3). Die anthropometrischen Messungen umfaftten n e b e n dem KSrpergewicht u n d der KSrpergrSBe die Bestimm u n g der Trizepshautfalte u n d des Oberarmumfangs. Das KSrpergewicht (KG) wurde in Prozent des optimalen KSrpergewichts (OKG) umgerechnet (73). O K G ffir M~nner = Broca-Gewicht - 0,2 • (Broca-Gewicht - 52), OKG ffir F r a u e n = Broca-Gewicht - 0,4 x (Broca-Gewicht - 52). Eine Erfassung des K6rperbautypus durch Messung des Brust- bzw. Handgelenkumfanges hielten wir im R a h m e n dieser Studie fiir nicht erforderlich. Ffir die B e s t i m m u n g der Trizepshautfalte (THF) als Mal~ des s u b k u t a n e n Fettgewebes benutzten wir einen Caliper nach Holtainl), der bei einer Kontaktfl~chengrSBe von 70 m m 2 mit einem k o n s t a n t e n Druck ,con 10 g/mm 2 miBt. Unterschiede in der MeBgenauigkeit zu den anderen gebr~iuchlichsten Calipern nach Harpenden, Lange oder Best bestehen nicht (75). D i e K r e a t i n i n a u s s c h e i d u n g i m 2 4 - S t u n d e n - U r i n (U-Krea) gilt als b r a u c h b a r e r P a r a m e t e r z u r S c h f i t z u n g d e r M u s k e l m a s s e bzw. d e r f e t t f r e i e n K 6 r p e r m a s s e (28). Sie w u r d e n v o n u n s i n zwei 1 2 s t f i n d i g e n S a m m e l p e r i o d e n f i b e r zwei a u f e i n a n d e r f o l g e n d e Nfichte (20-8 U h r ) g e m e s s e n . D a s E r g e b n i s w u r d e i n P r o z e n t d e s S t a n d a r d s als K r e a t i n i n i n d e x (KI) a u s g e d r f i c k t (4, 37). Aul3erdem bestimmten wir folgende Plasmaproteine zur Beurteilung des Eiwei~ernfihrungszustandes: Albumin (kolorimetrisch mit Bromkresolgrfin), P r i i a l b u m i n u n d T r a n s f e r r i n ( r a d i a l e I m m u n d i f f u s i o n (64) m i t P a r t i g e n - P l a t t e n ) u n d C h o l i n e s t e r a s e ( F a r b t e s t B o e h r i n g a r - M a n n h e i m , Nr. 124133). D i e E r f a s s u n g des immunologischen Reaktionsvermtigens erfolgte fiber die absolute Lymphozytenzahl im peripheren Blut und Intrakutantestungen mit Streptokinase-dornase ( V a r i d a s e | Lederle), M u m p s - H a u t t e s t a n t i g e n (Eli Lilly) u n d C a n d i d a v a k z i n e ( B e n card) (2, 90). I n d e n z i t i e r t e n P u b l i k a t i o n e n f i n d e n s i c h a u s f f i h r l i c h e A n g a b e n f i b e r die h i e r v e r w e n d e t e n M e t h o d e n und die Kategorisierung der Mangelsyndrome. Zur statistischen Analyse berechneten wir neben Mittelwert, Standardabw e i c h u n g u n d K o n f i d e n z b e r e i c h d e n V a r i a t i o n s k o e f f i z i e n t e n als V e r h i i l t n i s y o n Standardabweichung zu Mittelwert. Je kleiner der Variationskoeffizient, desto prfiziser ist e i n e K e n n g r 6 B e ; b e i e i n e m W e f t fiber 1 ist sie zu u n g e n a u . Wir z e i c h n e t e n K u r v e n d e r P r o t e i n w e r t v e r l f i u f e u n d b e r e c h n e t e n als R e a k t i o n s f l ~ i c h e die Fl~che zwischen der Kurve und dem Niveau des Ausgangswertes. Bei den a n t h r o p o m e t r i s c h e n K e n n g r 6 / 3 e n u n d d e r K r e a t i n i n a u s s c h e i d u n g z o g e n w i r die Differenzen zwischen erstem und drittem MeBwert heran. Die Mittelwerte, Stand a r d a b w e i c h u n g e n , K o n f i d e n z b e r e i c h e u n d V a r i a t i o n s k o e f f i z i e n t e n d i e s e r Diff e r e n z e n u n d R e a k t i o n s f l f i c h e n g i n g e n e b e n f a l l s i n die B e u r t e i l u n g m i t ein.
Ergebnisse Bei den Patienten tier Gruppe A verschlechterte sich der Ernfihrungszustand wfihrend der dreiw6chigen Beobachtung infolge der durch das G r u n d l e i d e n b e d i n g t e n K a t a b o l i e e r h e b l i c h ( T a b . 4). trischen KenngrSBen stellten sich der AU und die AMF als besonders a u s s a g e k r ~ f t i g ffir d i e V e r l a u f s b e o b a c h t u n g heraus. Das K6rpergewicht w a r b e i 7 y o n 15 P a t i e n t e n w e g e n v o r b e s t e h e n d e r oder sich entwickelnder Odemkrankheiten nicht verwertbar. THF und FF waren in ihrer Aussagek r a f t d e m A U u n d A M F u n t e r l e g e n ( A b b . 1). U-Krea und der KI zeigten bei allen Patienten die Verminderung der Muskelmasse bzw. der fettfreien KSrpermasse d e u t l i c h a n ( A b b . 2). A b b . 3. T a g d e u t l i c h e r als i n d e r e r s t e n P h a s e , w a s m S g licherweise mit der Ausdehnung von Lebermetastasen bei einigen Patient e n z u s a m m e n h ~ n g t ( A b b . 3). Infolge der Katabolie nahm das immunologische ReaktionsvermSgen b e i a l l e n P a t i e n t e n d e r O r u p p e A d e u t l i c h a b (Tab. 5 u n d A b b . 4). Bei den Patienten der Gruppe B wurde durch die dreiw6chige ErnShrungs-Rehabilitation ein ausgepr~gter anaboler Effekt mit Anstieg der m]thropometrischen Meltwerte, der Funktionsproteine und Verbesserung d e r I m m u n k o m p e t e n z b e o b a c h t e t (Tab. 6). Bei den anthropometrischen Mal3en war die Zunahme am deutlichsten beim KG und beim AU. Weniger deutlich und einheitlich waren die Verf_nderungen bei der THF. Keine wesentliche zus~tzliche Information ergab die Berechnung von FF und AMF. Bei Differenzierung der Patienten der Gruppe B nach Protein-Kalorien-Mangelern~hrung (PKM) und M i s c h b i l d e r n (MB) e r g a b e n s i c h k e i n e U n t e r s c h i e d e b e i d e r Z u n a h m e y o n K S r p e r g e w i c h t , F e t t d e p o t s u n d M u s k e l m a s s e ( A b b . 5). U-Krea bzw. KI zeigten bei allen Patienten der Gruppe B einen starken Anstieg und spiegelten damit die Anabolie besonders deutlich wider ( A b b . 6).

Bei den F u n k t i o n s p r o t e i n e n war bei allen Patienten eine positive Entw i c k l u n g festzustellen. Dabei war die Z u n a h m e bei den P a t i e n t e n mit M i s c h b i l d e r n (MB) d e u t l i c h e r als b e i d e n e n m i t P r o t e i n - K a l o r i e n - M a n g e l e r n f i h r u n g (PKM). O f f e n b a r r e a g i e r e n P a t i e n t e n i m Z u s t a n d d e s EiweiBm a n g e l s a u f e i n e P r o t e i n z u f u h r r a s c h e r als s o l c h e m i t k o m p e n s i e r t e m EiweiBhaushalt. Besonders einheitlich war das Verhalten von A l b u m i n u n d Pr~ialbumin, w~ihrend T r a n s f e r r i n u n d C h o l i n e s t e r a s e e i n e grSBere S t r e u u n g z e i g t e n (Abb. 7). Bei allen P a t i e n t e n der G r u p p e B verbesserte sich das i m m u n o l o g i s c h e ReaktionsvermSgen, wie die Z u n a h m e positiver H a u t r e a k t i o n e n eindeutig b e w e i s t . D a g e g e n w a r d a s V e r h a l t e n d e r L y m p h o z y t e n z a h l e n d i a g n o s t i s c h n i c h t v e r w e r t b a r (Tab. 7 + A b b . 8). Diskussion U b e r d i e n e g a t i v e n A u s w i r k u n g e n i n s u f f i z i e n t e r E r n ~ h r u n g liegt e i n e u m f a n g r e i c h e L i t e r a t u r v o r (7, 10, 17, 23, 38). Es b e s t e h t h e u t e k e i n Zweifel, d a b b e r e i t s e i n l a t e n t e r E i w e i B m a n g e l e i n e n s c h w e r w i e g e n d e n R i s i k o f a k t o r d a r s t e l l t . Es soll in d i e s e r A r b e i t n i c h t d e t a i l l i e r t d a r a u f e i n g e g a n g e n w e r d e n , d a b d i e S y m p t o m e , d i e e i n e M a n g e l e r n ~ h r u n g - also eine e x o g e n e Ursache - signalisieren, sich auch auf G r u n d e n d o g e n e r S t S r u n g e n a u s b i l d e n k S n n e n . Ffir d a s k l i n i s c h e B i l d d i e s e s S y m p t o m e n k o m p l e x e s w u r d e bereits der Begriff ,,Malnutritions-Syndrom" postuliert (79). M a n g e l e r n ~ i h r u n g ffihrt zu e i n e r B e e i n t r ~ c h t i g u n g v o n O r g a n f u n k t i o n e n u n d d a m i t zu e r h S h t e r M o r b i d i t ~ t u n d M o r t a l i t ~ t (16, 25, 41, 52, 65, 67, 69). B e s o n d e r s i n t e n s i v w u r d e n d i e n e g a t i v e n A u s w i r k u n g e n e i n e s P r o t e i n d e f i z i t s a u f d i e zellul~re I m m u n a b w e h r , d i e W u n d h e i l u n g u n d d i e Reif~festigkeit v o n A n a s t o m o s e n u n t e r s u c h t (7, 10, 17, 22, 38, 49, 63, 66, 81, 86). E r h e b u n g e n v e r s c h i e d e n e r A r b e i t s g r u p p e n h a b e n gezeigt, d a b 30-50 % aller K l i n i k p a t i e n t e n Anzeichen eines M a l n u t r i t i o n s - S y n d r o m s aufweisen (6, 8, 15, 36, 37, 60, 70, 97) u n d d a b sich d e r E r n ~ h r u n g s z u s t a n d w~ihrend d e s station~iren A u f e n t h a l t e s b e i e i n e m Gro/3teil d e r P a t i e n t e n i n f o l g e v e r o r d n e t e r N a h r u n g s k a r e n z z u r D u r c h f f i h r u n g d i a g n o s t i s c h e r Ma/3nahm e n w e i t e r v e r s c h l e c h t e r t e (92). B e i s p i e l s w e i s e s t e l l t e n Z u m t o b e l et al. (99) b e i i h r e n P a t i e n t e n m i t Intestinaltumoren negative Stickstoffbilanzen bis -5 g infolge untersuc h u n g s b e d i n g t e r N f i c h t e r n p e r i o d e n lest. B e s o n d e r s g e f ~ h r d e t ist d e r m u l t i m o r b i d e geriatrische Patient, bei d e m die P r o t e i n s y n t h e s e aus altersphy-

siologischen Grfinden v e r m i n d e r t ist u n d bei e i n e m E i n t r e t e n in ein Defizit n i c h t a u s r e i c h e n d an d e n B e d a r f adaptiert w e r d e n k a n n (37). Aus all d e m g e h t hervor, wie b e r e c h t i g t es war, als 1978 einer der A u t o r e n die F o r d e r u n g erhob, d e n S t o f f w e c h s e l n e b e n A t m u n g u n d Herz-KreislaufF u n k t i o n d e n V i t a l f u n k t i o n e n z u z u o r d n e n (78). Hieraus resultiert die Ford e r u n g n a c h einer E r n ~ h r u n g s b e h a n d l u n g im R a h m e n der Basistherapie, die den mangelern~ihrten P a t i e n t e n in die L a g e versetzt, seine K r a n k h e i t zu fiberwinden. Mullen u n d Mitarbeiter (69) h a b e n kiirzlich berichtet, d a b sie d u r e h a d e q u a t e p r e o p e r a t i v e E r n ~ h r u n g s b e h a n d l u n g bei i h r e n s c h w e r s t k r a n k e n I n t e n s i v t h e r a p i e p a t i e n t e n die p o s t o p e r a t i v e K o m p l i k a tionsrate u m das 2,5fache, die p o s t o p e r a t i v e Sepsisrate u m das 6fache u n d die Mortalit~tsrate u m das 5fache s e n k e n k o n n t e n . Bei der A u s w a h l der e r n ~ h r u n g s b e d f l r f t i g e n P a t i e n t e n w i e s e n die A u t o r e n d e n P l a s m a p r o t e inen A l b u m i n u n d T r a n s f e r r i n u n d der zellul~ren I m m u n r e a k t i v i t ~ t , g e m e s s e n mit Hilfe v o n I n t r a k u t a n t e s t u n g e n , die gr5f~te klinische Relevanz zu. U b e r die A u s s a g e k r a f t v e r s c h i e d e n e r a n t h r o p o m e t r i s c h e r , biochemis c h e r u n d i m m u n o l o g i s c h e r K e n n g r 6 B e n ftir die Definition des Ernfi_hr u n g s z u s t a n d e s b e s t e h t derzeit n o c h keine einheitliche Meinung. U n s e r e r A n s i c h t n a c h ist die A u s w a h l d e r P a r a m e t e r n i c h t n u r a b h ~ n g i g v o m P a t i e n t e n g u t , s o n d e r n a u c h davon, ob es sich u m die prim~re E r h e b u n g des E r n ~ h r u n g s z u s t a n d e s o d e r u m die V e r l a u f s b e o b a c h t u n g handelt. I m f o l g e n d e n wollen wir die Wertigkeit der einzelnen K e n n g r 6 B e n in vers c h i e d e n e n S i t u a t i o n e n a n h a n d tier v o r l i e g e n d e n L i t e r a t u r u n d u n s e r e r eigenen E r h e b u n g e n diskutieren. Bei der E r h e b u n g des E r n ~ h r u n g s z u s t a n d e s k 6 n n e n a n a m n e s t i s c h e A n g a b e n wichtige Hinweise geben. Dabei wird der kurzfristig eingetreteh e n G e w i c h t s v e r ~ n d e r u n g b e s o n d e r e B e d e u t u n g b e i g e m e s s e n . Dartiber h i n a u s g e h e n A n g a b e n fiber gastrointestinale o d e r S t o f f w e c h s e l e r k r a n k u n g e n , a b n o r m e Verluste d u r c h E r b r e c h e n , DiarrhSen, S o n d e n usw., v o r a u s g e g a n g e n e grS/3ere operative Eingriffe, o n k o l o g i s c h e B e h a n d l u n g , D a u e r m e d i k a t i o n (z. B. Cortison) in die B e w e r t u n g ein. Bei e i n e m chiru r g i s c h e n Eingriff sind die v o r a u s s i c h t l i c h e N a h r u n g s k a r e n z u n d die infolge der Katabolie im Verlauf eines P o s t a g g r e s s i o n s s t o f f w e c h s e l s zu e r w a r t e n d e n Stickstoffverluste zu beachten. Bei d e n a n t h r o p o m e t r i s c h e n Mef3grSBen h a b e n das K 6 r p e r g e w i c h t (KG) in K o r r e l a t i o n zur KSrpergr6Be, der O b e r a r m u m f a n g (AU), die Trizepshautfalte (THF) u n d der A r m m u s k e l u m f a n g (AMU) die gr61Ste diagnostische B e d e u t u n g (12, 31, 40, 50, 51, 75, 89, 96). Die ersten u n d e i n f a c h s t e n Hinweise liefert das K G als Mal3 der K 6 r p e r m a s s e . Ftir die B e u r t e i l u n g in der Klinik ist die A n g a b e in P r o z e n t des N o r m a l g e w i c h t e s n a c h der BrocaF o r m e l ausreichendo Die r e c h n e r i s c h u m s t ~ n d l i c h e r e n F o r m e l n n a c h Ott (73) bzw. B o r n h a r d t (in 93) ( E i n b e z i e h u n g des B r u s t u m f a n g e s ) sind ftir die klinische R o u t i n e n i c h t erforderlich. Das K G ist ein g u t e r P a r a m e t e r ffir die prim~re Definition des Ern~ihrungszustandes u n d ffir die Verlaufsbeo b a c h t u n g bei P a t i e n t e n m i t P r o t e i n - K a l o r i e n - M a n g e l e r n ~ h r u n g (PKM) o d e r M i s c h b i l d e r n (MB), solange k e i n e S t 6 r u n g des Wasser-ElektrolytH a u s h a l t e s mit De- oder H y p e r h y d r a t i o n vorliegt. Die Protein-Mangelern ~ h r u n g (PM) ist v o m K G h e r n i c h t zu erfassen.

D e r A U korreliert verl~Blich mit der K S r p e r m a s s e (12). Seine Bestimm u n g ist einfach d u r c h f f i h r b a r u n d eignet sich deshalb b e s o n d e r s fiir bettl~gerige Patienten, bei d e n e n in E r m a n g e l u n g einer Bett- o d e r Sitzw a a g e eine B e s t i m m u n g des K G n i c h t m b g l i c h ist. D e r A U gibt eine A u s s a g e fiber die M u s k e l m a s s e u n d die F e t t d e p o t s u n d ist d e s h a l b zur E r f a s s u n g der P K M u n d der MB geeignet. Bei P a t i e n t e n mit O d e m k r a n k heiten ist er aussagekr~ftiger als das KG. Seine Ver~inderungen bei anaboler o d e r k a t a b o l e r StoffwechseUage sind a u s r e i c h e n d groB ffir Verlaufsk o n t r o l l e n in e i n e m A b s t a n d v o n 2-3 W o c h e n (iibliche station~re Verweildauer). Z u r D i a g n o s e der P M tr~gt der A U n i c h t bei. Z u r E r m i t t l u n g des K6rperfettgehaltes stehen v e r s c h i e d e n e M e t h o d e n zur Verffigung, wie die B e s t i m m u n g des G e s a m t k a l i u m s m i t der B o d y c o u n t e r - M e t h o d e (14) oder die M e s s u n g des spezifischen G e w i c h t e s d u r c h Wasserverdr~ingung in e i n e m D e n s i m e t e r oder d u r c h U n t e r w a s s e r w ~ g u n g (93), die n u r u n t e r L a b o r a t o r i u m s b e d i n g u n g e n durchfi~hrbar sind. Ffir klinische U n t e r s u c h u n g e n bietet sich die Calipermetrie an. Die D i c k e der H a u t f a l t e n ist ein a n e r k a n n t e s quantitatives Kriterium ffir die B e u r t e i l u n g d e r F e t t d e p o t s (40, 50, 51, 74, 75, 87). Mehr als die Halfte des K b r p e r f e t t e s ist im U n t e r h a u t f e t t g e w e b e abgelagert. Die in der P r a x i s h a u p t s ~ c h l i c h b e n u t z e n Caliper n a c h Holtain, H a r p e n d e n , L a n g e u n d B e s t zeigen k e i n e U n t e r s c h i e d e in ihrer MeBgenauigkeit. Sie erftillen die Kriterien, die h e u t e an e i n e n Caliper gestellt w e r d e n , n~rnlich: e i n g e b a u t e D r u c k k o n t r o l l e , Parallelit~t der K o n t a k t f l ~ c h e n bei jeder Z a n g e n s t e l l u n g sowie k o n s t a n t e r F e d e r d r u c k fiir jede Offnungsweite. V o n allen H a u t f a l t e n wird die T H F bevorzugt. N a c h R e h s u n d Mitarbeitern (75) weist die T H F ffir alle Caliper die n i e d r i g s t e n Variabilit~tskoeffizienten auf. V o n v e r s c h i e d e n e n A u t o r e n w u r d e eine gute U b e r e i n s t i m m u n g y o n direkten M e s s u n g e n bei chiru r g i s c h e n O p e r a t i o n e n (29, 59) u n d s o n o g r a p h i s c h e n (11, 42, 87) u n d r a d i o l o g i s c h e n (3, 87) MeBergebnissen mit der Calipermetrie best~tigt. H e y m s f i e l d u n d Mitarbeiter (45) v e r g l i c h e n c o m p u t e r t o m o g r a p h i s c h e E r g e b n i s s e mit d e n a n t h r o p o m e t r i s c h e n Methoden. Sie stellten lest, daB bei einer THF, die kleiner als 5 m m ist, c o m p u t e r t o m o g r a p h i s c h kein F e t t m e h r n a c h w e i s b a r ist. I n einem Bereich y o n 60 bis 120 P r o z e n t des idealen K G zeigen die a n t h r o p o m e t r i s c h e n M e t h o d e n eine gute U b e r e i n s t i m m u n g mit der C o m p u t e r t o m o g r a p h i e . Die a n t h r o p o m e t r i s c h b e s t i m m t e Muskelfl~che (AMF) liegt u m etwa 15 bis 25 P r o z e n t h 6 h e r als bei der C o m p u t e r t o m o g r a p h i e . D u r c h B e r i i c k s i c h t i g u n g des K n o c h e n q u e r schnitts, der bei der a n t h r o p o m e t r i s c h e n M e t h o d e vernachl~ssigt wird, k a n n dieser Fehler w e i t g e h e n d a u s g e g l i c h e n werden. Die a n t h r o p o m e trisch b e s t i m m t e Fettfl~iche (FF) s t i m m t mit d e n c o m p u t e r t o m o g r a p h i s c h e n E r g e b n i s s e n in e i n e m Bereich y o n +_ 10 P r o z e n t fiberein. Bei e i n e m K G y o n m e h r als 120 P r o z e n t des I d e a l g e w i c h t s k o m m t es zu e r h e b l i c h e n Differenzen z w i s c h e n A n t h r o p o m e t r i e u n d C o m p u t e r t o m o g r a p h i e . Diese B e f u n d e zeigen, dab die A n t h r o p o m e t r i e zur D i a g n o s e y o n P K M u n d MB wesentlich beitragen kann. Dabei b e s t e h t die Tendenz, d a b die Muskelm a s s e mit a n t h r o p o m e t r i s c h e n M e t h o d e n eher fibersch~tzt wird, w a s teils a u e h d u r c h d e n K o m p r e s s i o n s e f f e k t des Calipers (13, 46) auf die Fettfalte zu erkliiren ist. Die Differenzierung in Fett- und Muskelanteil ist bei diesen Zust~nden nicht so bedeutungsvoll, dab sich der Aufw a n d d e r C a l i p e r m e t r i e g e n e r e l l v e r t r e t e n l~/3t. Die Kreatinin-Ausscheidung im 24-Stunden-Urin (U-Krea) ist in erster L i n i e v o n d e r Grb/3e d e r M u s k e l m a s s e a b h ~ n g i g u n d e r l a u b t d a m i t e i n e S c h ~ t z u n g d e r s e l b e n n a c h f o l g e n d e r F o r m e l : U - K r e a (in g) x 20 = M u s k e l m a s s e (in k g ) (28, 91). I n d e r k l i n i s c h e n R o u t i n e i s t e i n e 2 4 - S t u n d e n Sammelperiode manchmal nicht durchffihrbar. Hier mbchten wir das S a m m e l n d e s U r i n s f i b e r d i e N a c h t v o n 20.00 b i s 8.00 U h r e m p f e h l e n . Dieses Vorgehen ist erlaubt, da das Kreatinin relativ gleichm~Big ausgeschieden wird. H e y m s f i e l d (45) stellte fest, d a b der K I e m p f i n d l i c h e r ist als der c o m p u t e r t o m o g r a p h i s c h b e s t i m m t e M u s k e l q u e r schnitt in O b e r a r m m i t t e . N a c h u n s e r e n E r f a h r u n g e n ist der K I zur Beurteilung der M u s k e l m a s s e im R a h m e n der prim~iren E r h e b u n g des Ern~hr u n g s z u s t a n d e s u n d der V e r l a u f s b e o b a c h t u n g gleicherrnaBen gut geeignet, solange k e i n e deutliche Katabolie vorliegt. D a n n k o m m t es n ~ m l i c h d u r c h d e n e r h S h t e n M u s k e l a b b a u zu e i n e m A n s t i e g der Kreatinin-Auss c h e i d u n g (77), die bei v o r d e r g r f i n d i g e r B e t r a c h t u n g eine zu groBe Musk e l m a s s e u n d d a m i t e i n e n b e s s e r e n Ern~_hrungszustand a n n e h m e n l~/~t. D e m e n t s p r e c h e n d w a r in der v o r l i e g e n d e n Studie das V e r h a l t e n des K I in der A n a b o l i e ( G r u p p e B) einheitlicher als in der Katabolie ( G r u p p e A). Trotz seiner groBen A u s s a g e k r a f t hat der K I n o c h k e i n e n E i n g a n g in die klinische R o u t i n e g e f u n d e n . Das e x a k t e U r i n s a m m e l n steht i h m in vielen Kliniken i m m e r w i e d e r im Wege. Dies ist bedauerlich, da es fiir zuverl~ssige U b e r w a c h u n g e n z. B. des Kalium- u n d S t i c k s t o f f h a u s h a l t e s h~ufig v o n groBem N u t z e n w~ire. N e b e n d e m K I wird die 3 - M e t h y l h i s t i d i n - A u s s c h e i d u n g im U r i n a l s MaI~ ftir die M u s k e l u m s a t z r a t e a n g e s e h e n (30, 62, 98). Da das 3-Methylhistidin n a c h d e r H y d r o l y s e y o n M u s k e l p r o t e i n (Actin, Myosin) o h n e Strukt u r v e r ~ n d e r u n g e n direkt in d e n H a r n a u s g e s c h i e d e n wird, gilt es als idealer P a r a m e t e r fiir d e n Muskelumsatz. Auf die G r e n z e n seiner Aussagekraft h a b e n kfirzlich R o t h u n d Mitarbeiter (76) hingewiesen. Seine Bestirnm u n g ist n u r in Speziallaboratorien m6glich, in d e n e n A m i n o s ~ u r e a n a l y satoren zur Verffigung stehen. Die D i a g n o s e der P r o t e i n - M a n g e l e r n ~ h r u n g (PM) ist an die Bestimm u n g v o n P l a s m a p r o t e i n e n g e b u n d e n . Fiir die F r / i h d i a g n o s e eines Proteindefizits e i g n e n sich in erster Linie F u n k t i o n s p r o t e i n e mit k u r z e r biologischer H a l b w e r t s z e i t u n d h o h e r Umsatzrate. V e r s c h i e d e n e A r b e i t s g r u p p e n (35, 43, 48, 53, 55, 61) h a b e n in d e n letzten J a h r e n eine Reihe kurzlebiger F u n k t i o n s p r o t e i n e auf ihre Wertigkeit als F r f i h i n d i k a t o r e n eines Proteindefizits u n t e r s u c h t . Dabei erwiesen sich P r ~ a l b u m i n , retinolbindendes Protein, Transferrin, v e r s c h i e d e n e K o m p l e m e n t k o m p o n e n t e n u n d die Cholinesterase als relativ e m p f i n d l i c h fi]r die B e u r t e i l u n g des ProteinStickstoff-Pools. Ein frtihzeitiger K o n z e n t r a t i o n s a b f a l l dieser P l a s m a p r o teine in d e n u n t e r e n R e f e r e n z b e r e i c h w u r d e bei g e s u n d e n Freiwilligen bzw. U b e r g e w i c h t i g e n w ~ h r e n d kurzfristiger Reduktionsdifit bzw. absoluter N a h r u n g s k a r e n z b e o b a c h t e t (26, 33, 82). Ein d e u t l i c h e r Abfall findet sich in p o s t o p e r a t i v e n u n d p o s t t r a u m a t i s c h e n P h a s e n (18, 34, 43, 44, 53, 55). U m g e k e h r t k o n n t e bei P a t i e n t e n in m a n g e l h a f t e m E r n ~ h r u n g s z u s t a n d d u r c h a d e q u a t e E r n ~ h r u n g eine w e i t g e h e n d e N o r m a l i s i e r u n g d e r P l a s m a k o n z e n t r a t i o n e n dieser P r o t e i n e erzielt w e r d e n (33, 47, 48). Ein D e f e k t in der F~higkeit zur S y n t h e s e fftr diese P r o t e i n e b e s t e h t d e m n a c h nicht. A n die MSglichkeit des Vorliegens eines s o l c h e n muB aber bei der B e u r t e i l u n g e n t s p r e c h e n d e r B e f u n d e stets a u c h g e d a c h t w e r d e n . B e s o n ders e i n d e u t i g sind die K o n z e n t r a t i o n s ~ n d e r u n g e n y o n P r ~ a l b u m i n u n d r e t i n o l b i n d e n d e m Protein. Weniger zuverl~ssig reagiert Transferrin, dessen K o n z e n t r a t i o n u n t e r a n d e r e m a u c h yore S e r u m - E i s e n s p i e g e l beein-

fluftt w i r d (1, 71). Es ist d e s h a l b g e r a d e b e i M a n g e l e r n ~ h r u n g m i t g l e i c h z e i tiger Eisenmangelan~mie weniger aussagekr~ftig. Ein sehr empfindlicher P a r a m e t e r a k u t e r V e r ~ n d e r u n g e n des Proteinstatus ist auch die Choline s t e r a s e m i t e i n e r b i o l o g i s c h e n H a l b w e r t s z e i t v o n ca. e i n e m T a g (37, 55). I h r e B e s t i m m u n g e r f o l g t p h o t o m e t r i s c h u n d ist i m G e g e n s a t z z u r U n t e r suchung yon Pr~albumin, retinolbindendem Protein und Transferrin ( r a d i a l e I m m u n d i f f u s i o n ) in j e d e m k l i n i s c h e n L a b o r d u r c h f f i h r b a r . W e g e n d e r gro/3en i n d i v i d u e l l e n S t r e u u n g d e r N o r m a l w e r t e d e r C h o l i n e s t e r a s e werden bei akutem Proteinmangel bei vielen Patienten wegen ihres hohen individuellen Ausgangswertes subnormale Konzentrationen nicht e r r e i c h t , w e s h a l b h i e r d i e V e r l a u f s b e o b a c h t u n g u n e r l ~ f t l i c h ist. I n u n s e r e n U n t e r s u c h u n g e n z e i g t e sich, daft d i e F u n k t i o n s p r o t e i n e m i t k u r z e r H a l b w e r t s z e i t zu B e g i n n e i n e r K a t a b o l i e r a s c h in u n t e r e R e f e r e n z b e r e i c h e o d e r nur gering s u b n o r m a l e Bereiche abfallen, dann aber lange Zeit k o m p e n s i e r t b l e i b e n u n d e r s t b e i w e i t f o r t g e s c h r i t t e n e r M a l n u t r i t i o n w e i t e r abfallen. D i e s ist d a d u r c h zu erkl~iren, daft s i e h die K a t a b o l i e z u e r s t a m f u n k t i o n e l l e n P r o t e i n a u s w i r k t [ H a l b w e r t s z e i t e n : R e t i n o l b i n d e n d e s P r o t e i n ca. 12 S t u n d e n (47), C h o l i n e s t e r a s e ca. 24 S t u n d e n , P r ~ a l b u m i n ca. 50 S t u n d e n (85), T r a n s f e r r i n ca. 8 T a g e (88)] u n d d a n n a n d e r e Q u e l l e n , w i e z. I m w e i t e r e n V e r l a u f zeigt es d a n n z u n ~ c h s t e i n e n d e u t l i c h e r e n A b f a l l , d e r b e i l ~ n g e r e m F o r t b e stehen der Eiwei~mangelern~hrung durch Drosselung der Albuminabb a u r a t e w i e d e r v e r l a n g s a m t w i r d . D i e s e D a r s t e l l u n g v e r d e u t l i c h t , daft bereits mittelgradig erniedrigte Konzentrationen von Pr~albumin, Transferrin oder A l b u m i n einen fortgeschrittenen P r o t e i n m a n g e l anzeigen und d a m i t e i n e u n g f i n s t i g e P r o g n o s e ffir d e n P a t i e n t e n b e d e u t e n . K a m i n s k i (52) s t e l l t e b e i s e i n e m P a t i e n t e n g u t e i n e n A n s t i e g d e r M o r t a l i t i i t a u f alas 2,5fache fest, w e n n d e r S e r u m - T r a n s f e r r i n s p i e g e l u n t e r 170 m g % e r n i e d r i g t war. W u h r m a n n u n d M~rki (95) b e o b a c h t e t e n b e i i n t e r n i s t i s c h e n P a t i e n t e n e i n e d u r c h s c h n i t t l i c h e Z u n a h m e d e r Mortalit~it a u f d a s 5fache, w e n n e i n A l b u m i n s p i e g e l v o n 3 g/100 m l u n t e r s c h r i t t e n w u r d e . D i e s e Daten b e w e i s e n die b e s o n d e r e Gef~hrdung des P a t i e n t e n durch eine P r o t e i n - M a n g e l e r n ~ h r u n g (PM) u n d v e r d e u t l i c h e n a u f t e r d e m , daft auf P l a s m a p r o t e i n s p i e g e l - B e s t i m m u n g e n b e i d e r E r h e b u n g d e s Ern~ihrungszustandes nicht verzichtet werden kann. Kurzfristige Kontrollen der funktionellen Proteine mit kurzer Halbwertszeit erlauben zwar die Feststellung einer katabolen Stoffwechsellage, l a s s e n q u a n t i t a t i v e A u s s a g e n a b e r n i c h t zu. G e r a d e d i e s e D e f i n i t i o n k a n n a b e r b e i s c h w e r k r a n k e n P a t i e n t e n fiir d i e A u f s t e l l u n g e i n e s a d ~ q u a t e n E r n ~ h r u n g s r e g i m e s e r f o r d e r l i c h sein. H i e r e i n e S t i c k s t o f f b i l a n z d u r c h z u f f i h r e n , ist i m R o u t i n e b e t r i e b e i n e r K l i n i k m e i s t e n s n o c h n i c h t m S g l i c h . A l s a u s r e i c h e n d a u s s a g e k r ~ f t i g e P a r a m e t e r ffir d i e s e F r a g e s t e l l u n g w u r d e n v o n B i s t r i a n (5) d e r K a t a b o l i e - I n d e x , v o n W o o l f s o n (94) d i e H a r n s t o f f p r o d u k t i o n s r a t e in d i e D i s k u s s i o n g e b r a c h t . B e i d e K e n n g r 6 f t e n sind analytisch einfach festzustellen, erfordern allerdings eine exakte H a r n s a m m l u n g f i b e r 24 S t u n d e n (58, 76, 80). H i e r d u r c h ist d i e A n w e n d u n g d i e s e r M e t h o d e n in d e r K l i n i k auf a u s g e w ~ h l t e P a t i e n t e n b e s c h r ~ n k t .

Ffir die F e s t s t e l l u n g einer a n a b o l e n Stoffwechsellage ist die Verlaufsb e o b a c h t u n g der P l a s m a p r o t e i n e , des K I u n d des A U a u s r e i c h e n d . I n u n s e r e n U n t e r s u c h u n g e n zeigte sich in der a n a b o l e n P h a s e ein d e u t l i c h e r Anstieg der F u n k t i o n s p r o t e i n e . Dabei w a r die Z u n a h m e bei d e n P a t i e n t e n mit MB d e u t l i c h e r als bei d e n e n m i t PKM. Offenbar reagieren P a t i e n t e n im Z u s t a n d des EiweiBmangels auf eine P r o t e i n z u f u h r r a s c h e r als solche mit k o m p e n s i e r t e m E i w e i ~ h a u s h a l t (54). B e s o n d e r s einheitlich w a r das Verhalten y o n A l b u m i n u n d Pr~albumin, w ~ h r e n d Transferrin u n d Cholinesterase eine grSBere S t r e u u n g zeigten. U b e r die B e z i e h u n g e n z w i s c h e n E r n ~ h r u n g s z u s t a n d u n d i m m u n o l o g i s c h e m R e a k t i o n s v e r m S g e n liegt eine u m f a n g r e i c h e L i t e r a t u r v o r (7, 17, 20, 23, 57, 66, 83, 86). In H u n g e r z e i t e n n e h m e n lokale eitrige P r o z e s s e u n d septische A U g e m e i n i n f e k t i o n e n zu. I m D e u t s c h l a n d der N a c h k r i e g s z e i t w a r ein s p r u n g h a f t e s A n s t e i g e n der T u b e r k u l o s e zu verzeichnen. P n e u m o n i e n stellen die h~ufigste T o d e s u r s a c h e bei P a t i e n t e n mit Mangelern ~ h r u n g dar. I n der Regel ist die h u m o r a l e I m m u n a b w e h r bei Mangelern ~ r u n g k a u m beeintr~chtigt, d a g e g e n aber beide S y s t e m e d e r zellul~iren A b w e h r . N a c h Bistrian u n d Mitarbeitern (7) ist das i m m u n o l o g i s c h e Reakt i o n s v e r m S g e n bei P M w e s e n t l i c h deutlicher beeintr~chtigt als bei PKM. Der aktuelle I m m u n s t a t u s eines P a t i e n t e n k a n n a m e i n f a c h s t e n d u r c h H a u t t e s t s ~iberpr~ift werden. Das gilt i n s b e s o n d e r e a u c h for Verlaufsunt e r s u c h u n g e n (84). Bei u n s e r e n E r h e b u n g e n zeigte sich eine deutliche A b h ~ n g i g k e i t der zellul~ren I m m u n r e a k t i v i t ~ t y o n k a t a b o l e r u n d anaboler Stoffwechsellage. Dabei erwiesen sich die H a u t t e s t s als b e s o n d e r s a u s s a g e k r ~ t i g , w ~ h r e n d die L y m p h o z y t e n z a h l e n n u r in d e r Katabolie, nicht aber in der Anabolie diagnostisch v e r w e r t b a r waren. C o p e l a n d et al. (20), Meakins et al. (65) u n d Mullen et al. (68, 69) h a b e n auf die h o h e p r o g n o s t i s c h e A u s s a g e k r a f t der zellul~iren I m m u n r e a k t i v i t ~ t hingewiesen. Meakins (65) b e o b a c h t e t e in e i n e m groBen c h i r u r g i s c h e n K r a n k e n g u t bei P a t i e n t e n mit negativen Hauttests eine Mortalit~t y o n 74 % gegenfiber einer Mortalit~t y o n 5 % bei intakter I m m u n a b w e h r . C o p e l a n d (21) k o n n t e zeigen, dab bei NIalignompatienten mit gest6rter I m m u n k o m p e t e n z y o n v o r n h e r e i n mit e i n e m deutlich g e r i n g e r e n Therapieerfolg antineoplastis c h e r M a 6 n a h m e n (chirurgischer Eingriff, Zytostase, Bestrahlung) g e r e c h n e t w e r d e n m u 6 als bei intakter zellul~rer I m m u n r e a k t i v i t ~ t . N a c h d e n A r b e i t e n y o n Daly, D u d r i c k , Ota u n d C o p e l a n d (25, 72) k a n n die I m m u n l a g e des M a l i g n o m p a t i e n t e n d u r c h E r n ~ h r u n g s t h e r a p i e g e b e s s e r t w e r d e n , w o d u r c h die Toleranz g e g e n d b e r S t r a h l e n t h e r a p i e u n d C h e m o t h e r a p i e in h S h e r e n D o s e n e r h 6 h t wird u n d p o s t o p e r a t i v e K o m p l i k a t i o h e n v e r m i n d e r t werden. Beide A u t o r e n f o r d e r n deshalb bei s c h w e r k r a n k e n P a t i e n t e n die B e s t i m m u n g der zellul~ren I m m u n r e a k t i v i t ~ t im Rahm e n der A u f n a h m e u n t e r s u c h u n g , u m rechtzeitig die I n d i k a t i o n f0r eine a d e q u a t e E r n ~ h r u n g s b e h a n d l u n g zu stellen. Bei u n s e r e r G r u p p e y o n M a l i g n o m p a t i e n t e n h a n d e l t e es sich u m eine negative A u s w a h l mit infauster P r o g n o s e , die einer speziellen B e h a n d l u n g n i c h t m e h r zugefi~hrt w e r d e n k o n n t e n . Bei allen diesen P a t i e n t e n ffihrte die T u m o r k a t a b o l i e i n n e r h a l b k u r z e r Zeit zu einer d e u t l i c h e n V e r s c h l e c h t e r u n g v o n Ern~hrungs- u n d I m m u n s t a t u s . A b s c h l i e B e n d m 6 c h t e n wir n o c h e i n m a l d a r a u f hinweisen, d a b h e u t e allgemein die M6glichkeiten der E r n ~ h r u n g s b e h a n d l u n g n o c h n i c h t aus-

B e i u n s e r e n U n t e r s u c h u n g e n s t e l l t e n s i c h K G u n d A U al s a u s r e i c h e n d f/ir d i e D i a g n o s t i k d e r P K M h e r a u s . D i e k l i n i s c h e R e l e v a n z d i e s e r F o r m d e r M a n g e l e r n ~ h r u n g l~Bt s i c h a m b e s t e n m i t H i l f e d e r z e l l u l ~ r e n I m m u n r e a k t i v i t ~ t absch~itzen. V i e l w i c h t i g e r i st d i e E r k e n n u n g d e r P M und der MB, die nur durch die zus~tzliche Bestimmung von Plasmaprotei n e n g e l i n g t . F / i r d i e E r k e n n u n g l a t e n t e r P r o t e i n d e f i z i t e i st d i e B e s t i m m u n g k u r z l e b i g e r P l a s m a p r o t e i n e , w i e z. B. d e s P r ~ a l b u m i n s , u n e r l ~ B l i c h . Weiter fortgeschrittene Zust~nde von PM werden durch das Seruma l b u m i n a u s r e i c h e n d c h a r a k t e r i s i e r t . Zusammenfassung In der vorliegenden Arbeit wird die Aussagekraft anthropometriseher, laborchemiseher und i m m u n o l o g i s c h e r KenngrbBen fiir die Ersterhebung des Ern/ihrungszustandes und die Verlaufsbeobachtung bewertet. Hierffir werden die Ergebnisse eigener E r h e b u n g e n bei abstinenten Alkoholikern u n t er intensiver Ern~hrungsrehabilitation fiber drei Wochen u n d kataboler Malignompatienten mit infauster Prognose im Z u s a m m e n h a n g m i t der vorliegenden Literatur diskutiert. U n ser Ziel ist es, einfache und m6gliehst gewichtige MeBgr6Ben und A n g ab en in einem Bewertungsprofil zusammenzufassen, das aussagekr~ftig u n d praktikabel ist.
Summary In the present paper the significance of anthropometric, laboratory-chemical and i m m u n o l o g i cal indices for the initial assessment of the nutritional status and the observation of its d e v e l o p m e n t is evaluated. For this purpose the results of personal investigations in abstinent alcoholics u n d e r intensive nutritional rehabilitation during three weeks and of catabolic malignoma patients with infaust prognosis are discussed in connection with the available reference literature. It is our aim to c o m b i n e simple and highly significant parameters and data in an informative and practicable assessment profile.
